Mortality from early neonatal group B streptococcal sepsis: influence of obstetric factors.

Our objective was to determine if the neonatal mortality from early group B streptococcal (GBS) septicemia was associated with obstetric factors other than birthweight. Medical records from our institution for all neonates with positive blood cultures for GBS in the first 7 days of life between January 1981 and December 1992 were reviewed (n = 61). All the neonates had received broad-spectrum intravenous antibiotics within 3 h of birth, and all had cerebrospinal fluid (CSF) cultures obtained. In a multivariate model we found a significant association between neonatal mortality and birthweight

Significance of placental findings in early-onset group B streptococcal neonatal sepsis.

Assessment of placental pathology and its relationship to historical data, initial laboratory parameters, and outcome was undertaken in 22 cases of early-onset group B streptococcal sepsis of the neonate. Fourteen (64%) of the placentas demonstrated chorioamnionitis, six (27%) funisitis, and in nine (41%) gram stain demonstrated organisms within the membranes. Focal villous edema was observed in five (23%) cases and diffuse villous edema in four (18%). No placenta demonstrated chorangiosis. Placental inflammation was significantly (p less than 0.05) associated with prematurity, prolonged ruptu

Machine learning identifies neutrophil-related signatures for diagnostic value in neonatal sepsis.

Neonatal sepsis (NS) is one of the leading causes of neonatal mortality. The nonspecific clinical manifestations and the limited timeliness of existing biomarkers (such as C-reactive protein) highlight the urgent need for highly accurate diagnostic tools. Neutrophils, as key effector cells of innate immunity, are closely involved in the progression of NS. This study integrated training (GSE69686) and validation (GSE25504) datasets from the GEO database. Neutrophil infiltration characteristics were analyzed utilizing CIBERSORT, and weighted gene co-expression network analysis (WGCNA) was introd…

A single-cell transcriptomic atlas reveals the immune panorama of neonatal sepsis.

Neonatal sepsis remains a leading cause of infant mortality, yet mechanisms driving concurrent hyperinflammation and immunosuppression remain unclear. Here, we perform single-cell RNA sequencing on 26 blood samples from 18 neonates, spanning acute sepsis, convalescence, and healthy controls. We identify 57 cell subtypes, revealing acute lymphoid depletion and myeloid expansion. S100A8+ myeloid-derived suppressor cell-like (MDSC-like) cells represent a putative cytokine-storm source, potentially amplified by a feedforward S100-TLR4-MYD88 circuit. Innate-like lymphocytes fail to expand, succumbi…

FXR protects against neonatal sepsis by enhancing the immunosuppressive function of MDSCs.

Myeloid-derived suppressor cells (MDSCs) play a protective role against neonatal inflammation during the early postnatal period. However, the mechanisms regulating neonatal MDSC function remain to be fully elucidated. In this study, we report that the bile acid receptor farnesoid X receptor (FXR) acts as a positive regulator of neonatal MDSC function. The FDA-approved FXR agonist obeticholic acid (OCA) protects against neonatal sepsis in an FXR-dependent manner. Genetic deficiency of FXR impairs the immunosuppressive and antibacterial functions of MDSCs, thereby exacerbating the severity of ne…

The predictive value of maternal inflammation markers for neonatal early-onset sepsis.

PURPOSE: Maternal infection is a known risk factor for neonatal early-onset sepsis (EOS). However, the relationship between maternal inflammatory makers near delivery and neonatal EOS remains unclear. This study aimed to evaluate these associations and explore whether maternal blood parameters could contribute to EOS risk assessment strategies. METHODS: In this retrospective multicenter study in Hong Kong, we included mother‑neonate pairs where the mother underwent a complete blood count (CBC) or C-reactive protein (CRP) test between 48 h before and 72 h after delivery from January 1, 2006 to

Role of high-mobility group box 1 in late onset neonatal sepsis.

Neonatal sepsis is an important cause of morbidity and mortality. High mobility group box1 protein (HMGB1) is a cytokine that can mediate inflammation. The aim of this research was to investigate the role of HMGB1 in diagnosis and prognosis of late onset neonatal sepsis. This observational case-control study included 80 newborn infants ≥37 weeks of gestation. Newborn infants were assigned into two groups: the late-onset neonatal septic group included 40 infant cases, and the control group included 40 newborn infants. Clinical sepsis score, hematological sepsis score and serum level of C-reacti…

Neonatal sepsis management in Africa: A rapid systematic review and meta-analysis.

Neonatal sepsis is a leading cause of morbidity and mortality in Africa. This study aimed to examine neonatal sepsis treatment guidelines in Africa, compare them with WHO recommendations, identify similarities and deviations, and explore the impact of antimicrobial resistance and implementation challenges. A rapid systematic review was conducted following PRISMA-ScR guidelines. Five databases (Science Direct, PubMed, CINAHL, MEDLINE via Ovid, and Scopus) were systematically searched for studies published between 2014 and 2024 that reported national or regional guidelines on neonatal sepsis tre…

The potential role of heparin-binding protein in neonatal sepsis: research progress.

Neonatal sepsis is a major global health challenge, leading to significant morbidity and mortality in newborns. The search for precise biomarkers for its early prediction in clinical settings has been ongoing, with heparin-binding protein (HBP) emerging as a promising candidate. Originating from granules in neutrophils, HBP is released into the bloodstream in response to infection and plays a pivotal role in the body's inflammatory response. Its significance extends beyond its inflammatory origins; research indicates dynamic changes in HBP levels are strongly linked to reduce in-hospital morta…

Early onset neonatal sepsis and its associatited factors: a cross sectional study.

BACKGROUND: Sepsis is the 3rd leading cause of neonatal mortality in Ethiopia contributing to 16% of neonatal death. In a hospital study, neonatal sepsis was the leading diagnosis at admission and the second leading cause of neonatal death at the neonatal intensive care unit. Among other factors repeated vaginal examination during labor is known to contribute to sepsis in low-income settings. However, there is limited evidence in the Ethiopian setting. OBJECTIVE: The objective of this study was to examine the association between early-onset neonatal sepsis and repeated vaginal examinations. …

Neonatal sepsis with meningitis, ventriculitis and brain abscess caused by Edwardsiella tarda.

A case of neonatal sepsis caused by Edwardsiella tarda, an uncommon pathogen typically associated with aquatic lifeforms, is described. The infant presented in septic shock with seizures and respiratory failure and was found to have meningitis, ventriculitis and a brain abscess requiring drainage. Only a small number of case reports of neonatal E. tarda infection, several with sepsis with poor auditory or neurodevelopmental outcomes or meningitis, have been described in the literature. This case report suggests that E. tarda, while uncommon, can be a cause of serious central nervous system dis…
