INTRODUCTION: Hereditary neuropathy with liability to pressure palsy (HNPP) is an autosomal dominant disorder, typically presenting with recurrent episodes of mononeuropathy in nerves susceptible to compression, with similar neurophysiological characteristics. However, other clinical and neurophysiological presentations have been reported.
METHODS: We retrospectively analysed the clinical and neurophysiological characteristics of 20 patients with genetically confirmed HNPP. Sixteen patients were studied in our department between 1996 and 2016.
RESULTS: In addition to the typical characterist

OBJECTIVES/HYPOTHESIS: Bell's palsy is highly associated with diabetes mellitus (DM). Either the reactivation of herpes simplex virus type 1 (HSV-1) or diabetic mononeuropathy has been proposed to cause the facial paralysis observed in DM patients. However, distinguishing whether the facial palsy is caused by herpetic neuritis or diabetic mononeuropathy is difficult. We previously reported that facial paralysis was aggravated in DM mice after HSV-1 inoculation of the murine auricle. In the current study, we induced HSV-1 reactivation by an auricular scratch following DM induction with streptoz

BACKGROUND: Cranial mononeuropathy is one of the not so common forms of diabetic neuropathy that often appears to be a serious problem from a diagnostic and therapeutic point of view.
AIM: Objective of this study was to determine the incidence, the clinical characteristics, and risk factors associated with cranial nerve palsies among persons with diabetes.
METHODS: We have performed a retrospective study of all diabetic patients with cranial nerve palsies who were hospitalized in a metabolic department over a 12-yr period.
RESULTS: During the period of the survey, a total of 8150 diabetic s

The effect of lidocaine pretreatment on thermal hyperalgesia and thermal skin asymmetries provoked by experimental mononeuropathy was investigated in rats. Forty anesthetized rats were given sciatic nerve ligatures according to the technique of Bennett and Xie. Rats were divided into 3 groups: 16 were ligated without lidocaine, 16 were ligated after lidocaine bathing of the nerve, and 8 were ligated after systemic lidocaine (6-8 mg/kg). Six sham-operated rats for each group were also prepared. From the first postoperative day the responses to the hot-plate test were assessed daily for 4 weeks

Proximal tibial neuropathy is an uncommon focal mononeuropathy that is most often caused by trauma, ischemia, or neoplastic infiltration or compression of the tibial nerve. We report a patient who presented with a tibial neuropathy following a leg injury, which initially mimicked a lumbosacral radiculopathy but which was the result of a proximal tibial neuropathy. Electrophysiologic studies confirmed a proximal tibial neuropathy and MRI revealed a popliteus muscle hemorrhage with mass effect on the tibial nerve. Following conservative management the patient had little recovery of function afte

Vasculitic neuropathy is an important cause of peripheral neuropathy because of its potentially severe prognosis and an early recommended therapy. Diagnosis is easy when the clinical presentation is an acute, painful, multiple mononeuropathy but more difficult when presenting as a polyneuropathy. Electrophysiologic studies play an important role in the diagnosis. Muscle and nerve biopsies confirm the vasculitic process and demonstrate ischemic neuropathy lesions. Polyarteritis nodosa, rheumatoid arthritis and non-systematic vasculitic neuropathy are the main conditions of vasculitic neuropathy

Thirty-two patients with a mononeuropathy multiplex associated with a systemic necrotizing vasculitis were studied. The main abnormality was a loss of motor and sensory axons confirmed by electrophysiological and histological methods. A conduction block was observed in five patients, but only one was at a usual site of compression. Based on previous pathological studies and the experimental data in human and animals, the mechanism of the block is proposed to be ischemic. It is suggested that a conduction block in one nerve in a neuropathy with two or more individual nerves affected and with el

BACKGROUND AND OBJECTIVES: Neurolymphomatosis (NL) is characterized by lymphomatous infiltration of the peripheral nervous system presenting … nfirmed different neuropathy patterns were observed, but patients with SNL had increased numbers of mononeuropathies (n = 8) compared with PNL (n = 1, p = 0.01). MRI studies detected NL more frequently (86%) compared with fluorodeoxyglucose (FDG)-PET CT imaging studies (60%) (p = 0.007). Nerve biopsies revealed B-cell lymphoma (PNL n = 32, SNL n = 22), followed by …

This report discusses a patient in his late 60s with a diagnosis of eosinophilic granulomatosis with polyangiitis (EGPA) who was found to have radiological periaortitis. He presented with acute mononeuritis multiplex and constitutional symptoms. Investigations revealed marked eosinophilia, positive myeloperoxidase (MPO) antibodies and imaging evidence of periaortitis. Nerve conduction studies showed axonal mononeuropathies, and a sural nerve biopsy confirmed a vasculitic neuropathy with eosinophilic infiltrates. He was treated with high-dose corticosteroids and intravenous cyclophosphamide, wi…

BACKGROUND: Dengue infection may cause neurological manifestations such as encephalitis, myelitis, mononeuropathies, acute disseminated encephalomyelitis, and Guillain Barré syndrome (GBS). In endemic regions, the infection course can be oligosymptomatic making difficult the diagnosis of the neurological picture associated with dengue infection.
OBJECTIVE: To report dengue infection and GBS association, even in oligosymptomatic cases of this infection.
METHOD: During the dengue epidemic in Rio de Janeiro city we looked for GBS cases, testing IgM antibodies for dengue and dengue polymerase ch…

The posterior femoral cutaneous nerve (PFCN) has been widely studied with context to its origin, distribution and mononeuropathies. Due to the vulnerability of the gluteal region to iatrogenic injuries and pressure ulcers etc., an understanding of the cutaneous innervation of the gluteal region holds immense relevance. The communication between the PFCN and the inferior gluteal nerve (IGN) was observed at the back of thigh of a 56-year-old male cadaver while exploring distribution of nerves in gluteal region during routine anatomical dissection. The above communication holds importance in nume…

Hereditary neuropathy with liability to pressure palsy (HNPP) is characterized by acute, painless and recurrent mononeuropathies. Genetic testing shows PMP22 gene deletion of chromosome 17p11.2 can provide evidence for the diagnosis of HNPP. Reports on tibial neuropathy as the main manifestation of HNPP are very rare. We report a 14-year-old girl who was admitted to our hospital due to plantar foot numbness and plantar flexion weakness of her left foot. The patient had a history of lateral dorsal numbness and right foot drop when she was 3 years old. Clinical symptoms, and neurological examina…

Meralgia Paresthetica (MP) is one of the most common mononeuropathies of the lower limb. MP usually resolves on its own, even without treatment. However, many physicians are not aware of this diagnosis and may confuse patients with another nerve disease such as radiculopathies. Although no motor symptoms are associated with this condition, the sensory dysfunctions are potentially debilitating to patients. The variable course of the lateral femoral cutaneous nerve also complicates treatments. Thus, the author recommends the use of ultrasonography to help locate the nerve. Many treatments for MP…

Sciatic nerve injury is one of the frequent mononeuropathies in children that occurs due to different causes such as nerve compression, trauma and stretch during surgery. Gluteal injection is an uncommon cause of sciatic injury in developed countries. Poor techniques and frequent injections are the common cause of injection palsy. Proneal division of the sciatic nerve is more prone to injury due to anatomic and structural characteristics. The diagnosis is based on electrophysiological studies and the recovery rate is poor. In this study, in a period of 2 years between 2012 and 2013, we report …

To study pathogenetic features of chronic joint pain, we examined 183 patients with rheumatoid arthritis (RA) and 80 patients with osteoarthrosis (OA). The presence of mixed pain syndrome was found. A neuropathic component of pain (NCP) was observed in some patients with nociceptive pain (43 and 30% with RA and OA, respectively). Patients with RA had tunnel syndromes (14%), polyneuropathies (55%), mononeuropathies (19%) and sensitive specific disorders, characteristic of NCP, which were localized in anatomic zones corresponding to affected nerves. No signs of the damage of the somatosensory ne…

Isolated cranial mononeuropathies in patients over 50 years of age most commonly result from microvascular ischemic demyelination. A 51-year-old female without vasculopathic risk factors presented with isolated sixth nerve palsy. Magnetic resonance imaging (MRI) of the brain and orbits revealed a cavernous sinus lesion that was diagnosed as a meningioma. Laboratory tests showed abnormal liver function, and an abdominal computerized tomogram demonstrated an obstructive colonic mass with liver metastasis. The pathology tests of specimens taken during a laparotomy diagnosed colon adenocarcinoma. …

Shoulder pain is a common complication in poststroke hemiplegia that reduces functional recovery. Many types of shoulder pathology have been suggested as causes of shoulder pain in hemiplegia, including shoulder subluxation, capsulitis, tendinitis, rotator cuff injury, bursitis, impingement syndrome, spasticity, complex regional pain syndrome, brachial plexus injury, and proximal mononeuropathies. More than one type of pathology may exist in a given patient. Shoulder pain improves in many cases with prompt diagnosis and appropriate management. Although the relationship between subluxation and p…

Autonomic and sensory nerve dysfunction are progressive forms of diabetic neuropathies, often accompanied by other microvascular complications which are usually characterized by gradual onset of symptoms. Mononeuropathies, radiculopathies, motor palsies and acute painful neuropathies are more reversible forms of neuropathies. Their evolution is often unrelated to glycaemic control, vascular factors are more important in the pathogenesis and these complications are more common in middle aged and elderly men with type 2. diabetes mellitus. The diagnosis of neuropathies is based on following crit…
