Patterns of personality disorders in women with chronic eating disorders.

The aim of this study was to describe patterns of personality disorders (PDs) in women with chronic eating disorders (EDs). An index group of nineteen women who have had EDs for an average of 8.5 years was compared with a control group of same-aged women from the general population. At the time of the study the index group received treatment at a tertiary treatment center in Stockholm. The PDs were assessed using the DSM-IV part of the DSM-IV and ICD-10 Personality Questionnaire (DIP-Q). In the index group, eighteen of nineteen fulfilled the criteria for one or more PD. The number of PD diagno

The genetics of schizophrenia: new perspectives.

Despite the evidence for genetic factors in schizophrenia, the underlying hereditary defect is unknown. The author reviews recent advances in genetic research methodologies and discusses their strengths and limitations. He concludes that in the face of genetic heterogeneity current models of segregation analysis are not likely to unravel the mode of inheritance of schizophrenia. The author underscores the potential contribution of both molecular genetics (as a means to generate DNA markers) and studies with biologic vulnerability traits to identifying and quantitating the genetic component in

Do psychiatric advance directives protect autonomy?

A psychiatric advance directive (PAD) is a legal resource that allows a person to document his or her wishes about receiving mental health services in the event that he or she is unable to provide consent at a future time. Advantages for completing a PAD include promoting the person's autonomy and authentic self, minimizing harms, and promoting beneficence. However, more research is needed to describe the positive and negative consequences associated with completing and implementing a PAD in the clinical setting.

Some personality patterns and dimensions of male alcoholics: a multivariate description.

The assumption that alcoholics form a homogeneous population has been found to be questionable. Recent research has been done to empirically define possible personality subtypes of alcoholics. This study extended the typological work done previously by Goldstein and Linden (1969a) and Whitelock, Overall, and Patrick (1971). They each found four alcoholic subtypes, three of which replicated across studies. For this research, MMPI profiles of 208 male alcoholics were submitted to a hierarchical clustering procedure. Seven subtypes were found. These results were compared to the results of the pri

Stalin's last years: delusions or dementia?

The sheer scale of Stalin's achievements and institutionalized terror has prompted some authors to label him as a paranoid megalomaniac. Whatever the merits of this diagnosis, his undeniable accomplishments and the rationality of many of his actions cannot be explained by the workings of a disturbed mind. In his last years, however, his life-long suspiciousness became florid paranoia. He eschewed medical advice, listening to a veterinarian and treating his hypertension with iodine drops. Stalin feared his own shadow and trusted no-one, even himself. He increasingly withdrew from official funct

Paranoid-schizoid or symbiotic?

This critique of Mahler's derivation of a developmental stage of normal symbiosis from her work with psychotic children reveals some clear parallels with Klein's description of the paranoid-schizoid position. The defence mechanisms of splitting, and of projective and introjective identification are already operative in the symbiotic stage and imply some distinction between self and other in the unconscious. Both theorists described the same confusion between self and other, but they attributed different explanations to the phenomenon. Whereas Mahler described the experience of feeling at one w

Urinary excretion of bufotenin (N,N-dimethyl-5-hydroxytryptamine) is increased in suspicious violent offenders: a confirmatory study.

We previously reported that violent offenders with paranoid symptoms or whose violent actions had been directed against family members had higher urinary levels of bufotenin than other violent offenders. In the present study, patients were evaluated with the Karolinska Scales of Personality (KSP), and urinary levels of bufotenin were determined by mass spectrometry. In drug-free patients suspiciousness was positively correlated, and socialization was negatively correlated, with urinary bufotenin excretion. These two personality variables were strongly interdependent. In drug users, bufotenin e

Effects of biofeedback on muscular tension in selected personality states.

The effects of electromyographic biofeedback on three selected personality states from the MMPI and one comparison group were investigated. Forty in-patients (28 males and 12 females) ranging in age from 15 to 52 in a psychiatric hospital were classified as manic, agitated, depressed, or designated as a comparison group, according to their MMPI scores and Gilberstadt and Duker's assessment of personality. Fourteen sessions of electromyographic biofeedback were given each individual. Significant differences in EMG readings were found between the manic group and the the comparison group, the agi

A comparative study of opinion and knowledge about mental illness in different societies.

An extensive survey of two communities in Western Nigeria, one urban and the other rural, was conducted to find out some characteristics of opinion and knowledge about mental illness among the Yoruba. A questionnaire containing descriptions of four different types of mental illness (simple schizophrenia, paranoid schizophrenia, anxiety neurosis/depression, and alcoholism), was administered to 771 respondents, randomly selected. The description of paranoid schizophrenia was more readily recognized as mental illness than the others. Respondents tended to typify persons who fit the other three de

A descriptive case-register study of delusional disorder.

OBJECTIVE: A few empirically based studies' data on delusional disorder (DD) exist. We aim to describe sociodemographic and clinical correlates of DD and to identify clinical profiles associated to DD and its subtypes. METHODS: This is a case-register study based on all those subjects attending community mental health services within a geographically well-defined area. Four hundred and sixty-seven patients had been diagnosed as DD cases at psychiatric services serving a catchment area of some 607,494 inhabitants living in South Barcelona (Spain) during a three-year period (2001-2003). A thoro

Clinician safety.

Clinician safety is a serious concern for health care professionals because of the great risk of violence in health care workplaces, especially emergency and mental health departments. Assaults on mental health staff are often encountered from patients who are intoxicated with, or suffering withdrawal symptoms from, substances. Additionally, violence toward mental health professionals is frequently encountered from patients diagnosed with paranoid schizophrenia, personality disorders, learning disabilities, and mental retardation. Incidents of assault are often ignored by victims and administr

Taijin Kyofusho: diagnostic and cultural issues in Japanese psychiatry.

Taijin Kyofusho or 'anthrophobia' is a Japanese culture-specific disgnostic label for the presenting problems of various fear reactions in interpersonal situations. The starting point of the present study was accumulating studies on Japanese cases of Taijin Kyofusho and the assertion in Japanese psychiatry that the symptom complex called Taijin kyofusho is Japanese culture-bound. In light of previous works on culture-bound disorders, the present study examines whether or not American mental health professionals could diagnose Japanese case descriptions of Taijin Kyofusho and what kinds of labe

The heritability of schizophrenia and schizoaffective disorder. A family study.

Ninety-one consecutively admitted patients with schizophrenia (n = 21), schizoaffective depression (n = 43), or psychotic depression (n = 27) entered a blind family study along with 36 never-ill controls. Though schizophrenia spectrum disorders clustered within families, they were not significantly more prevalent in the families of schizophrenic probands. In contrast, morbid risks for affective disorder clearly separated the families of psychotically depressed probands from the families of both schizophrenics and controls. Family study data for schizoaffective probands indicated links to both

Childhood-, teenage-, and adult-onset depression: diagnostic and individual characteristics in a clinical sample.

BACKGROUND: The age at which a depressive episode is first experienced may be associated with particular individual and clinical characteristics. This study compares individual, clinical, and family characteristics across individuals who experienced their first major depressive episode when a child, teenager, or adult. METHODS: Participants were 372 depressed outpatients who participated in 2 completed randomized trials for depression. The first compared fluoxetine and nortriptyline, whereas the second compared cognitive behavior therapy and interpersonal psychotherapy. Assessment across the

The impact of psychiatric and medical comorbidity on the risk of mortality: a population-based analysis.

BACKGROUND: As life expectancy increases, more people have chronic psychiatric and medical health disorders. Comorbidity may increase the risk of premature mortality, an important challenge for health service delivery. METHODS: Population-based cohort study in Ontario, Canada of all 11 246 910 residents aged ⩾16 and <105 on 1 April 2012 and alive on 31 March 2014. Secondary analyses included subjects having common medical disorders in 10 separate cohorts. Exposures were psychiatric morbidity categories identified using aggregated diagnosis groups (ADGs) from Johns Hopkins Adjusted Clinical Gr
