The role of nuclear medicine in the management of thymomas.

AIM OF THE STUDY: To evaluate the role of nuclear medicine techniques in the management of patients with thymoma. MATERIALS AND METHODS: The Authors performed a review of the literature about the usefulness of traditional nuclear medicine and positron emission tomography in patients with thymic tumours. RESULTS AND DISCUSSION: Nuclear medicine imaging can provide additional information in cases of thymoma such as in differential diagnosis between thymic tumours, staging and restaging. Thallium-201 (201Tl), 99mTc-sestamibi and 99mTc-tetrofosmin are useful tumour-imaging agent. Another radioph

[Carcinosarcoma of the esophagus].

Carcinosarcoma of the esophagus is a rare neoplasm of the esophagus, composes both of carcinomatous and sarcomatous elements. From Oct. 1982 to Sept. 1991, there were 516 neoplasms of the esophagus, 2 (0.39%) carcinosarcoma, treated at our hospital. The first patient was a 62-year-old man, pathological stage III (T3N2M0). The second patient was a 68-year-old man, pathological stage I (T1N0M0). The tumors were polypoid and located at the middle portion of the esophagus. They had received subtotal esophagectomy, mediastinal lymph node dissection and cervical esophagogastrostomy, retrosternally.

[Contemporary management of neuroendocrine neoplasms of the female genital organs].

Neuroendocrine neoplasms are a rare and heterogeneous group of diseases that account for only 2% of all gynecologic malignancies. The most common types are ovarian carcinoid tumor and small cell neuroendocrine carcinoma of the cervix. The tumors are staged according to FIGO clinical staging system. The diagnosis is usually made retrospectively after obtaining the results of histopathological evaluation of the primary tumor They rarely cause syndromes related to hormone overexpression. Neuroendocrine neoplasms are characterized by aggressive behaviour Even at an early stage there is high incide

Incidentally Detected Low-Grade Appendiceal Mucinous Neoplasm Following Laparoscopic Appendectomy for Acute Appendicitis.

Appendiceal mucinous neoplasms are rare entities that account for a small proportion of all gastrointestinal malignancies and are most often discovered incidentally on histopathological examination of appendectomy specimens. We present the case of a 36-year-old woman who presented to the emergency department with acute right lower quadrant abdominal pain and was found to have acute appendicitis on computed tomography imaging. She underwent an uncomplicated laparoscopic appendectomy, and final pathology revealed a low-grade appendiceal mucinous neoplasm (LAMN) confined to the appendix, staged a

High-Grade Mixed Adenoneuroendocrine Carcinoma in the Cecum: A Case Report.

Gastrointestinal neoplasms with an exocrine and a neuroendocrine component are rare. Such neoplasms are called "mixed adenoneuroendocrine carcinomas" (MANECs) according to the most recent World Health Organization classification of gastrointestinal tract neoplasms. MANECs have no specific findings that distinguish them from pure adenocarcinomas. In addition, the optimal management strategy of MANECs is largely unknown. We describe the case of a 32-year-old man with dizziness and abdominal bloating. A cecal mass was suspected based on an image study done at a local clinic. We evaluated the ceca

Periampullary Carcinoma: Unusual Sites of Metastasis.

BACKGROUND: Periampullary neoplasms include carcinomas of the duodenum, ampulla of Vater, distal common bile duct, and pancreas. The aggressive course of these neoplasms is attributable to the delay in diagnosis, as patients have no symptoms until advanced stages of the disease. More than half of patients have distant metastasis at the time of diagnosis. The most frequent sites of metastasis are the liver, lymph nodes, peritoneum, lung, bone, kidney, and, rarely the skin. CASE REPORT: We report the case of a 45-year-old female patient with adenocarcinoma of the ampulla of Vater metastasizing

When a haematoma is not a haematoma.

Cutaneous angiosarcoma (cAS) is a rare but highly aggressive vascular malignancy, often associated with chronic lymphoedema or previous radiotherapy. Its nonspecific clinical presentation can mimic benign conditions, leading to diagnostic delays. We report the case of a 70-year-old woman with chronic lymphoedema of the right arm following mastectomy and lymphadenectomy for breast cancer. She presented with a slowly enlarging haematoma-like lesion. Magnetic resonance imaging (MRI) performed externally suggested a benign haematoma. Due to progressive changes, the patient was referred to our depa

Tumor-intrinsic KLRC1 exerts tumor-suppressive functions in colorectal cancer.

Colon adenocarcinoma (COAD) represents a prevalent malignant neoplasm within the gastrointestinal tract characterized by significant morbidity and mortality rates, and the majority of patients are in intermediate to advanced stages at the time of diagnosis. Killer cell lectin-like receptor C1 gene (KLRC1) is an inhibitory receptor in the NKG2 receptor family that can be recognized by HLA-E, thereby inhibiting cytotoxic activity and cytokine secretion from NK cells. However, little is known about its function in various cancers, including COAD. In the present study, we revealed that reduced exp

Current Status of Treatment among Patients with Appendiceal Tumors-Old Challenges and New Solutions?

The 5th edition of the World Health Organization (WHO) classification of tumors of the digestive system distinguishes four categories of appendiceal tumors (ATs): serrated lesions and polyps, mucinous neoplasms, adenocarcinomas, and neuroendocrine neoplasms (NENs). The differential diagnosis of ATs can be challenging in medical practice, due to their rarity and lack of data from randomized controlled trials on a large, diverse group of patients. ATs are usually noted in specimens obtained during appendectomies due to clinical acute appendicitis. In the European population, most ATs (65%) occur

Primary gastric epithelioid angiosarcoma: A case report of diagnostic biopsy pitfall.

RATIONALE: Angiosarcoma is an aggressive neoplasm derived from endothelial cells that may develop anywhere within the body. Here, we report a case of primary gastric epithelioid angiosarcoma initially misdiagnosed as poorly differentiated adenocarcinoma from the preoperative biopsy. PATIENT CONCERNS: The patient was a 60-year-old male admitted to the hospital due to abdominal discomfort. The gastroscopy examination suggested advanced gastric cardia carcinoma. Subsequent biopsy pathology examination confirmed the diagnosis of poorly differentiated adenocarcinoma. Therefore, the patient underwe

Primary splenic angiosarcoma: a rare entity often associated with rupture and hemoperitoneum.

Primary splenic angiosarcoma (PSA) is a rare neoplasm of vascular origin associated with aggressive behavior and poor prognosis. The clinical presentation is usually non-specific and is mostly characterized by a wasting disease with anemia and splenomegaly, mimicking a wide range of entities. The authors present the case of an 80-year-old woman with cardiovascular comorbidities with a 6-month history of weight loss, fatigue, weakness, pallor, and abdominal pain. The physical examination showed massive splenomegaly and pallor. After a thorough evaluation that ruled out lymphoproliferative disea

Clinical and biologic prognostic factors in malignant pleural mesothelioma.

Malignant pleural mesothelioma is an extremely aggressive neoplasm of the pleura mainly attributable to asbestos exposure. Conventional medical, physical, and surgical treatments and their combinations are basically ineffective and just a few subjects experience some benefit. No definite guidelines can be provided in patient selection and therapeutic strategies. Currently, malignant pleural mesothelioma therapy is guided by clinical stage and patient characteristics, which are quite unreliable, rather than by the histological or molecular features of the tumor. In the present review the impact

Incidental chronic lymphocytic leukaemia in a biopsy of Merkel cell carcinoma.

Chronic lymphocytic leukaemia (CLL) shows cutaneous involvement in 2% of cases. Merkel cell carcinoma (MCC) is a rare primary cutaneous epithelial neoplasm most commonly found in sun-exposed sites in elderly male Caucasians. A 66-year-old man presented with a 2-month history of a purple painless 2 cm tumor on the scalp. Excision biopsy revealed an incompletely excised MCC, and at the periphery of the MCC, a lymphocytic infiltrate interpreted as reactive. A re-excision biopsy showed residual MCC as well as dense aggregates of small lymphocytes within and surrounding the MCC. Immunohistochemistr

6-[F-18]fluoro-L-DOPA positron emission tomography in the imaging of Merkel cell carcinoma: preliminary report of three cases with 2-deoxy-2-[F-18]fluoro-D-glucose positron emission tomography or pentetreotide-(111In) SPECT data.

PURPOSE: Merkel cell carcinoma (MCC) is an uncommon and aggressive cutaneous neoplasm of neuroendocrine origin. Somatostatin receptor scintigraphy (SRS) and positron emission tomography (PET) using 2-deoxy-2-[F-18]fluoro-D-glucose (FDG) have been proposed to stage MCC and to detect early recurrences. As 6-[F-18]fluoro-L-DOPA (FDOPA) is taken up by other neuroendocrine tumors, we speculated that FDOPA-PET could image MCC. PROCEDURE: FDOPA-PET was performed together with FDG-PET (three patients) and SRS (two patients) in different clinical settings: localization of the primary tumor, staging, a

Spectrum of thymic uptake at 18F-FDG PET.

Fluorine-18 fluorodeoxyglucose (FDG) positron emission tomography (PET) has emerged as a strong diagnostic tool for the diagnosis and staging of neoplasms. Uptake in the thymus at (18)F-FDG PET complicates the assessment of mediastinal involvement by tumor in children and young adults. Increased thymic (18)F-FDG uptake may represent normal physiologic uptake but may also indicate the presence of thymic hyperplasia, lymphomatous infiltration, primary thymic neoplasm, or metastatic disease. Familiarity with the patterns of (18)F-FDG uptake that characterize these pathologic conditions is crucial

Expression of nm23-H1 in transitional cell carcinoma of the upper urinary tract.

Transitional cell carcinoma of the upper urinary tract is an uncommon neoplasm. Relatively little information is available regarding the clinical relevance of molecular markers. This study was performed to examine the importance of nm23-H1 gene expression (NM23-H1) in this type of tumors. Immunohistochemical expression of NM23-H1 was analyzed in 90 cases of upper urinary tract cancer, and was compared for its prognostic significance with conventional biological indicators. High expression of NM23-H1 was found in 7 cases (8%), intermediate expression in 32 cases (36%), and low expression in 51

Juvenile nasopharyngeal angiofibroma: an update of therapeutic management.

BACKGROUND: Juvenile nasopharyngeal angiofibroma (JNA) is a benign tumor of the nasopharynx composed of fibrous connective tissue and an abundance of endothelium-lined vascular spaces. The name of the disease is derived from the fact that it occurs during adolescence, almost exclusively in boys. METHODS: This study examines the natural characteristics of JNA, the imaging techniques used to diagnose and stage the neoplasm, and the treatment approaches used to manage the disease. RESULTS: Forty-three cases of JNA diagnosed and treated at the University of Texas M. D. Anderson Cancer Center ove

Management of the adnexal mass.

The methods for preoperative diagnosis and evaluation of the woman with a suspected ovarian neoplasm have evolved significantly with little or no impact on the surgical treatment, which remains removal of the abnormal ovary. Although most adnexal masses are benign, the primary goal of the diagnostic evaluation is the exclusion of malignancy. A complete history, physical examination, and ultrasonic imaging are vital in evaluating a suspected ovarian mass. In postmenopausal women, serum CA-125 determinations further improve sensitivity and specificity. Once the presence of an ovarian mass is est
