Osteomyelitis caused solely by an anaerobic organism is uncommon. We report a case of recurrent Clostridium bifermentans bacteremia resulting in metastatic osteomyelitis involving the sacrum, spine, and ribs. The emergence of resistance of this organism to imipenem and metronidazole is noteworthy because of the usual susceptibility of clostridial species to these antibiotics.

Infections involving bone continue to be a common problem. In children this is usually an acute haematogenous osteomyelitis. Early diagnosis with culture of an aspiration specimen is of paramount importance. Treatment with antibacterial agents is often successful unless pus is obtained in aspiration. In cases with an established abscess, surgical drainage is often needed in addition to antibiotic treatment. Staphylococcus aureus is the most common causative organism, although other microbes are often found in special circumstances such as in neonates, patients with sickle cell disease and thos…

An 11 3/4 year old girl contracted Toxic-Shock-Syndrome following osteomyelitis. The symptoms comprised a severe fall in blood-pressure with centralisation, petechiae and haematoma particularly in the lower extremities, hyperaemia of the oral mucosa and of the throat with profuse and unstaunchable bleeding in this area within in a short period of time. In order to stabilise the blood-pressure the patient had be given approx. 15 l colloidal solution (350 ml/kg) intravenously, mainly frozen-plasma. The risk of aggravation of a pulmonary failure along the lines of ARDS particularly due to the bac…

Two patients with suspected tuberculous spondylitis and one patient with previous Pott's disease were evaluated preoperatively with magnetic resonance imaging (MRI). The MRI provided more exact anatomic localization of vertebral and paravertebral tuberculous abscesses in multiple planes not previously available with more conventional diagnostic methods in the patients with suspected tuberculous spondylitis. This was helpful for localization in planning of surgical approaches. In the patient with previous Pott's disease, spinal cord compression was detected using MRI, which showed no evidence o

Six patients with subacute diaphyseal osteomyelitis of 200 with acute osteomyelitis who were seen and treated at the Red Cross Children's Hospital in a 3-year period (1985-1987) are reported. All six patients had a history of a minimum of 2 weeks' duration and the radiographic picture of a linear periosteal reaction, which was either single or laminated. This radiographic picture was indistinguishable from round cell tumors of bone. The final diagnosis was established by a histology consistent with a subacute osteomyelitis. In two patients, a positive culture of Staphylococcus aureus was obtai…

Vertebral osteomyelitis due to hematogenous seeding of Streptococcus agalactiae occurred in a 29-year-old farmer. The patient was treated with immobilization and parenteral antibiotics but developed recurrent back pain requiring a posterior spinal fusion. In a review of the literature, Group B streptococcal vertebral osteomyelitis seems not to have been previously reported in an adult.

A case of cat-scratch disease in an 8 year-old girl is reported. The inoculation injury followed by regional lymphadenopathy was typical of Parinaud syndrome. This patient also had vertebral lesions on scintigraphy, x-rays and CT scan. While the oculoglandular localization spontaneously improved within a few weeks, osteomyelitic lesions remained grossly unchanged after almost 2 years of follow-up.

The problem of the differential diagnosis between subacute osteomyelitis and primary bone tumors is a difficult one. Presenting symptoms, duration of disease, laboratory data, and location of the pathologic process are of little assistance. The authors reviewed eight typical cases. Preoperative diagnosis included benign and malignant bone tumors, as well as osteomyelitis; but the final diagnosis of osteomyelitis was made only after open biopsy and culture. The presenting symptoms and signs included: pain, usually dull; soft tissue mass; and, rarely, low grade fever. The radiographic features w…

We have evaluated serological tests for the diagnosis of Staphylococcus aureus osteomyelitis. Antiteichoic acid antibodies were elevated in 17 of 23 patients with acute and 16 of 46 with chronic S. aureus osteomyelitis but in none of 33 patients infected with other gram-positive or gram-negative bacteria. Immunoglobulin G antibodies to S. aureus were elevated in 12 of 23 patients with acute and 22 of 47 with chronic S. aureus osteomyelitis, in 2 of 12 infected with other gram-positive bacteria, and in 4 of 21 with other gram-negative bacteria. Assays for S. aureus antibodies may be useful for …

The sequelae of chronic osteomyelitis include not only the common clinical and radiographic signs and symptoms unique to it, but also the frequent residual deformity or amputation secondary to surgical intervention. Exacerbations and relapses are common and may occur as a result of trauma or a decrease in the resistance of the patient. Treatment of this condition over the years has been controversial. Some authors (1, 2) advocate treatment with high oral doses of antibiotics over a period of time up to 6 months. Although good results have been obtained with this regimen, other factors must be …

Osteomyelitis with a fistulous tract of the left fronto-nasal area. The patient received an Anzio Beachhead WW II right fronto-nasal injury repaired with a lucite cranioplasty. A staphylococcal infection involved the remnant of the left frontal sinus with a fistula at the nasion near the orbital ridge. Bone scan revealed increased activity consistent with osteomyelitis. Patient was treated with intravenous oxacillin. The lucite skull prosthesis was removed 2 weeks after admission via the bilateral osteoplastic coronal flap approach with fat obliteration of the residual frontal sinus. The intra…

A simple and reproducible model of acute haematogenous staphylococcal osteomyelitis is described. Twenty-nine day-old chickens were inoculated intravenously with 10(4)-10(8) viable organisms Staphylococcus aureus per kg body weight and were killed 1-8 days after inoculation. Macroscopic septic foci could be detected within 24 hr of inoculation and were most commonly situated in the metaphyseal region of the proximal tibia and distal femur. Lesions in other organs were not observed. The production of osteomyelitis was dependent on the bacterial inoculum size. It was estimated that 5.5 X 10(5) v…

Five cases of pyogenic vertebral osteomyelitis diagnosed clinically and with radioisotope studies are reported. All patients presented with symptoms of back pain, an elevated erythrocyte sedimentation rate, and a normal leukocyte count. Three had positive blood cultures and four had changes on plain radiographs. All five had positive phosphate bone and Ga-67 imaging, and two showed paravertebral uptake of gallium giving a "butterfly"-like appearance.

The diagnosis of pyogenic vertebral osteomyelitis was made in seven narcotic addicts between 1967 and 1972. Vertebrae involved were either cervical or lumbar. Bacteriologic diagnosis was made in each case by percutaneous needle biopsy and aspiration. Staphylococcus aureus was cultured in two patients. Five patients had infections due to Gram-negative bacteria, including Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter. All patients were cured by treatment with antibiotics and immobilization.
