Iatrogenic peripheral nerve injuries can result from numerous medical procedures, particularly transection, stretching, compression, injections, heat, radiation, and the use of anticoagulant agents. Late diagnosis may lead to atrophy of the motor endplate and result in poor outcomes. We report a case in which the posterior tibial nerve was accidentally sectioned as the posterior tibial tendon for transfer to the anterior tibial tendon in the reconstruction of drop foot. This iatrogenic complication ultimately required foot amputation. Physicians must be aware of the anatomy of the posterior ti

OBJECT: The objective of this study was to report the results of elbow, thumb, and finger extension reconstruction via nerve transfer in midcervical spinal cord injuries.
METHODS: Thirteen upper limbs from 7 patients with tetraplegia, with an average age of 26 years, were operated on an average of 7 months after a spinal cord injury. The posterior division of the axillary nerve was used to reinnervate the triceps long and upper medial head motor branches in 9 upper limbs. Both the posterior division and the branch to the middle deltoid were used in 2 upper limbs, and the anterior division of …

BACKGROUND: Ipsilateral C7 nerve transfer is an available procedure in C5C6 2-root avulsion injury of the brachial plexus. However, concomitant injury of a normal-looking C7 cannot be ruled out. The efficiency of a concomitant injury of C7 transfer was investigated.
METHODS: Forty-two Sprague-Dawley rats were randomly assigned to 5 groups. They all underwent a 2-stage procedure. In the first stage from dorsal spine approach, left C5 and C6 roots were avulsed and C7 was crushed with jeweler's forceps with different degrees: group A (n = 6), C7 not injured; group B (n = 10), C7 crushed for 10 s…

PURPOSE: The standard palmaris longus (PL)-to-rerouted extensor pollicis longus (EPL) transfer was modified by taking the PL with an extension of the palmar aponeurosis (PA) and performing the transfer at the level of the thumb metacarpal. Our purpose was to evaluate whether this modified transfer could restore both the extension and the radial abduction of the thumb without rerouting the EPL.
METHODS: We restored thumb function of 5 patients with unrecovered radial nerve palsy (4 men and 1 women; mean age at surgery, 47 years; mean duration between onset of palsy and surgery, 13 months; and

Shoulder stabilization is of utmost importance in upper extremity reanimation following paralysis from devastating injuries. Although secondary procedures such as tendon and muscle transfers have been used, they never achieve a functional recovery comparable to that following successful reinnervation of the supraspinatus, deltoid, teres minor, and infraspinatus muscles. Early restoration of suprascapular and axillary nerve function through timely brachial plexus reconstruction offers a good opportunity to restore shoulder-joint stability, adequate shoulder abduction, and external rotation func

Three patients, a woman aged 33 years, a man aged 42 and a woman aged 66, had a loss of hand function. This was due to denervation of a peripheral nerve, in essence the radial, ulnar and median nerves respectively. In the first two cases this was caused by trauma and in the third by compression neuropathy. The problems were loss of wrist and finger extension (a dropping hand) in the first, loss of intrinsic muscle function and thumb adduction in the second and loss of opposition in the third patient. Tendon transfers were performed by diverting a tendon insertion of a still functional muscle t

In vivo visualization of the neuromuscular junction with epifluorescence imaging techniques has become a successful method of observing the … post-synaptic staining indicate a non-reoccupation rate up to 37 percent at a 55-day interval after nerve transfer, and up to 34 percent at a 66-day postoperative interval. Morphologic findings suggest that these high non-reoccupation rates are caused jointly by intramuscular missprouting, an insufficient intramuscular guidance apparatus, and intramuscular micro …

Magnetization transfer (MT) imaging has been successfully applied to patients with multiple sclerosis (MS), showing lesion heterogeneity, subtle changes in the normal-appearing white matter, and a better correlation with disability, in comparison with conventional magnetic resonance imaging. MT imaging is a fairly simple technique, which allows a quantitative analysis with high spatial resolution to delicate structures like the optic nerve and spinal cord. In the spinal cord, MT imaging can be applied as a contrast augmentation technique. Using the MT ratio (MTR), two studies have reported a m

PURPOSE: Nerve transfers to restore or augment function after spinal cord injury is an expanding field. There is a paucity of information, however, on the use of nerve transfers for patients having undergone spine surgery. The incidence of neurologic deficit after spine surgery is rare but extremely debilitating. The purpose of this study was to describe the functional benefit after upper extremity nerve transfers in the setting of nerve injury after cervical spine surgery.
METHODS: A single-center retrospective review of all patients who underwent nerve transfers after cervical spine surgery…

We compared the outcomes of 23 partial ulnar nerve and 15 intercostal nerve transfers for elbow flexion reconstruction in patients with C56 or C567 brachial plexus injuries using manual muscle power, dynamometric measurements of elbow flexion strength and electromyography. The range of elbow flexion and muscle strength recovery to Grade 3 or 4 were comparable between the two groups. The patients with C567 injuries had significantly stronger eccentric contraction after the partial ulnar nerve transfer than after the intercostal nerve transfer (p < 0.05). Electromyography of individual muscles d…

PURPOSE: Single and double fascicular nerve transfer using the ulnar or median nerve is performed to restore elbow flexion following injuries to the brachial plexus or nerve root. However, little is known regarding the postoperative changes involved in the sensory alteration of the hand after a single and double fascicular nerve transfer. We evaluated the sensory alteration of the hand in patients who underwent single and double fascicular nerve transfer for two years.
METHODS: A total of five patients that underwent single or double fascicular nerve transfer participated in this study. The i…

Loss of hand function after cervical spinal cord injury (SCI) impacts heavily on independence. Multiple nerve transfer surgery has been applied successfully after cervical SCI to restore critical arm and hand functions, and the outcome depends on nerve integrity. Nerve integrity is assessed indirectly using muscle strength testing and intramuscular electromyography, but these measures cannot show the manifestation that SCI has on the peripheral nerves. We directly assessed the morphology of nerves biopsied at the time of surgery, from three patients within 18 months post injury. Our objective …

PURPOSE: We report on the results we obtained with reconstruction for total paralysis of the brachial plexus using long nerve grafts that connect nonavulsed roots to the musculocutaneous and radial nerve. Nerve transfers were performed to restore function of the suprascapular nerve, triceps long head, and pectoralis major muscle.
METHODS: We studied 22 young adults with complete brachial plexus palsy who had surgical repair an average of 5 months after trauma. Nerve grafts connected the C5 root to the musculocutaneous nerve. The C6 root was connected by grafts to the radial nerve. When the C6…

BACKGROUND: In skeletally immature individuals, vascularized epiphyseal transfer allows for reconstruction of complex bony defects while preserving longitudinal growth and providing a reliable articular surface for effective joint function. The shape and size of the proximal fibular epiphysis allow for gradual remodeling of the articular surface, facilitating favorable reconstruction and rehabilitation over time. Moreover, the proximal fibula is highly expandable, with less weight-bearing function, contributing to low donor-site morbidity. However, peroneal nerve dysfunction-related foot drop
