Junctional Modulation of Round Window Membrane Enhances Dexamethasone Uptake into the Inner Ear and Recovery after NIHL.

Delivery of substances into the inner ear via local routes is increasingly being used in clinical treatment. Studies have focused on methods to increase permeability through the round window membrane (RWM) and enhance drug diffusion into the inner ear. However, the clinical applications of those methods have been unclear and few studies have investigated the efficacy of methods in an inner ear injury model. Here, we employed the medium chain fatty acid caprate, a biologically safe, clinically applicable substance, to modulate tight junctions of the RWM. Intratympanic treatment of sodium caprat

Response of the inner ear to lipopolysaccharide introduced directly into scala media.

In an attempt to develop an animal model of immune mediated Meniere's disease, we have injected lipopolysaccharide (LPS) directly into scala media of guinea pigs and monitored functional and morphological changes over a period of 6 weeks. Depending on the concentration of LPS, changes ranged from moderate-to-severe hearing loss and endolymphatic hydrops with minimal cellular infiltrate or fibrosis, to dense cellular infiltration that filled the scalae. Interestingly, higher concentrations of LPS not only induced severe cellular infiltration, hydrops, and hearing loss, but also a substantial en

Magnetic resonance imaging of the inner ear after both intratympanic and intravenous gadolinium injections.

CONCLUSION: Using magnetic resonance imaging (MRI), endolymphatic hydrops could be visualized on both sides after intratympanic (IT) injection of gadolinium contrast agents (Gd) in one symptomatic ear and subsequent intravenous (IV) Gd injection. The MRI revealed a difference of intracochlear Gd distribution between the IT injection side and the contralateral IV side. OBJECTIVES: Although the IT method allows greater enhancement of the perilymph, many patients feel reluctance in receiving the IT injection in asymptomatic ears. We attempted to evaluate endolymphatic space size on both sides wi

Tympanic membrane displacement analysis after glycerol intake: effects on intracranial and intracochlear fluid pressure.

BACKGROUND: Intracranial pressure is transmitted to the perilymph of the cochlea via the cochlear aqueduct and to the endolymph via Reissner's membrane. The tympanic membrane displacement analyzer (TDA) is a new system that may provide a useful, noninvasive, indirect method to identify intracranial and intracochlear pressure changes. METHODS: We performed measurements in 20 persons with normal hearing and in 29 patients with various unilateral diseases of the inner ear such as sudden hearing loss or Meniere's disease. Another series of measurements were done in 15 normal-hearing persons and i

Effects of K(+)-channel blockers on cochlear potentials in the guinea pig.

The effects of different K+ channel blockers, 4-aminopyridine (4-AP), tetraethylammonium (TEA) and quinine, on the various cochlear potentials were observed by the means of perilymph infusion. Each of the three blockers depressed the compound action potential. However, they exerted quite different effects on other cochlear potentials, especially comparing 4-AP, a fast K(+)-channel blocker, with two other blockers. 4-AP induced a significant increase in the magnitude of summating potential, while TEA and quinine decreased it; 4-AP showed no effect on the general endocochlear potential (G-EP, th

[Plasma level of atrial natriuretic peptide following oral glycerol administration].

Atrial natriuretic peptide (ANP) is a cardiac hormone known to mediate increased capillary permeability, vasodilation, and natriuresis. In 1988 the authors' experimental results indicated the presence of ANP receptors in the inner ear. From this they hypothesized that ANP might be involved in the regulation of inner ear fluids and therefore investigated the effects of orally administered glycerol on ANP and cGMP plasma levels. As regards the initial values, there was no apparent significant difference between the 14 subjects with Ménière's disease and the 34 subjects without. During the first

In vitro models of viral-induced congenital deafness.

Cytomegalovirus (CMV) infects 1 to 2 percent of liveborn infants in the United States and causes varying degrees of perceptive hearing loss. There are eight reported pathologic studies of temporal bones in CMV-infected neonates. Viral replication occurs in nonsensory endolabyrinthine epithelium, but viral antigen is also found in the organ of Corti and spiral ganglion neurons, and CMV has been cultured from perilymph. Further clinicopathologic correlation is frustrated, since the inner ear cannot be biopsied during life, and the number of temporal bones available for study is limited, owing to

Sensorineural hearing loss in chronic ear surgery.

The author's series of 3000 consecutive operations of tympanoplasty from 1960 to 1975 were reviewed in regard to the occurrence of sensorineural hearing loss as a consequence of the surgical procedure. Worsening of bone conduction thresholds by 10 dB through the frequencies 500 to 4000 cps, or a 10% reduction in speech discrimination scores were considered significant. Whereas in transcanal tympanoplasty the incidence of cochlear damage was greater in ears when initially the ossiculare chain was incomplete, by contrast in combined approach tympanoplasty the risk was greater when the chain was

Inner-ear delivery of AAV2-retro robustly labels lateral olivocochlear efferents but results in off-target transfection in the contralateral cochlea and the brain.

Gene delivery via adeno-associated viruses (AAVs) is a valuable tool for understanding the organization and function of auditory neuronal circuitry. In this study, we explored the use of AAVs injected into the inner ear to transfect the lateral olivocochlear (LOC) pathway, a poorly understood ipsilateral component of the auditory efferent feedback system. AAV serotypes exhibit a wide range of properties, including cell type selectivity and the capacity for retrograde transport, but none of the AAV serotypes used for inner ear delivery have been reported to successfully transfect the LOC pathwa

A 3D Microscaffold Cochlear Electrode Array for Steroid Elution.

In cochlear implants, the electrode insertion trauma during surgery can cause damage residual hearing. Preserving the residual hearing is an important challenge and the localized administration of drugs, such as steroids, is one of the most promising ways, but remains a challenge. Here, a microscaffold cochlear electrode array (MiSCEA) consisting of a microfabricated flexible electrode array and a 3D microscaffold for steroid reservoir is reported. The MiSCEA without loaded drug is tested by measuring the electrically evoked auditory brainstem response of the cochlea in guinea pigs (n = 4). Th

Vertigo and dizziness--a clinical approach.

Dizziness is a term which is used to describe a variety of sensations. It is possible to group these complaints into four types: a rotational sensation (Type I dizziness), impending faint (Type II dizziness), dysequilibrium (Type III dizziness) and vague lightheadness (Type IV dizziness). Type I dizziness or vertigo is due to disease of the vestibular system--peripheral or central, and is characterized by a feeling of movement relative to one's surrounding. The majority of dizzy patients, however, belong to Types II, III and IV, collectively called the non-vestibular system disorders. The dist

[Posterior crus stapedectomy: an obsolete method in otosclerosis surgery?].

BACKGROUND: Stapedectomy and stapedotomy with interposition of prostheses are the methods of choice for surgical treatment of otosclerosis. For the present study we resumed and reevaluated the posterior crus stapedectomy, a method based on the principal of renouncing a prosthesis by cutting the posterior crus of the stapes close to the footplate and the anterior crus close to the stapes head. METHODS: The posterior crus is temporarily transposed with the incudostapedial joint remaining intact. After performance of platinectomy and sealing of the oval window with fascia it is repositioned onto

Endolymphatic and intracellular resting potential in the alligator lizard cochlea.

Endolymphatic potential (EP) and intracellular resting potentials (RPs) in the cochlea of the alligator lizard were measured with micropipets. The EP (defined as the dc electric potential recorded between the endolymph in scala media and the perilymph in scala tympani or scala vestibuli) had a mean value of +16 mV (in 96 ears). The RPs (defined as the dc potentials recorded between the intracellular and perilymphatic spaces) were different in the distal portion of the cochlear nerve (range to -56 mV, mean of -23 mV in 158 cells) from those in the region of the basilar papilla (range to -154 mV

Kinetics of gentamicin uptake in the inner ear of Chinchilla langier after middle-ear administration in a sustained-release vehicle.

The search for a safe, effective treatment for the vertigo associated with Meniere's disease has long been an important topic in otolaryngology. In recent years many groups have begun using intratympanic gentamicin to treat this vertigo. Although reported cure rates are as high as 90%, many questions remain regarding this type of treatment. Current limitations are the necessity for repeated treatments and a lack of clear dosing guidelines. In addition, the gentamicin must be delivered in a manner that allows maximal vestibulotoxic effect without injury to hearing. Until investigators can contr

Neutrophil Extracellular Traps Affect Human Inner Ear Vascular Permeability.

The integrity of the blood-labyrinth barrier (BLB) is essential for inner ear homeostasis, regulating the ionic composition of endolymph and perilymph and preventing harmful substance entry. Endothelial hyperpermeability, central in inflammatory and immune responses, is managed through complex intercellular communication and molecular signaling pathways. Recent studies link BLB permeability dysregulation to auditory pathologies like acoustic trauma, autoimmune inner ear diseases, and presbycusis. Polymorphonuclear granulocytes (PMNs), or neutrophils, significantly modulate vascular permeabilit
