Spatial disparities and determinants of perinatal mortality in Lesotho using multiscale geographically weighted regression.

Perinatal mortality is a major public health challenge in many developing countries and has profound social, economic, and health implications for families and broader society. Despite its serious and devastating effects, there are a few studies that describe the spatial distribution and factors associated with perinatal mortality in Lesotho. Understanding the burden and geographic variation is crucial for implementing targeted interventions, allocating resources efficiently, and achieving the Sustainable Development Goals. Therefore, this study explores the spatial variations and predictors o…

Data-driven insights into post-autopsy consultations: causes of perinatal mortality and parental outcomes.

OBJECTIVE: Perinatal mortality deeply affects parents and healthcare providers. Post-autopsy consultations (PACs) help parents understand the causes of fetal death and offer emotional support. This study evaluates their effectiveness in emotional healing, preventing medico-legal disputes, and identifying key death causes. METHODS: This retrospective study analyzes 360 fetal autopsies at San Martino Hospital, Genoa (2013-2021). Among them, 120 parents sought PACs. Causes of fetal death were classified using the relevant condition at death (ReCoDe) system, and parental emotional outcomes were e…

Fetal echocardiographic prediction score for perinatal mortality in tricuspid valve dysplasia and Ebstein's anomaly.

OBJECTIVES: Tricuspid valve dysplasia (TVD) and Ebstein's anomaly (EA) diagnosed by fetal echocardiography vary greatly in terms of clinical … y flow, direction of ductal flow and LV-Tei index. The TRIPP score was found to predict efficiently perinatal mortality in fetuses with TVD/EA. CONCLUSIONS: Our novel combinatorial score of echocardiographic parameters, the TRIPP score, including LV-Tei index, is easy to measure and provides a good tool for the prediction of perinatal mortality in fetuses diagnosed …

Weather extremes and perinatal mortality - Seasonal and ethnic differences in northern Sweden, 1800-1895.

BACKGROUND: Many studies have shown the impact of heat and cold on total and age-specific mortality, but knowledge gaps remain regarding weather vulnerability of very young infants. This study assessed the association of temperature extremes with perinatal mortality (stillbirths and deaths in the first week of life), among two ethnic groups in pre-industrial northern Sweden. METHODS: We used population data of indigenous Sami and non-Sami in selected parishes of northern Sweden, 1800-1895, and monthly temperature data. Multiple logistic regression models were conducted to estimate the associa…

Effectiveness of an integrated approach to reduce perinatal mortality: recent experiences from Matlab, Bangladesh.

BACKGROUND: Improving perinatal health is the key to achieving the Millennium Development Goal for child survival. Recently, several reviews suggest that scaling up available effective perinatal interventions in an integrated approach can substantially reduce the stillbirth and neonatal death rates worldwide. We evaluated the effect of packaged interventions given in pregnancy, delivery and post-partum periods through integration of community- and facility-based services on perinatal mortality. METHODS: This study took advantage of an ongoing health and demographic surveillance system (HDSS) …

Perinatal periods of risk: phase 2 analytic methods for further investigating feto-infant mortality.

The perinatal periods of risk (PPOR) methods provide a framework and tools to guide large urban communities in investigating their feto-infant mortality problem. The PPOR methods have 11 defined steps divided into three analytic parts: (1) Analytic Preparation; (2) Phase 1 Analysis-identifying the opportunity gaps or populations and risk periods with largest excess mortality; and (3) Phase 2 Analyses-investigating these opportunity gaps. This article focuses on the Phase 2 analytic methods, which systematically investigate the opportunity gaps to discover which risk and preventive factors are

Perinatal periods of risk: analysis of fetal-infant mortality rates in Kansas City, Missouri.

OBJECTIVES: The Perinatal Periods of Risk (PPOR) technique was used to analyze resident fetal and infant death data from Kansas City, Missouri, for the period 1998-2002. Results offer important information that can be used to develop community-based prevention strategies related to racial/ethnic disparities in infant mortality rates (IMR). METHODS: The PPOR approach for fetal and infant mortality can be mapped by birthweight at delivery and time of death into four strategic prevention areas: 1) Maternal Health/Prematurity (MHP), 2) Maternal Care (MC), 3) Newborn Care (NC), and 4) Infant Healt

[Effect of Doppler ultrasound on perinatal mortality of high risk pregnancies: required patient sample?].

The clinical value of Doppler ultrasound in obstetrics is related to the question whether pregnancy course and outcome will be improved when this method is introduced into obstetrical management. Recently a couple of randomized controlled management studies have been published and the majority of these trials showed significant beneficial effects. Even perinatal mortality may possibly be reduced, but none of these trials was large enough to achieve a significant reduction. Giles and Bisits [12] performed a metaanalysis by combining evidence from 6 of these trials in high risk pregnancies and c…

Causes and factors affecting perinatal mortality at Princess Basma Teaching Hospital in North Jordan.

This is the first study about the perinatal mortality in North Jordan. Between 1st June 1991 and 31st May 1992, 8,146 deliveries took place at Princess Basma Teaching Hospital. There were 250 perinatal deaths comprised of 124 stillbirths and 126 early neonatal deaths. The perinatal mortality rate was 30.6 per 1,000. The factors which might have affected this rate were mainly prematurity, low birth weight, unexplained intrauterine fetal death, and congenital malformations. The possible ways of improving the results are discussed.

Perinatal mortality in rural India: a strategy for reduction through primary care. I Stillbirths.

In a prospective community based study of the distribution and determinants of stillbirths in a rural area of Maharashtra, India, that was carried out for two years, 3129 singleton and 22 twin births were recorded in a population of 47 000. Of the 3173 babies, 85 singletons and five of the twins were stillborn giving a stillbirth rate of 28.4/1000 births. The causes of stillbirths are analysed and the possibility of reducing the unacceptably high stillbirth rate by adequate training of grassroot level workers in screening pregnant women for detection of "at risk" mothers and their timely refer

Improvement in mortality of very low birthweight infants and the changing pattern of neonatal mortality: the 50-year experience of one perinatal centre.

AIM: Neonatology is a relatively new sub-specialty so we aimed to review survival data in the context of advances in neonatal care. METHOD: Review of neonatal survival for very low birthweight babies over the last 50 years. RESULTS: In the data collected from a single tertiary neonatal unit, survival for babies 501-1000 g improved from below 10% in 1959 to over 60% in 2009. Similarly, survival for babies 1001 to 1500 g has improved from approximately 50% to over 90%. During the study period, death due to extreme prematurity or cardiorespiratory problems, namely respiratory distress syndrome,
