[Neurophysiological aspects of pain and its consequences for the anesthetist].

Nociception is a protective system of the body which prevents it from injury and tissue damage. Human beings respond to noxious stimuli by moving away. They learn by pain to avoid these situations in future. Shortly after major injury, there is a limited analgesic period allowing the body to flee the area of danger, later on, emerging pain compels the body to rest and supports recuperation. While acute pain has a certain meaning, chronic pain does not. It induces a comprehensive suffering including loss of initiative, appetite and vigilance. It reduces life-quality, often accompanied by depres

Guillain-Barré syndrome: clinical neurophysiologic studies.

Guillain-Barré syndrome is a well-defined clinical entity corresponding to primary inflammatory demyelinating lesions of peripheral nerves and spinal roots in the majority of cases seen in Western Europe and North America. Documentation of conduction abnormalities characteristic of demyelination in the context of rapidly developing weakness confirms the clinical diagnosis of acute inflammatory demyelinating polyradiculoneuropathy. These abnormalities include (1) conduction block, (2) markedly prolonged distal latencies, (3) marked slowing of motor conduction velocities, and (4) absent or imper

Hemiparkinsonism-hemiatrophy syndrome: neuroradiological and neurophysiological findings.

A rare form of secondary parkinsonism has been recently described as 'hemiparkinsonism-hemiatrophy syndrome'. We report a case with such clinical features. A neurophysiological study of the evoked potentials, somatosensory and motor, has been conducted. We stress the neuroradiological findings at nuclear magnetic resonance, which documented the presence of an area of altered signal in the mesencephalon, between the cerebral pedunculus and the red nucleus on the right side. The etiopathogenetic considerations and diagnostic criteria are discussed. Moreover we evaluate the efficacy of levodopa t

Neurophysiological monitoring of acute neurological decompression illness.

Somatosensory evoked potentials recorded over the scalp in response to posterior tibial nerve stimulation may provide a useful means of assessing the integrity of the neuraxis during the treatment of neurological decompression illness (DCI). The neurophysiological assessment of a case of DCI is described.

Tension type headache: a neuropsychological and neurophysiological study.

Characteristic personality profiles of patients suffering from either migraine headache (MH) or tension type headache (TTH) have been described in the light of Minnesota Multiphasic Personality Inventory (MMPI). In the present study we evaluated personality profile changes following modifications of the painful symptoms after treatment, through the administration of the MMPI to MH and TTH patients. In TTH patients with no clinical improvement the depression scale scores were higher after 6 months of treatment, whereas in MH patients similar scores were observed before and after therapy, thus s

Progressive myoclonus epilepsies: clinical and neurophysiological diagnosis.

Diagnosis and management of the progressive myoclonus epilepsies (PMEs) provides a challenge to the clinician and neurophysiologist. Over 15 … wo forms), in addition to a number of even rarer disorders. Here we review the clinical aspects and neurophysiology of these disorders, which can now be diagnosed in life by relatively simple methods in the vast majority of cases.

A multimodal neurophysiological assessment in terminal renal failure.

A prospective multimodal neurophysiological study was conducted on 36 patients with end-stage renal failure, 16 of whom subsequently underwent renal transplantation (TR). Nerve conduction study and somatosensory evoked potentials revealed that peripheral conduction deficit, often subclinical, was the commonest abnormality, and TR resulted in substantial improvement. Visual evoked potentials demonstrated subclinical impairment, which did not improve after TR. The brainstem auditory evoked potentials were essentially normal and unaffected by TR.

Neurophysiological dysfunction in young women with intractable constipation.

Fifteen women with intractable chronic idiopathic constipation dating from adolescence were investigated by anorectal manometry, neurophysiological evaluation of the conus medullaris and external anal sphincter. Comparison was made with 25 asymptomatic female control subjects. Urological disturbances were common amongst the constipated, in five of whom incidental lumbosacral spinal dysraphism was found. No differences in sphincter pressures or the rectosphincteric reflex were demonstrable between the two groups. Rectal defecatory sensation was blunted and the compliance was increased in the co

Traumatic optic neuropathy: anatomical and neurophysiological observations.

The case presented is that of a patient who developed optic atrophy after a fall involving head trauma in the orbital area. A diagnostic workup including electrophysiological testing and photodocumentation is presented. Testing identified frank optic neuropathy with normal retinal function.

Neurophysiological and magnetic resonance studies in a patient with hemiballismus.

A case of hemiballismus is reported. Magnetic resonance imaging revealed a cavernous hemangioma in the mesencephalon. The hemiballismus in the biceps and triceps muscles was associated with prolonged bursts of EMG activity.

Neurophysiological and neuroradiological features preceding infantile spasms.

Neonatal EEG, cerebral evoked potentials, CT scan findings and their evolution were investigated in 42 neonates who eventually developed the West syndrome. In infants with perinatal hypoxia, the EEG displayed most often marked or maximal depression in the first week of life. The highly abnormal initial EEG improved progressively with time. The most frequent findings in early infancy was discontinuous tracing and persistent alternating tracing at 1-2 months, and absent sigma rhythms at 3-4 months. The VEP, AEP and CT scan also showed findings suggesting severe degree of brain damage. In infants

Neurophysiological studies in adrenomyeloneuropathy. A report on five cases.

Neurophysiological studies were performed in 5 patients in two families suffering from adrenomyeloneuropathy (AMN). The diagnosis was supported by electron microscopy of nerve twigs in all cases and by the demonstration in 2 cases of increased levels of saturated very long chain fatty acids in cultured fibroblasts (Moser, personal communication). Measurements of the sensory-motor conduction velocities demonstrated the variability of the peripheral nerve damage in AMN, further confirmed by quantitative studies of sensory nerve biopsies. Somatosensory evoked potentials (SEP) were abnormally dela

Hypno-analgesia and acupuncture analgesia: a neurophysiological reality?

The effects of hypnosis, acupuncture and analgesic drugs on the subjective experience of pain and on objective neurophysiological parameters were investigated. Pain was produced by brief electric stimuli on the wrist. Pain challengers were: hypnosis (induced by two different video tapes), acupuncture (at specific and unspecific loci, with and without electrical stimulation of the needles), morphine and ketamine. Evaluation of clinical parameters included the subjective experience of pain intensity, blood pressure, puls, temperature, psychosomatic symptoms and side effects. Neurophysiological p

Intraoperative neurophysiology monitoring in pediatric epilepsy surgery: a case series from a tertiary Italian center.

OBJECTIVES: To characterize intraoperative neurophysiological monitoring in pediatric epilepsy surgery in a single tertiary center. METHODS: We reviewed intraoperative neurophysiological monitoring procedures during pediatric epilepsy surgeries at Meyer Children's Hospital, Florence, Italy, from 2018 to 2025. RESULTS: We analyzed 39 procedures in 37 children with lesional epilepsy. Mean age at surgery was 9.4 years. All patients underwent a comprehensive presurgical workup. Frontal lobe resection was the most frequent surgery, and focal cortical dysplasia was the most common histopathology.
