BACKGROUND: Medial displacement calcaneal osteotomy is a common procedure often used as part of pes planovalgus deformity correction. Traditionally the osteotomy is performed using a direct lateral or extended lateral approach, which may carry the risk of wound problems, infection and neurovascular injury. The authors describe a minimally invasive technique to perform the osteotomy and achieve the desired correction. The article illustrates our experience and learning curve with the use of this technique as an option for calcaneal osteotomy.
METHODS: We retrospectively reviewed the records of

This investigation has combined microangiography, immunocytochemistry, coupled with transmission and scanning electron microscopy to discuss the neuroanatomical interactions that occur in the brains of Brattleboro rats with diabetes insipidus, following stereotaxic placement of normal fetal hypothalamic fragments into the third cerebral ventricle. Following surgical placement of 17 day post-coitus hypothalamic fragments, host rats with chronic autosomal homozygous diabetes insipidus were killed and their brains were prepared for analysis. A significant degree of explants (68%) flourished and g

Neurovascular coupling (NVC) maintains appropriate cerebral blood flow (CBF) in response to neuronal activity, and its disturbance, known as neurovascular uncoupling (NVU), is increasingly recognised as a major contributor to neurodegenerative disease. Alzheimer's disease (AD) NVU is caused by Aβ buildup, tau pathology, endothelial dysfunction, and persistent neuroinflammation, leading to poor CBF control and blood-brain barrier (BBB) disintegration. Parkinson's disease (PD) is characterised by α-synuclein aggregation, oxidative stress, mitochondrial dysfunction, and dopaminergic neuronal loss…

RATIONALE AND OBJECTIVES: Cerebral small vessel disease (CSVD) is a major contributor to vascular cognitive impairment, with respective changes in neurovascular coupling (NVC) and glymphatic function, but the association between them has not been illustrated. This study aimed to investigate the relationship between NVC and glymphatic function in patients with CSVD coexisting cognitive impairment.
METHODS: We prospectively enrolled 78 patients with CSVD with cognitive impairment (CSVD-CI) and 57 with normal cognition (CSVD-NC). NVC metrics were assessed by calculating correlation coefficients …

Purinergic signaling is centrally involved in mediating the degeneration of the injured and diseased retina, the induction of retinal gliosis, and the protection of the retinal tissue from degeneration. Dysregulated calcium signaling triggered by overactivation of P2X7 receptors is a crucial step in the induction of neuronal and microvascular cell death under pathogenic conditions like ischemia-hypoxia, elevated intraocular pressure, and diabetes, respectively. Overactivation of P2X7 plays also a pathogenic role in inherited and age-related photoreceptor cell death and in the age-related dysfu

The goal of this study was to demonstrate that a novel resting state BOLD ALFF (amplitude of low frequency fluctuations)-based correction method can substantially enhance the detectability of motor task activation in the presence of tumor-induced neurovascular uncoupling (NVU). Twelve de novo brain tumor patients who underwent comprehensive clinical BOLD fMRI exams including task fMRI and resting state fMRI (rsfMRI) were evaluated. Each patient displayed decreased/absent task fMRI activation in the ipsilesional primary motor cortex in the absence of corresponding motor deficit or suboptimal ta

OBJECTIVE: Vascular dementia (VaD) is a major form of cognitive impairment associated with cerebrovascular injury, neuroinflammation, oxidative stress, impaired neurovascular coupling, and progressive disruption of brain homeostasis. Traditional Chinese medicine (TCM)-related interventions for VaD encompass heterogeneous categories, including herbal formulas, single herbs, natural products, plant-derived bioactive compounds, acupuncture, electroacupuncture, and moxibustion. Within this broad landscape, the herbal and natural-product branch provides an important interface linking TCM research w…

BACKGROUND: Drug refractory focal epilepsy (DRFE) often gives rise to structural and functional damage in the brain accompanied by neuronal activity and cerebral vascular hemodynamics changes, which may result in neurovascular decoupling. However, neuroimaging evidence on neurovascular decoupling remains scarce. This study aimed to assess the manifestation of neurovascular coupling (NVC) in childhood DRFE using resting-state functional magnetic resonance imaging (rs-fMRI) and arterial spin labeling imaging (ASL).
METHODS: The rs-fMRI and ASL imaging data were obtained from 24 children with DR…

Transcranial magnetic stimulation (TMS), a non-invasive and non-pharmacological intervention, is increasingly being explored for mitigating age-related cognitive decline and dementia. Its therapeutic potential is largely attributed to its capacity to modulate neuronal firing rates and induce neuroplastic changes and modulate neurovascular coupling within distributed neural networks that support memory, attention, and executive function. Despite growing interest, gaps remain in understanding how specific stimulation parameters engage in neural circuits and translate into meaningful cognitive ou…

Late-onset Alzheimer's disease (LOAD) is an age-related disease that is strongly associated with vascular risk factors and cerebrovascular impairments. As such, changes in the vasculature with advancing age likely contribute to LOAD, but the mechanisms underlying these contributions remain incompletely understood. With advancing age, there is dysregulation of cerebral blood flow, impairment of neurovascular coupling, and increased blood-brain barrier permeability, which may initiate or contribute to the neuropathology associated with LOAD. Changes to the vasculature outside of the brain, inclu…

Alcohol is a well-known teratogen, and prenatal alcohol exposure (PAE) leads to a greater incidence of many cardiovascular-related pathologies. Alcohol negatively impacts vasculogenesis and angiogenesis in the developing fetal brain, resulting in fetal alcohol spectrum disorders (FASD). Ample preclinical evidence indicates that the normal reactivity of cerebral resistance arterioles, which regulate blood flow distribution in response to metabolic demand (neurovascular coupling), is impaired by PAE. This impairment of dilation of cerebral arteries may carry implications for the susceptibility o…

Knee osteoarthritis (KOA) is a heterogeneous whole-joint disorder in which mechanical loading, aging, metabolic stress, synovitis, microvascular dysfunction, matrix injury, subchondral remodeling, pain-related neurovascular change, and impaired repair interact over time. The infrapatellar fat pad (IFP) is increasingly recognized as an active synovio-adipose tissue, yet IFP inflammation, lipid disturbance, hypoxia, angiogenesis, matrix catabolism, and subchondral change are often considered parallel abnormalities rather than as a spatially organized propagation process. This Perspective propose

Perivascular endfeet of astrocytes are highly polarized compartments that ensheath blood vessels and contribute to the blood-brain barrier. They experience calcium transients with neuronal activity, a phenomenon involved in neurovascular coupling. Endfeet also mediate the uptake of glucose from the blood, a process stimulated in active brain regions. Here, we demonstrate in mouse hippocampal tissue slices that endfeet undergo sodium signaling upon stimulation of glutamatergic synaptic activity. Glutamate-induced endfeet sodium transients were diminished by TFB-TBOA, suggesting that they were g…

Type 2 diabetes mellitus (T2DM) increases vulnerability to cognitive decline through interacting disturbances in insulin signaling, mitochondrial energetics, redox and lipid homeostasis, neuroimmune regulation, neurovascular coupling, and experience-dependent plasticity. This review integrates molecular, cellular, preclinical, and clinical evidence linking these processes to altered gene expression, glial dysfunction, synaptic instability, and impaired cognition. We distinguish well-supported mechanisms from unresolved questions concerning causality, cell-type specificity, human hippocampal re…

Cerebrovascular dysfunction is increasingly recognized as a central contributor to the initiation and progression of Alzheimer's disease (AD). The mammalian/mechanistic target of rapamycin (mTOR) pathway has emerged as a key driver of AD-related vascular dysfunction. Consistent with this role, attenuation of mTOR signaling restores multiple aspects of cerebrovascular function in AD models, including vascular density, cerebral blood flow, vascular reactivity, and neurovascular coupling. However, the molecular mechanisms linking mTOR dysregulation to cerebrovascular dysfunction in AD remain poor…
