Of 42 patients undergoing a microvascular decompression for trigeminal neuralgia with a follow-up of 4 to 7 years, 32 (76.19%) had no postoperative recurrence of neuralgic pain, 4 (9.52%) had a minor recurrence and 6 (14.29%) had a major recurrence. It was considered that the neuralgic recurrence might be due to some possibly missed pathogenic vessel during operation or a new compression of the trigeminal nerve root occurring after operation.

We present the principal differential characteristics of neuralgia of symptomatic and idiopathic trigeminus. Out of 39 consecutive patients with neuralgia of trigeminus evaluated clinically and with neuroimage study, we find recognizable pathology in 10 (3 arterio-venous malformations, 1 dolichoestasia of basilar artery, 3 tumors, 3 multiple sclerosis). Symptomatic neuralgia occurs in patients with lower average age and is accompanied by atypical symptoms or abnormal neurological signs and is no different from idiopathy as a response to pain from medical treatment. Painful affectation of the t…

One of the possible causes of the so-called "essential" trigeminal neuralgia is a neurovascular compression of the fifth cranial nerve root at the pons. The demonstration of this hypothesis could orientate the surgical treatment to microvascular decompression. In order to evaluate the role of MRI in the diagnosis of trigeminal neuralgia due to neurovascular compression, the authors present the results of a prospective evaluation of the cranial MR studies of 18 neuralgic patients in comparison with a retrospective evaluation of the cranial MR studies of 50 healthy control subjects. The results …

The authors describe a clinical case of severe neuralgia of the third branch of the trigeminal nerve, in whose etiology and pathogenesis a role was played by allergic vasomotor rhinosinusopathy, general allergization of the body, and recurrent herpetic infection. The patient was treated by carbamazepine, tranquilizers, prednisolone, antihistamine and diuretic agents, calcium drugs, bonaphthon, vitamin B1, essentiale, acupuncture, local hydrocortisone phonophoresis and laser therapy. Such treatment made it possible to effectively remove the neuralgic painful syndrome. The given case demonstrate…

129 cases of trigeminal neuralgia were treated microsurgically. The roots of trigeminal nerves were found to be affected by arteries in 96 cases, by veins in 23, and by both arteries and veins in 10. According to these relations, the disease was divided into 3 types: (1) contiguous type (49 cases, 37.9%), vessels 1 mm close to the nerves without any compression; (2) compressive type (42, 32.5%), vessels close to the nerves with compression; (3) adhesive type (38, 29.5%), vessels and nerves adhered to each other. A piece of dacron was used to wrap the nerve root and fixed by gelfoam and silver …

Somatic sensory perception thresholds (warm, cold, hot pain, touch, pinprick, vibration, two-point discrimination), allodynia and skin temperature were assessed in the affected area of 42 patients with unilateral postherpetic neuralgia (PHN) and 20 patients who had had unilateral shingles not followed by PHN (NoPHN), and in the mirror-image area on the other side. There was no difference between the two groups for age or length of time after the acute herpes zoster infection. The PHN group showed significant changes in all sensory threshold measurements when the affected area was compared with…

Immunological investigation was performed in patients with trigeminal neuralgies and painful myofascial temporomandibular joint dysfunction before and after treatment. The data suggest a connection between the state of humoral immunity and the peculiarities of pain syndrome in trigeminal neuralgia and clinically related prosopalgies that was most probably due to different mechanisms of realization of pain syndrome.

An analysis of 57 patients with trigeminal neuralgia (TN) treated with microvascular decompression (MVD) is presented. Mean follow-up time was 3.1 years. Vascular compression of the trigeminal nerve root was noted in 54 cases. In the remaining 3 patients, adhesions were observed in two, whereas no obvious cause was found in one case. Among the patients with vascular compression, 43 (80%) became painfree immediately after surgery, and have remained so during the observation period. There was an indisputable relationship between the degree of observed vascular compression of the nerve and long-t…
