Patient safety incidents from acupuncture treatments: a review of reports to the National Patient Safety Agency.

BACKGROUND: Acupuncture is frequently employed to treat chronic pain syndromes or other chronic conditions. Nevertheless, there is a growing literature on adverse events (AEs) from treatments including pneumothorax, cardiac tamponade and spinal cord injury. Acupuncture is provided in almost all NHS pain clinics and by an increasing number of GP's and physiotherapists. Considering acupuncture's popularity, its safety has become an important public health issue. OBJECTIVES: To evaluate the harm caused to patients through acupuncture treatments within NHS organisations. METHODS: The National Re

To pad or not to pad? The use of containment products in healthcare.

Containment products, commonly known as incontinence pads, are medically used in healthcare for patients with bladder and bowel symptoms. However, using them without a clinical need or selecting the wrong style or absorbency can lead to avoidable patient harm. In some cases, product formularies are developed with a greater focus on cost than clinical need, restricting access to certain styles and limiting the number of products prescribed per day. While registered healthcare professionals are accountable for assessing and prescribing containment products for patients under their care, they may…

A Case of Skin Picking Disorder of a Patient with a History of Childhood Abuse.

Skin picking (excoriation) disorder is the recurrent excoriation of one's own skin, resulting in noticeable skin damage. People pick their skin for different reasons. For the majority of patients, first skin picking is associated with a history of childhood abuse and personal problems. Subjects who moderately to severely cause injurious self-harm are more likely to have a history of exposure to domestic violence and childhood abuse than those who do not self-harm. At the same time, these conditions could be related to the etiology for majority of other psychiatric disorders. We report herein,

Pregnancy as a risk factor for undertreatment after bariatric surgery.

A pregnant woman presented at the emergency department with severe nausea and vomiting at 20 weeks of gestational age; she was known with gastric banding. Advanced imaging studies were avoided of fear to harm the fetus. The patient continued to vomit and at 23 weeks intrauterine fetal death was noted. The symptoms did not resolve after delivery and CT scan demonstrated slippage of the gastric band over the pylorus resulting in a high digestive obstruction as the cause of hyperemesis and finally resulting necrosis of the vasa brevia. The gastric band was laparoscopically removed along with the

Determinants of Harm in Fall Incidents in Hospital Settings With 200 or More Beds in Korea.

OBJECTIVE: Falls are a significant patient safety concern in hospital settings, often resulting in unintended harm. This study aimed to investigate the prevalence and risk factors for falls in Korean hospitals with 200 or more beds, analyzing 13,034 incidents reported to the Korean Patient Safety Reporting and Learning System from 2017 to 2021. METHODS: The level of harm was classified into 3 categories: near-miss, adverse, and sentinel events. Hospital-related factors (hospital type, bed capacity, and location and time of fall incident) and patient-related factors (sex, age group, and admitt

Pleural procedural safety in the UK: is everyone's house in order? Reflections from the BTS National Pleural Service Organisational Audit and a national review of patient safety incidents.

INTRODUCTION: The 2022 British Thoracic Society Pleural Services Organisational Audit highlighted evidence of ongoing risk of harm from pleural procedures. To better understand the underlying causes of these safety concerns we undertook a review of patient safety incidents from the National Reporting and Learning System (NRLS). METHODS: Incident-level patient safety data from NRLS were requested from any level 3, 4 and 5 incidents describing harm resulting from pleural intervention, specifically chest drain insertion or pleural aspiration for pleural effusions (fluid), submitted between 1 Apr

Clostridium difficile and other adverse events from overprescribed antibiotics for acute upper respiratory infection.

BACKGROUND: Guidelines widely recommend avoiding antibiotics for many acute upper respiratory infections (aURIs) to avert adverse events in the absence of likely benefit. However, the extent of harm from these antibiotics remains a subject of debate and could inform patient-centered decision-making. Prior estimates finding a number needed to harm (NNH) between 8 and 10 rely on patient-reported adverse events of any severity. In this analysis, we sought to estimate adverse events by only measuring comparatively severe events that require subsequent clinical evaluation. METHODS: We constructed

Beauty Abroad, Burden at Home: Complications and NHS Impact of Cosmetic Tourism-Insights From the BAAPS National Database.

BACKGROUND: Cosmetic surgery tourism has increased rapidly, with growing numbers of UK patients returning with complications requiring NHS care. Limited regulation and inconsistent perioperative standards contribute to preventable harm. OBJECTIVES: To characterise the nature, severity, and causes of complications from overseas cosmetic surgery and estimate the resulting burden on the NHS. METHODS: A national retrospective study analysed all 198 cases reported to the British Association of Aesthetic Plastic Surgeons (BAAPS) Cosmetic Tourism Complications Database between September 2022 and Se

Gender, suicide, and the sociocultural context of deliberate self-harm in an urban general hospital in Mumbai, India.

Recognizing the complementary effects of social contexts and psychiatric disorders, this study clarifies the role of gender in suicidal behavior in urban Mumbai by considering psychiatric diagnoses and patient-identified sociocultural features. The cultural epidemiological approach suggests the critical impact of situational sociocultural factors that complement the customary psychopathological accounts for those who harm or kill themselves. The cultural epidemiology of deliberate self-harm (DSH), it is argued, is critical to planning for suicide prevention, community mental health and psychia

Intentional overdose of azathioprine in a patient with systemic lupus erythematosus.

A 14-year-old girl with systemic lupus erythematosus presented with a mixed overdose of paracetamol, ibuprofen and azathioprine (1500 mg) following a deliberate self-harm attempt. The patient was admitted and monitored. No adverse effects were observed. A review of the literature showed very few reported azathioprine overdoses. Lupus patients are at risk of developing low mood and depression (and related self-harm including overdose of medication). This can be as a consequence of the disease process itself or in reaction to the stresses of living with a chronic disease, which are perhaps parti

Epidemiologic treatment of syphilis.

Epidemiologic treatment refers to antibiotics administered when a diagnosis is considered likely on clinical, laboratory, or epidemiologic grounds, but before the results of confirmatory laboratory tests are known. This treatment is justified on the grounds that the potenial benefits of treating the patient outweigh the potential harm of not treating. This potential harm may affect the individual or the community. Individual sequelae are most significnat when congenital syphilis is allowed to develop due to delays in treating the pregnant woman or newborn child. Community sequelae occur when a

Harm Reduction Principles in a Street Medicine Program: A Qualitative Study.

There is ample evidence that homelessness is associated with high rates of morbidity and mortality. Street Medicine seeks to eliminate these disparities by providing healthcare on the streets to people who are unsheltered. While extant research describes health disparities for the unsheltered and programmatic approaches to addressing housing instability, there are few published studies describing how healthcare providers build and maintain relationships with patients on the street. This insight is central to specifying how street medicine differs from traditional forms of care and defining asp

Disenfranchised Grief: The Silent Price of Minimising Negative Behaviours in Nursing.

Workplace bullying can damage nurses' mental health, professional identity, relationships, psychological safety, and belonging. When these losses are unsupported, the associated grief may become disenfranchised. This discursive paper examines bullying, exclusion, professional undermining, vexatious complaints, and weaponised complaints through a grief-informed lens. It proposes that workplace harm can create losses of trust, reputation, career opportunity, safety, identity, and belonging. Bullying does not inevitably produce grief, but grief may arise when valued aspects of professional life a

Sustainable Healthcare Practices in Pediatric Anesthesia.

BACKGROUND: Healthcare accounts for 4%-5% of global CO2 equivalent (CO2e) emissions, of which hospitals form a considerable component. Identifying evidence-based targets for carbon reduction in pediatric anesthesia can help guide meaningful reductions in healthcare-related environmental harm. METHODS: A narrative review was conducted integrating published data on carbon emissions associated with anesthetic agents, perioperative workflows, waste generation, and hospital energy systems. Quantitative CO2e estimates were incorporated when available. RESULTS: Preoperative strategies with measurab

Exploring Van Gogh Syndrome: A Case Report on Schizoaffective Disorder and Self-Harm.

Deliberate self-harm (DSH) is defined as the intentional infliction of physical injury on oneself without the intent to end one's life. Common behaviors associated with DSH include cutting with a knife or razor, scratching or hitting oneself, and intentional drug overdose. Additionally, these behaviors may encompass restricting food intake and engaging in other risk-taking activities, such as driving at high speeds and participating in unprotected sexual activities. DSH is a strong indicator of suicide risk, particularly in individuals with schizophrenia. The death of Vincent Van Gogh on July

Pain is associated with reduced adherence to safer injection practices among people who inject opioids.

BACKGROUND: Injection opioid use is associated with fatal and non-fatal harms including accidental overdose and infectious diseases. Syringe service programs offer sterile injection equipment and safer injection training to reduce the incidence of injection-related harms. Nevertheless, factors that might reduce adherence to safer injection practices are understudied. This report explores pain as one such factor. METHODS: People who inject opioids (n = 130) were recruited from a syringe service program for a cross-sectional survey assessing the relationship between fluctuations in pain severit

The effectiveness of joint crisis plans for people with borderline personality disorder: protocol for an exploratory randomised controlled trial.

BACKGROUND: Borderline personality disorder (BPD) is a common mental disorder associated with raised mortality, morbidity and substantial economic costs. Although complex psychological interventions have been shown to be useful in the treatment of BPD, such treatments are expensive to deliver and therefore have limited availability and questionable cost-effectiveness. Less complex interventions are required for the management of BPD. A joint crisis plan (JCP) is a record containing a service user's treatment preferences for the management of future crises and is created by the service user wit
