Advanced practice nursing in child maltreatment: practice characteristics.

INTRODUCTION: Child maltreatment is a problem of epidemic proportions in the United States. Pediatric nurse practitioners and other advanced practice nurses (APNs) have been caring for maltreated children for decades, yet to date no comprehensive assessment of their practice characteristics or their clinical and academic contributions to the field has been performed. The purpose of this study is to describe the practice characteristics of APNs who care for maltreated children. METHOD: A descriptive design was used for this study. Child advocacy centers and children's hospitals were contacted

Diagnosis and treatment of traumatic Descemet's membrane detachment: A case series.

RATIONALE: The Descemet layer is a dense layer of tissue that does not detach under normal circumstances. Descemet layer detachment may occur after intraocular surgery, but the Descemet layer spontaneously detached after trauma in this child, which is relatively rare. After looking for the cause, we found that the child was diagnosed with congenital glaucoma, and the trauma induced the Descemet's membrane detachment. PATIENT CONCERNS: The parents of the patient expected the child to recover the normal shape of the cornea as soon as possible, improve vision, and solve the problem of congenital

Anxiety and Depression in Pediatric Patients with Celiac Disease: A Large Cross-Sectional Study.

Mental health is a growing concern in pediatric celiac disease (CD). This study utilized the Revised Children's Anxiety and Depression Scale (RCADS) to investigate anxiety and depression symptom rates. Participants were children ages 8 to 17 years (M = 11.7, SD = 2.7; N = 175) with biopsy-proven CD (Median = 1.1 years post-diagnosis, IQR = 0-4) categorized into groups based on the child's age, caregiver or child respondent, presence or absence of comorbidities, and gluten-free diet duration. Self-reported RCADS scores showed 39% of children having clinically significant concerns for anxiety or

Breastfeeding: Neglect or Excessive Support? A Case Report of Child Abuse by a Negligent Heroin-Dependent Mother.

BACKGROUND: Breastfeeding is one of the best ways to promote, develop, and secure the health of infants. Child abuse is one of the most common and most important problems in the world, and one of the factors that increase its incidence is substance dependency of the parents. Breastfeeding beyond the normal age range can be harmful to the health of the mother and baby, and may represent a pathological parent-child relationship. CASE REPORT: A little girl, who was breastfed until the age of 8 years by her heroin-dependent mother, was hospitalized in a child and adolescent psychiatric ward due t

Social and familial factors in the development of early childhood asthma.

The role of social and familial factors in the development of childhood asthma by age 6 years was studied in a birth cohort of New Zealand children. Rates of asthma varied markedly with the child's sex; boys had twice the rate of asthma as girls. In addition, the factors associated with asthma varied with the child's sex. For boys, wheeze during infancy, early eczema, and parental asthma were all significant risk factors; for girls, the only risk factor was early eczema. Proportional hazards modeling of the data failed to show any significant associations between the development of asthma and

Enuresis in child psychiatric clinic patients.

The clinical profile and the outcome of 386 enuretic children attending a child psychiatric clinic were studied and compared with 2,404 other child psychiatric cases assessed during the same period. The profiles of developmental and family history, psychiatric diagnoses, specific delays in development at assessment, and intelligence resembled each other. The only exceptions were an overrepresentation of delayed developmental milestones in the history and an underrepresentation of neuroses and eating disorders among enuretics.

The relative role of the Child-Pugh classification and the Mayo natural history model in the assessment of survival in patients with primary sclerosing cholangitis.

The Child-Pugh classification is a simple, convenient prognostic measure in patients with liver cirrhosis. We investigated the relative role of the Child-Pugh classification and the Mayo model in the assessment of survival in patients with primary sclerosing cholangitis (PSC). Of the 173 patients described in the original Mayo PSC natural history model, 147 patients had sufficient information in the medical record to allow computation of the Child-Pugh score. We used our most recent modification of the Mayo model to compute the risk score, based on patient's age, serum levels of bilirubin, alb

Cross-Sectional Associations Between Functional Independence and Behavioral Problems in Children with Cerebral Palsy: The Relational Roles of Family Impact and Maternal Self-Blame.

Background/Objectives: Children with cerebral palsy, a permanent neurodevelopmental condition that primarily affects movement and posture, exhibit higher rates of behavioral problems. This study examined the cross-sectional associations linking child functional independence in motor and cognitive domains and maternal self-blame to family adaptation and child behavioral presentations within an integrated analytical framework. Methods: In a cross-sectional design, 79 child-mother dyads in the cerebral palsy group and 62 dyads in a typically developing control group involving children aged 6 to 1

Associations of Maternal Prenatal Stress and Depressive Symptoms With Childhood Neurobehavioral Outcomes in the ECHO Cohort of the NICHD Fetal Growth Studies: Fetal Growth Velocity as a Potential Mediator.

OBJECTIVE: Maternal prenatal stress and mood symptoms are associated with risk for child psychopathology. Within the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) Fetal Growth Studies (ECHO-FGS), a racially and ethnically diverse cohort, we studied associations between prenatal stress and depressive symptoms with child neurobehavior, and potential mediation by fetal growth velocity (FGV) in low-risk pregnancies. METHOD: For 730 mother-child pairs, we had serial ultrasound measurements, self-reports of prenatal stress and depression, observations of ch

Pediatric residency training in child abuse and neglect in the United States.

BACKGROUND: Child abuse and neglect are leading public health problems with significant morbidity and mortality. Previous studies indicate that physicians often lack knowledge and confidence in addressing child abuse and neglect. OBJECTIVES: Our goal was to assess the child abuse and neglect curricula in pediatric residency programs as reported by chief residents and to identify levels of preparedness of residents to address child abuse and neglect on graduation. We analyzed variables related to preparedness. METHODS: A 28-item survey was sent to chief residents of all 203 Accreditation Coun

Identifying latent profiles of child abuse and their gendered effect on adolescent mental health: a school-based study from Rio de Janeiro, Brazil.

This study aimed to identify patterns of child abuse and neglect and to examine the associations between these patterns and psychological distress during adolescence. A school-based survey was conducted with 693 high school students in Rio de Janeiro, Brazil, during 2016-2017. Latent class analysis was used to identify child abuse and neglect patterns based on the Childhood Trauma Questionnaire items' combination. Psychological distress was measured using the General Health Questionnaire (GHQ-12). Multiple linear regression models were employed to examine the associations between child abuse a

Influence of multi-party consultations on child involvement in clinical encounters: An observational study of paediatric palliative care.

OBJECTIVE: To investigate how the presence of multiple parties during paediatric palliative care encounters influences involvement of child patients. METHODS: This mixed-methods observational study analysed 60 video recorded routine clinical encounters within three paediatric palliative care services in Australia. These were classified using a coding scheme that was iteratively developed using conversation analysis methods. Coded data were analysed statistically, with qualitative conversation analysis methods used to explore statistical findings. RESULTS: Statistical analysis of coded data i

Caregiver Experiences With Oral Bilingualism in Children Who Are Deaf and Hard of Hearing in the United States: Impact on Child Language Proficiency.

PURPOSE: Best practices recommend promoting the use of the home language and allowing caregivers to choose the language(s) that they want to use with their child who is deaf or hard of hearing (DHH). We examined whether Spanish-speaking caregivers of children who are DHH receive professional recommendations on oral bilingualism that follow best practices. We also assessed whether professional recommendations, caregiver beliefs, and language practices had an impact on child language(s) proficiency. METHOD: Sixty caregivers completed a questionnaire on demographic questions, language(s) use and

Parenting Programs to Reduce Recurrence of Child Maltreatment in the Family Environment: A Systematic Review.

Physical maltreatment is a public health issue affecting millions of children in their lifetime, with a high risk of recurrency. Although there are several parenting programs (PPs) available, existing reviews on their effectiveness in preventing physical abuse recurrences have many limitations. The current systematic review aims at (1) providing a summary of evidence on the effectiveness of behavioral/cognitive-behavioral PPs in preventing physical re-abuse; (2) extending previous reviews by including reduction of child maltreatment recurrence as the main outcome but also focusing on the effec

Interparental Hostility Interacts with Interparental Cooperative Conflict to Predict Adolescent Social Competence Through Parent-Child Relationship Quality.

An emerging (yet still scant) body of research has linked interparental hostility to youth compromised social competence over time among adolescents. Moreover, little is known about the conditions under which and the processes through which this association might occur. Using prospective data from 878 youth (50.23% females) and their parents and teachers, this study examined how interparental hostility and cooperative conflict might work in conjunction with each other to predict youth social competence over time via parent-child relationship quality. Results demonstrated that interparental coo

Paternal depression and child externalizing behaviors: A meta-analysis.

Although there is a consensus that maternal depression is strongly related to child externalizing behaviors, research on the association between paternal depression and child externalizing behaviors is mixed. Some research shows that paternal depressive symptoms are positively associated with symptoms of externalizing behaviors, including oppositional-defiant behavior, conduct problems, and overall externalizing behavior, while other studies failed to find an association, or demonstrated a weak or negative association. Given that the most recent meta-analysis on paternal depression and child e

[Therapy of childhood schizophrenia].

Due to the multifacet genesis and variability of clinical phenomenology, the therapy of childhood schizophrenia must be multidimensional. Formerly applied techniques like electroshock- and insulin-therapy are now replaced by pharmaco-therapy, primarily with phenothiazines, butyrophenones and chlorprothixens. The dosage depends on age, body weight or body surface. Because of extrapyramidal motor side effects, combinations with anticholinergic drugs may be necessary. Psychopharmaco-therapy alone, however, is insufficient. High emphasis must be placed on psychotherapy and educational guidance and
