Approaches to anterior and anterolateral foramen magnum lesions: A critical review.

Foramen magnum (FM) lesions represent some of the most complex cases for the modern neurosurgeon because of their location near vital brainstem structures, the vertebral arteries, and lower cranial nerves. In particular, anterior or anterolaterally located FM tumors have traditionally been most difficult to resect with high morbidity and mortality resulting from approaches through the posterior midline or transorally. For many neurosurgeons, the far lateral, extreme lateral approach, and more recently, endoscopic endonasal approaches have become the preferred modern methods for the resection o…

Perspectives and practices in pediatric cranial fixation.

OBJECTIVE: Cranial fixation is essential to many neurosurgical procedures for ensuring patient head immobilization and optimal surgical outcomes. However, complications such as skull fractures, scalp lacerations, and clamp slippage remain underreported and understudied in the pediatric patient population. This study aimed to identify pediatric neurosurgeon perspectives on skull clamping risks and protocols. METHODS: An anonymous 19-question survey was distributed to 295 pediatric neurosurgeons with an email address registered with the American Board of Pediatric Neurological Surgery and/or th…

The role of the vascular surgeon in anterior lumbar spine surgery.

OBJECTIVES: Advances in spinal fusion techniques have led to an increase in the need for safe access to the lumbar spine anteriorly. The aim … ys (median of 5 days). CONCLUSIONS: The anterior lumbar approach is not generally favoured by many neurosurgeons, despite its many advantages, due to the significant risk of vascular injuries as reported in the literature. This risk is especially acknowledged by the emerging generation of neurosurgeons with very little general surgical exposure during the trai …

Nevertheless, learning the technical nuances of the MFA adds to the neurosurgeon's armamentarium, especially for treatment of small intracanalicular VSs in young patients who wish to preserve hearing.

Optic nerve sheath meningioma--report of three cases.

Meningiomas involving the optic nerve and the anterior visual pathways have intrigued neurosurgeons, neurologists and ophthalmologists for more than a century. They may present with the classical triad of progressive painless visual loss, optic atrophy and opto-ciliary bypass vessels. Optic nerve sheath meningiomas are most frequently diagnosed radiologically and lately, magnetic resonance imaging has greatly improved their visualisation. Surgical removal to preserve vision has met with dismal results. We present three patients with optic nerve sheath meningiomas which were diagnosed based on …

Treatment of tumors involving the anterior cranial fossa.

The potential for a tumor of the upper face, either malignant or nonmalignant, to involve the anterior cranial base is often not appreciated. This leads to inadequate preoperative investigation and to surgery performed by the head and neck surgeon without the help of the neurosurgeon. In this way, complete tumor resection may be compromised or delayed. Neither of these situations is desirable. If the potential for anterior cranial fossa invasion is recognized, there should be prior consultation with the neurosurgeon and a combined operative procedure. Exposure of these lesions has considerably

Navigation-Guided Endoscopic Intraventricular Injectable Tumor Model: Cadaveric Tumor Resection Model for Neurosurgical Training.

BACKGROUND: Intraventricular tumors present difficult challenges to the neurosurgeon. Neurosurgeons have begun to explore the possibilities of using the endoscope in the radical resection of solid intraventricular lesions. There is a steep learning curve when dealing with such lesions with an endoscope. OBJECTIVE: The aim of this study was to create a laboratory training model for neuroendoscopic surgery of intraventricular lesions guided by the navigation system. We believe this technique is more reliable than the traditional approach using contrast injection with C-arm x-ray guidance. MATE…

Minimizing blood transfusions in the surgical correction of craniosynostosis: a 10-year single-center experience.

INTRODUCTION: Our center previously reported low transfusion rates for craniosynostosis surgery by two experienced neurosurgeons using standard intraoperative techniques and acceptance of low hemoglobin levels. This study evaluated whether low rates were maintained over the last 10 years and if a less experienced neurosurgeon, trained in and practicing in the same environment, could achieve similar outcomes. METHODS: All craniosynostosis operations performed in children between 2004 and 2015 were reviewed retrospectively. Transfusion rates were calculated. Analyses examined the relationship o…

Single piece craniotomy in the management of a patient with large vertex epidural haematoma and deteriorating brain function in a limited resources community. Case report from Southwestern Uganda.

Large and expanding haematomas are fatal and require urgent surgical evacuation by neurosurgeons. In rural centres in lower- and middle-income countries like our own, surgery is often more challenging. This is mainly due to nonavailability of neurosurgeons and lack of medical supplies. OBJECTIVE: To review the outcomes of noninstrumented lumbar spine surgery performed by trainees as primary operators versus the outcomes of surgery performed by board-certified faculty neurosurgeons. METHODS: This prospective observational study involved patients undergoing noninstrumented surgery for lumbar disc herniation or lumbar spinal stenosis with a 4.5-year follow-up. Teaching cases (patients were operated on by neurosurgical residents in training) and nonteaching cases (patients were operated on by board-certified faculty neurosurgeons) were reviewed. Radiotherapy as an adjuvant in the management of intracranial meningiomas: are we practising evidence-based medicine?

Although increasingly used, the precise role of radiotherapy in the management of meningiomas is still disputed. The objective of this study, therefore, was to appraise the evidence for adjuvant radiotherapy in benign and atypical intracranial meningiomas, and to compare and contrast it with the current opinion and practice of neurosurgeons in the United Kingdom and the Republic of Ireland. The use of radiotherapy as a primary treatment strategy or its use in the treatment of recurrence was not considered. We performed a systematic review of the evidence for adjuvant radiotherapy in benign and…

Contralateral parafalcine approach for parasagittal and callosal arteriovenous malformations.

Most neurosurgeons prefer an ipsilateral approach for medial surface arteriovenous malformations. Based on recent surgical experiences, we believe that a contralateral approach has some advantages in dealing with such lesions. Two cases are reported, and the rationale for the contralateral approach is discussed.

Challenging Microvascular Decompression Surgery for Hemifacial Spasm.

OBJECTIVE: Microvascular decompression (MVD) is the most effective treatment for hemifacial spasm (HFS). However, surgical difficulties due to complex anatomy or revision surgery can endanger the functional integrity of the brainstem. We describe surgically challenging cases and provide operative guidance that may be helpful for neurosurgeons who perform MVDs. METHODS: Of 3028 patients with HFS who underwent MVDs consecutively by a single neurosurgeon, complex or unusual cases associated with surgical difficulty were selected. Medical charts and images were reviewed, with the primary focus be…

A Canadian perspective on anterior cervical discectomies: practice patterns and preferences.

BACKGROUND: The purpose of this study is to elucidate the current practice patterns of Canadian neurosurgeons with regards to anterior cervical discectomy (ACD). METHODS: A one-page questionnaire was sent out using SurveyMonkey to all neurosurgeon members of the Canadian Neurological Sciences Federation (CNSF). End points were surgeon preference for ACD surgical method, graft source, the length of collar usage and the recommended time before returning to work. RESULTS: Response rate was 74.0%. Of the responders, 75.0% performed single level ACD and 18.3% had completed spine fellowships. The …
