[Scaphoid-trapezium-trapezoid dislocation. 5 cases].

The authors report 5 cases of scaphoid-trapezium-trapezoid dislocation (STT) which occurred in crush injuries of the hand. A good functional result was achieved by closed reduction and pinning whenever there was no associated soft tissue damage. However, lateral mid carpal osteoarthritis may develop and produce reduction of the abduction of the thumb as well as slight stiffness of the wrist.

Minimally invasive treatment of pectus excavatum.

Pectus excavatum (PE) is the most common congenital chest wall deformity. It consists of a concavity of the sternum, and the costal cartilages derived from an unbalanced growth of the costochondral regions of the anterior chest wall. The standard operative treatment for PE has been the Ravitch procedure. This technique requires a long incision in the anterior chest wall and bilateral resection of the affected costal cartilages, needing in most cases a posterior metal bar support. The belief that the treatment of PE is basically esthetic led Donald Nuss to develop in 1998 a minimally invasive s

A critical review on bone plates: state of the art and future directions.

The article provides an in-depth overview of the mechanical, chemical, and biological properties of engineering materials used for orthopedic bone plates, along with their designs and fabrication methods. This review addresses the benefits and drawbacks of various materials that have been utilized as bone plates for the treatment of fractures and bone abnormalities. Due to their excellent mechanical properties, metallic bone plates have traditionally been employed for bone fracture fixation. However, the mismatch in mechanical properties and high density of metallic bone plates can lead to str

Rationale and Options for Choosing an Optimal Closure Technique for Primary Midsagittal Osteochondrotomy of the Sternum, Part 2: A Theoretical and Critical Review of Techniques and Fixation Devices.

The intact one-piece sternum is indispensible to the thorax for its normal physiological biomechanics. To overcome tri-planar forces acting on the sternum following midsagittal osteochondrotomy, it must be reconstructed using an optimal technique to withstand distraction loads of normal respiration and violent cough. It should be fixed rigidly to reconstitute the anterior coronal column of the axial skeleton to maintain erect posture and prevent kyphosis. Sterile or infective non-union of the sternum compromises the physical endurance of patients and has an immense psychological effect. From a

[Miniaturized osteosynthesis of articular fractures of the fingers. Results of a series of 60 cases].

The importance of anatomical reduction of articular fractures and early mobilisation of these severe forms of trauma which frequently damage the extensor apparatus (58% of cases, led us to propose direct miniaturised osteosynthesis as described by the Nancy-Strasbourg team. Of the 1,000 cases of osteosynthesis performed between 1978 and 1985, we selected 12 fractures of the MP joint, 38 fractures of the PIP joint and 10 fractures of the DIP joint, differentiated into 4 stages according to the condition of the skin and tendons and into 9 categories according to the anatomical type and the site

A biomechanical evaluation of cervical spinal stabilization methods in a bovine model. Static and cyclical loading.

A bovine model was developed for biomechanical evaluation of surgical procedures stabilizing traumatic cervical injuries disrupting the anterior and posterior spinal column. Cervical spinal segments and C4-5 functional spinal units were tested statically, and C4-5 functional spinal units were tested cyclically in evaluation of 1) the intact cervical spine, 2) Rogers' wiring method, 3) Bohlman's triple-wire technique, 4) sublaminar wiring, 5) anterior cervical plate instrumentation, and 6) posterior hook plate stabilization. Anterior cervical plate instrumentation proved inadequate, and was the

External fixation of the foot and ankle.

The use of Hoffmann external fixation in the treatment of ankle and foot trauma in 26 patients is reviewed. There were 21 males and five females, ranging in age from 14 to 56 years. There were 22 fresh fractures and four arthrodeses. Fourteen of the 22 injuries were open fractures. Eight of 22 involved foot joint disruptions. This form of treatment required a complete set of external fixation equipment, sound knowledge of foot and ankle anatomy, and technical skill in frame construction and application. This fixateur allowed three plane stabilization of complex fractures, mobilization of unaff

The treatment of fractures of the proximal humerus.

Fractures of the proximal humerus, which can not be kept in place after reposition--and therefore not acceptable for conservative treatment--can be sufficiently fixed by a thin and long Rush-pin percutaneously introduced. So the damaged arm can be moved early. No inflammation and no pseudarthrosis was observed in 124 cases. The follow up examination of 98 patients after 6 to 12 months revealed good results. In 4 children the fractured neck of the humerus could not be reposed into a proper position (Interposition of periosteum and joint capsule). After open reduction fixation was performed by K

Surface finishing of Nitinol for implantable medical devices: A review.

Nitinol (NiTi), a nickel-titanium alloy, has been used for various cardiovascular, orthopedic, fracture fixation, and orthodontic devices. As with most other metallic biomaterials, the corrosion resistance and biocompatibility of NiTi are primarily determined by the properties of the surface oxide layer such as thickness, chemical composition, structure, uniformity, and stability. Currently, a number of finishing methods are used to improve the properties of surface oxide of NiTi with an ultimate goal to produce a defect-free, impurity-free, thin homogeneous oxide layer that is stable and comp

[Meaning of living with external fixation for grade III open fracture of lower limbs: patient view].

The present study is aimed to understand the meaning of living with an external fixation device for grade III open fractures of the lower limbs from the perspective of the patient. The data were collected with six young adults who were undergoing outpatient orthopedic treatment in a public hospital in the city of São Paulo, through semi-structured interviews with open questions, between June and August 2010. Seeking to understand the meaning of this experience, we have maintained a phenomenological attitude during the analysis, which made it possible to reveal the phenomenon "try to live in sp

Halo-vest treatment of unstable traumatic cervical spine injuries.

Eighty-three patients with unstable cervical spine injuries were treated with halo-vest stabilization in a prospective consecutive series during a 10 year period. At the follow-up 2-7 years after the trauma, six patients had died and eight patients had been surgically stabilized. Sixty-seven of the remaining 69 patients (97%) were subjected to the follow-up performed with validated protocols. All patients but three were reexamined clinically and radiographically. Flexion-extension motion and sidebending of the neck was measured radiographically. Rotation was measured with the aid of a compass

A novel medical trauma plate using bioactive ceramic composites - design and numerical modelling analysis.

In traditional orthopaedics and traumatology, titanium alloy trauma plates (TATPs) are commonly used as internal fixators for fractured bones, typically with screws. However, this technique presents challenges: TATPs often require removal after healing due to non-degradability and potential corrosion, and their stiffness mismatch with bone can cause stress-shielding that may delay healing. To address these issues, a nacre-like biomimetic apatite-wollastonite composite has been introduced to develop a new trauma plate for orthopedic treatments and prosthetics. This bioactive ceramic composite t

Nonunion in Long Bone Fractures: A Comprehensive Review of Current Treatment Strategies.

Nonunion is a significant complication of long bone fractures, occurring when bone healing fails to progress despite adequate time and appropriate initial management. This condition affects a significant proportion of fractures, with rates varying considerably based on the anatomical location, fracture pattern, patient factors, and treatment methods. The management of nonunion remains challenging and requires a comprehensive understanding of the mechanical and biological factors influencing bone healing. This narrative review systematically examines the current treatment options available for
