Incidence of facial nerve sacrifice in parotidectomy for primary and metastatic malignancies.

INTRODUCTION: The parotid gland may become involved by primary parotid malignancies and secondarily by metastases from other primary sites. Surgical resection of these tumors can be technically challenging due to the intimate relationship of the parotid gland and the facial nerve. The primary aim of this project was to determine the incidence of facial nerve sacrifice in parotidectomy for primary and secondary malignancies of the parotid. METHODS: A retrospective chart review of was performed. Patients who received parotidectomy with final pathology consistent with a malignant neoplasm were i

Mucoepidermoid carcinoma of the parotid presenting as periauricular cystic nodules: a series of four cases.

Mucoepidermoid carcinoma is a relatively common neoplasm of the major and minor salivary glands that can secondarily involve skin. In the vicinity of the ear lobe, mimicry of a benign cyst, both clinically and histopathologically is a diagnostic pitfall to avoid. The clinical manifestations, diagnostic histopathology, and clinical course of mucoepidermoid carcinoma of the parotid gland presenting as a clinically benign periauricular cystic nodule in four patients ranging in age from 11 to 63 years, are analyzed in the present report. Illustrating the challenge of accurate diagnosis, three of t

Malignant phyllodes tumor of the breast metastatic to the parotid gland diagnosed by fine needle aspiration biopsy. A case report.

BACKGROUND: Phyllodes tumor (cystosarcoma phyllodes) is a rare fibroepithelial neoplasm of the breast. Malignant phyllodes tumor is characterized by an infiltrative border and marked degree of hypercellular stromal overgrowth with > 5 mitoses per 10 high-power fields. Distant metastasis occurs in 10-20% of patients with malignant phyllodes tumor. The most common sites of distant metastases are the lungs, bone and abdominal viscera. Although theoretically any organ may have metastasis, the parotid gland has not been documented before in the English-language literature. CASE: A 40-year-old, Cau

Loss of BAP1 Protein Expression by Immunohistochemistry in the Salivary Duct Carcinoma Component of an Intracapsular Carcinoma ex Pleomorphic Adenoma of the Parotid Gland.

BACKGROUND: BRCA1-associated protein 1 (BAP1) is a tumor suppressor gene that is altered in a variety of neoplasms as well as in BAP1 tumor predisposition syndrome. BAP1 alterations are associated with aggressive behavior in some malignancies and may have treatment implications in future. We present the first documented case of loss of BAP1 protein expression by immunohistochemistry in the salivary duct carcinoma (SDC) component of an intracapsular carcinoma ex pleomorphic adenoma (CXPA) in the context of molecular loss of function of BAP1 in the neoplasm. METHODS: A woman of approximately 55

Orbital medulloepitheliomas -- with extensive local invasion and metastasis: a series of three cases with review of literature.

Intraocular medulloepithelioma is a rare embryonal neoplasm which usually presents in childhood as a ciliary body mass. This tumor is usually confined to the orbit and rarely tends to be locally aggressive and metastatic. We describe three rare cases of metastatic malignant teratoid medulloepithelioma. The patients were males aged 7, 16 and 25 years with right eye involvement in all the cases. All patients had local recurrence (multiple in two) and metastasis to the intraparotid and cervical lymph nodes. Distinction from round cell tumors and primary salivary gland neoplasms occurring in the p

Pediatric parotid mucoepidermoid carcinoma: a case report.

Salivary gland neoplasms are rare, comprising less than 5% of pediatric head and neck tumors, with mucoepidermoid carcinoma (MEC) being the most common malignant type. MEC in the parotid gland is exceedingly rare in children. Histologically, MEC tumors consist of squamous cells, mucus-secreting cells, and lymphoid infiltration, reflecting their complex cellular composition. This case report presents a 9-year-old female with a one-year history of progressive, painless swelling in the left cheek, accompanied by facial nerve palsy. Imaging studies revealed a heterogeneously enhancing mass in the

Unicentric Castleman disease in intraparotid lymph node: a case report.

BACKGROUND: Castleman disease (CD) is a rare lymphoproliferative disorder characterized by non-neoplastic lymph node hyperplasia. Unicentric Castleman disease (UCD), the most common clinical form, typically presents as a solitary, asymptomatic mass. Involvement of intraparotid lymph nodes is rare and often mimics salivary gland neoplasms, complicating preoperative diagnosis. CASE PRESENTATION: A 35-year-old female presented with a painless, enlarging mass in the left parotid region. Contrast-enhanced CT revealed a well-demarcated, homogeneously enhancing mass with additional smaller lesions i

Partial parotidectomy via periauricular incision: Retrospective cohort study and comparative analysis to alternative incisional approaches.

BACKGROUND: We investigated a periauricular approach to the parotid and compared outcomes to alternative parotidectomy incisions. METHODS: … study to date that demonstrates the periauricular incision is a safe and feasible approach for most parotid neoplasms. The cosmetic advantage of this approach is that the resulting scar is smaller and does not extend into the neck.

Acute Parotitis: A Rare Complication of Fine Needle Aspiration in Warthin Tumour.

UNLABELLED: Salivary gland tumours are a group of neoplasms with considerable heterogeneity regarding their histology and biological behaviour. Warthin tumour (WT) is the second most common benign parotid tumour. Options for tissue diagnosis include fine needle aspiration (FNA) and ultrasound-guided core needle biopsy. Complications related to FNA are rare. We present the case of a 49-year-old man admitted with parotitis after FNA and discuss management and an alternative investigative approach when WT is strongly suspected. LEARNING POINTS: Warthin tumour (WT) can be clinically suspected bas

When Crystalloids Tell the Story: Amylase Crystalloids in Salivary Gland Cytology.

Crystalloids in salivary gland cytology are uncommonly recognised findings that may provide important diagnostic clues in differentiating neoplastic from non-neoplastic lesions. Among these, amylase crystalloids are typically associated with benign salivary gland lesions, including inflammatory conditions such as sialadenitis and cystic lesions, as well as benign neoplasms such as Warthin tumour and pleomorphic adenoma. We describe a case of a 59-year-old female presenting with a parotid swelling. Ultrasonography revealed a cystic lesion in the parotid gland. Fine needle aspiration yielded yel

Oncocytic Hyperplasia of the Parotid Gland: A Report of a Rare Case.

Oncocytic hyperplasia of the parotid gland is a rare, benign condition marked by the proliferation of mitochondria-rich oncocytes, often mimicking neoplasms like pleomorphic adenoma. Its etiology remains unclear but may involve inflammatory triggers, as suggested by an associated granulomatous reaction in this case, possibly linked to prior COVID-19 infection. The condition is exceptionally rare, with limited data on prevalence. Patients typically present with painless parotid swelling, as seen in our 45-year-old male patient, initially suspected to have pleomorphic adenoma based on ultrasound

Basal Cell Adenocarcinoma Arising from the Parotid Gland.

Basal cell adenocarcinoma (BCAC) is a rare malignant tumor of the salivary glands, representing 1-2% of salivary gland neoplasms. It is considered a low-grade tumor, often associated with a good prognosis. We report a case of a 60-year-old man with 3-month history of a growing, painless mass in the right ascending ramus of the mandible. Ultrasound and CT scan showed an asymmetry between parotid glands, depicting a nodular structure on the right side. A parotid fine needle aspiration cytology revealed neoplastic cells suggestive of adenoid cystic carcinoma. The patient underwent a total parotid

Carcinoma of the salivary glands: guidelines and case report of sustained remission with docetaxel.

WHAT IS KNOWN AND OBJECTIVE: Salivary glands tumours are rare neoplasms for which there are few clinical trials. The most common malignant parotid tumour is the mucoepidermoid carcinoma. High-grade mucoepidermoid carcinomas are highly aggressive tumours. The initial therapy of localized disease is known, but when there is a recurrence, several options are possible and chemotherapy is generally reserved for palliative treatment. We comment on published guidelines and report a case of sustained remission with docetaxel. CASE SUMMARY: Our case concerns a 64-year-old woman with a high-grade mucoe

Kaposi's sarcoma of an intraparotid lymph node leading to a diagnosis of HIV.

OBJECTIVE: Kaposi's sarcoma is a common malignancy in patients infected with HIV but is rarely seen in the major salivary glands. If a patient is known to be HIV-positive, however, Kaposi's sarcoma must be considered in the differential diagnosis of salivary gland masses in addition to the benign and malignant neoplasms that occur in immunocompetent patients. We present a unique case in which an otherwise healthy patient was diagnosed with HIV after resection of his enlarged parotid gland revealed Kaposi's sarcoma. STUDY DESIGN: Case report. METHODS: A 58-year-old man presented with slowly e

Imaging of salivary gland tumours.

Salivary gland neoplasms account for <3% of all tumors. Most of them are benign and parotid gland is the commonest site. As a general rule, the smaller the involved salivary gland, the higher is the possibility of the tumor being malignant. The role of imaging in assessment of salivary gland tumour is to define intra-glandular vs. extra-glandular location, detect malignant features, assess local extension and invasion, detect nodal metastases and systemic involvement. Image guided fine needle aspiration cytology provides a safe means to obtain cytological confirmation. For lesions in the super

Poorly Differentiated Neuroendocrine Carcinoma of the Parotid Gland and Moderately Differentiated Hepatic Metastases: A Discordant Histopathology Clarified by Dual-Tracer PET/CT.

This teaching case study illustrates a metastatic neuroendocrine neoplasm (NEN) of the parotid gland in which the histopathologic findings were discordant between the primary parotid tumor (poorly differentiated small cell neuroendocrine carcinoma) and a large hepatic metastasis (atypical carcinoid with moderately differentiation status; Ki-67, 15%-20%). The case study also illustrates the value of dual-tracer PET/CT imaging (68Ga-DOTATATE and 18F-FDG) in such a clinical setting. Minimal uptake of 68Ga-DOTATATE and high-grade uptake of 18F-FDG in the lesions indicated a poor differentiation st

Presentation of Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma in a Warthin Tumor: Case Report and Literature Review.

Warthin tumor is the second most common salivary gland neoplasm. It occurs more commonly in males than in females. Malignant transformation in Warthin tumor is a rare but well-recognized phenomenon; however, the development or presentation of lymphoma in a Warthin tumor is rare. An 80-year-old man presented with painless mass of the right parotid gland of 2 years duration with recent ulceration of the overlying skin and right cervical lymphadenopathy underwent a surgical resection of parotid mass and biopsy of the periglandular lymph nodes. The histological diagnosis was malignant lymphoma, ch

Myopericytoma of the parotid region treated by extracapsular dissection.

Myopericytoma (MPC) is a vascular neoplasm exhibiting differentiation towards perivascular cells. Variable cytoarchitechtural features are visible within MPC, and there is much overlap between MPC, myofibroma and glomus tumours. MPC have a local recurrence rate of 10-20% and malignancy has been described in a few published cases. Previously, superficial parotidectomy has been recommended for MPC but, in this case, the surgical approach was via extracapsular dissection (ECD). A 66-year-old Caucasian man presented with a palpable mass arising from the superficial lobe of the right parotid gland.
