Anti-Ri antibodies associated with opsoclonus and progressive encephalomyelitis with rigidity.

A 57-year-old woman without a known neoplasia developed opsoclonus, myoclonus, and ataxia. Positive anti-Ri antibodies were present in both serum and CSF. The patient also had progressive encephalomyelitis with rigidity, an association not previously described.

Muscle and brain disease: an update.

A number of syndromes included under this rubric are considered, and their main features discussed. The congenital muscular dystrophy of the Fukuyama type as it occurs in Japan and in the western world are almost certainly the same condition. The muscle disorder is associated with cerebral lesions which may be due to an arrest of neural migration or to demyelination. Muscle, eye and brain disease, or Santavuori's syndrome, shows ocular abnormalities, as well as those of the muscle and brain, as does the Walker-Warburg syndrome. In the latter disorder the cerebral lesions tend to be more severe

[A case of HTLV-I associated myelopathy with flutter-like oscillation].

We reported a 37-year-old woman showing HTLV-I associated myelopathy (HAM) with flutter-like oscillation of the eyes. She had a 15-year history of spastic paraplegia and noticed visual quaking at the age of 37 years. On neurological examination, she showed spasticity in both lower extremities, hyperreflexia in the extremities, neurogenic bladder, and transient, horizontal and pendular ocular oscillation. Anti HTLV-I antibody titers in serum and cerebrospinal fluid were significantly elevated. Brain CT scan revealed no particular abnormalities. In electrooculography, the oscillation consisted o

Topography of cerebellar deficits in humans.

The cerebellum is a key-piece for information processing and is involved in numerous motor and nonmotor activities, thanks to the anatomical characteristics of the circuitry, the enormous computational capabilities and the high connectivity to other brain areas. Despite its uniform cytoarchitecture, cerebellar circuitry is segregated into functional zones. This functional parcellation is driven by the connectivity and the anatomo-functional heterogeneity of the numerous extra-cerebellar structures linked to the cerebellum, principally brain cortices, precerebellar nuclei and spinal cord. Major

Atypical presentation of restrictive orbital myositis.

The histories of two patients with restrictive ophthalmopathy are reported in whom the initial diagnosis was delayed primarily due to the relatively acute onset. This form of onset of dysthyroid orbital myositis is unique in our experience and demonstrates the importance of realizing that the restrictive ocular motility disorder of Graves' ophthalmopathy may occur abruptly. By such recognition, one may proceed with the appropriate laboratory investigation and spare the patient invasive procedures.

Eye movements in neurodevelopmental disorders.

PURPOSE OF REVIEW: The aim of this paper is to review the literature on eye-movement abnormalities associated with neurodevelopmental disorders. Eye-movement testing is a non-invasive quantitative approach for evaluating brain systems across the age spectrum. It thus provides a promising methodology for characterizing and documenting maturational abnormalities in brain systems associated with neurodevelopmental disorders. RECENT FINDINGS: Recent oculomotor studies have made significant contributions to the understanding of neurodevelopmental disorders, most notably in autism, attention-defici

Chronic progressive external ophthalmoplegia.

Chronic progressive external ophthalmoplegia (CPEO) is a descriptive term for a heterogenous group of disorders characterized by chronic, progressive, bilateral, and usually symmetric ocular motility deficit and ptosis. Significant pain, proptosis, or pupil involvement are not features of CPEO and should prompt evaluation for alternative etiologies. Mitochondrial DNA mutations are increasingly being recognized as the etiology for CPEO syndromes. Clinicians should recognize the specific syndromes associated with CPEO, characterized by variable systemic, neurologic, or other findings. Treatment

Space motion sickness: etiological hypotheses and a proposal for diagnostic clinical examination.

The general notion that space motion sickness (SMS) is due to a "conflict" between vestibular, visual, and other sensory inputs has gained popular acceptance. We have reviewed three specific hypotheses for SMS and identified characteristic disorders of ocular motility that each hypothesis would predict. Accurate recording of horizontal and vertical eye movements during free head movements in space craft presents technical difficulties. We suggest that careful clinical examination may be useful, provided the examination is directed towards detecting those specific abnormalities predicted by eac

Severe Acquired Restrictive Strabismus Fixus in Patients with Trisomy 21.

Acquired strabismus fixus is a rare form of severe esotropia described most frequently in patients with high axial myopia and infrequently associated with other conditions. Refractive errors, cataracts, and ocular motility disorders are common in patients with trisomy 21. We report two unusual cases of patients with trisomy 21 who developed severe acquired restrictive strabismus fixus. Both patients were successfully treated with medial rectus recession followed by lateral rectus and superior rectus myopexy combined with additional medial rectus weakening, including free tenotomy in one case.

Ocular-induced abnormal head postures: A systematic review and analysis.

Abnormal head postures (AHPs) are frequently adopted as compensatory mechanisms by individuals affected by various ocular diseases to optimize the utilization of their visual field or alleviate symptoms such as diplopia. We review the causal relationship between ocular diseases and the resulting AHPs. An extensive literature search was conducted to identify and summarize relevant medical studies providing evidence of AHPs in patients diagnosed with conditions like Duane retraction syndrome, Brown syndrome, superior oblique palsy, and other ocular disorders. The analysis explores the proportion

Nystagmus and Saccadic Intrusions.

OBJECTIVE: This article describes the diagnosis and differentiation of the many possible localizations and causes of nystagmus. LATEST DEVELOPMENTS: The eyes move to keep the fovea on the object of visual regard. To account for the movement of targets, the environment, or the self, different classes of eye movement are necessary to achieve visual stability. These movements involve the vergence, smooth pursuit, saccadic, vestibular, and optokinetic systems, as well as the ability to suppress the vestibuloocular reflex and other movements for steady fixation. When the equipoise of one or more o

Pearls & Oy-sters: INO Plus From Downward Herniation-A Cautionary Tale Regarding Neuro-Ophthalmologic Signatures of Brainstem Compression.

Pupillary assessment is a quintessential part of the clinical examination in neuro-intensive care patients because it provides insight into the integrity of midbrain reflex arcs. Abnormal pupils, particularly anisocoria and later bilateral fixed mydriasis, are classically used to assess expansive intracranial processes because they are frequently considered early indicators of transtentorial midbrain compression due to elevated intracranial pressure. Complex ocular motor deficits mapping to the midbrain are rarely described in the setting of high transtentorial pressure. This is likely because

Altered Large-Scale Resting-State Functional Network Connectivity in Convergence Insufficiency Young Adults Compared With Binocularly Normal Controls.

PURPOSE: To investigate the underlying resting-state functional connectivity (RSFC) of symptomatic convergence insufficiency (CI) compared with binocularly normal controls (BNC) using functional magnetic resonance imaging (fMRI) under The Convergence Insufficiency Neuro‑mechanism Adult Population Study (NCT03593031). METHODS: A total of 101 participants were eligible for this study. After removing datasets with motion artifacts, 49 CI and 47 BNC resting-state functional magnetic resonance imaging datasets were analyzed. CI was diagnosed with the following signs: (1) receded near point of conv

Efferent neuro-ophthalmic complications of coronavirus disease 2019.

PURPOSE OF REVIEW: We set out to describe efferent neuro-ophthalmological complications that have been reported in association with coronavirus disease 2019 (COVID-19) infection. We describe syndromes affecting ocular motility and elaborate on mechanisms of disease, including para-infectious inflammation, hypercoagulability, endothelial damage, and direct neurotropic viral invasion. Despite global vaccination programs, COVID-19 continues to pose an international threat that may rarely result in diplopia or nystagmus. RECENT FINDINGS: Efferent complications include cranial nerve palsies leadin

Microstimulation of Interstitial Nucleus of Cajal Evokes Directionally Disconjugate Eye Movements in Monkeys With Pattern Strabismus.

PURPOSE: Pattern strabismus is characterized by a horizontal misalignment of the eyes that varies with vertical eye position. This disorder has traditionally been described, and treated, as overaction or underaction of the oblique muscles. In recent years, evidence has accumulated that indicate that the disorder is associated with abnormal cross-talk between brainstem pathways that contribute to the horizontal and vertical components of eye movements. The present study was designed to investigate the hypothesis that the key abnormalities are at the level of, or downstream from, the interstitia
