Recreating the village: the patient experience with a hybrid model of Group Perinatal Care (GPPC) in an academic family health team.

BACKGROUND: Group prenatal care (GPC) has been shown to have a positive impact on social support, patient knowledge and preparedness for birth. We developed an interprofessional hybrid model of care whereby the group perinatal care (GPPC) component was co-facilitated by midwives (MW) and family medicine residents (FMR) and alternating individual visits were provided by family physicians (FP's) within our academic family health team (FHT) In this qualitative study, we sought to explore the impact of this program and how it supports patients through pregnancy and the early newborn period. Associations of inflammation related prenatal adversities with neurodevelopment of offspring in one year: a longitudinal prospective birth cohort study.

BACKGROUND: The recent Maternal Immune Activation (MIA) theory suggests maternal systemic inflammation may serve as a mediator in associations between prenatal maternal adversities and neurodevelopmental diseases in offspring. Given the co-exposure to multiple adversities may be experienced by pregnant person, it is unclear whether a quantitative index can be developed to characterize the inflammation related exposure level, and whether this index is associated with neurodevelopmental delays in offspring. METHODS: Based on Jiangsu Birth Cohort (JBC), a total of 3051 infants were included in t

Affect dysregulation as a way of life.

Storms of affect, states of panic, and other apparently dysregulated affect states may not be as dysregulated as they seem. While initial states of panic during a rape, etc. may have the quality of chaos and be associated with dissociative experience, rekindled affect may actually burst forth in an unconsciously scripted manner, telling a story for which words have not yet been found. The story of abusive experience may only appear to be dysregulated. The patient with an apparent dysregulation of affect may be suffering from complex affect scripts to which she is subject via unconscious attemp

Development of a spatial skills test for 4th grade primary school students: validity and reliability study.

Although several instruments have been developed to assess spatial skills, most were designed for older students, emphasize specific components of spatial ability, or have not been specifically designed to reflect the developmental characteristics and mathematics curriculum of fourth-grade primary school students. Therefore, this study aimed to develop a valid and reliable spatial skills test specifically designed for fourth-grade primary school students. The study employed a quantitative methodological instrument development design. First, a literature review was conducted to generate an item

Money Helps: Financial Incentives for Smoking Cessation Among People Experiencing Homelessness.

INTRODUCTION: Over 70% of people experiencing homelessness report current cigarette smoking. We conducted a randomized controlled trial of an extended (6-month) contingency management (CM) intervention for smoking cessation among adults experiencing homelessness and who were engaged in primary care. We examined experiences and perspectives on cessation among a subset of participants in the trial. METHODS: The CM group received escalating incentives in the form of gift cards contingent on carbon monoxide-verified abstinence; the control group received fixed incentives for attending visits, not

Machine learning-assisted photoelectrochemical aptasensor for nalbuphine monitoring using a BiOBr/SrWO4 S-scheme heterojunction.

Precise therapeutic drug monitoring is essential for optimizing nalbuphine treatment in uremic pruritus, yet rapid and sensitive analytical tools suitable for clinical application remain limited. Herein, we report a machine learning-guided photoelectrochemical (PEC) aptasensor for nalbuphine detection based on a BiOBr/SrWO4 S-scheme heterojunction. Clinical data from 102 peritoneal dialysis patients were first analyzed using seven machine learning algorithms to identify determinants of therapeutic response. Gradient Boosting exhibited the best predictive performance, with an accuracy of 84.6%,

Non-pharmacologicaL InterVEntions for Antipsychotic-Induced Weight Gain (RESOLVE) in People Living With Severe Mental Illness: A Realist Synthesis.

INTRODUCTION: Antipsychotic medications are used to treat individuals with severe mental illness (SMI) but are associated with rapid weight gain and several physical and mental risk factors. Early, proactive weight management is necessary to preempt these risk factors. The aim of this research was to understand and explain how, why, for whom, and in what contexts non-pharmacological interventions can help to manage antipsychotic-induced weight gain. METHODS: A realist review was conducted to identify contextual factors and underlying mechanisms associated with effective, non-pharmacological w

Childhood maltreatment, multilocus HPA-axis genetic variation and adolescent comorbidity profiles of depressive and anxiety symptoms.

BACKGROUND: Despite a growing body of evidence showing both genetic and environmental influences on adolescent depression and anxiety, the involved comorbid mechanisms regarding gene-by-environment (G × E) interaction remain unclear. OBJECTIVE: The current study was the first to investigate the extent to which multilocus hypothalamic-pituitary-adrenal (HPA)-axis genetic variants moderated the association between childhood maltreatment and adolescent comorbid depression and anxiety. METHODS: The participants were 827 Chinese Han adolescents (Mage = 16.45 ± 1.37 years; 50.2 % girls). A theory-

Women football observers' experiences: a perspective from system justification and glass ceiling.

INTRODUCTION: The legitimization theory of the system explains that despite people's inherent drive towards personal and group interests, they tend to support social systems. Understanding the sources of social pressure and the glass ceiling perceptions of female football observers is the main aim of this study in terms of examining attitudes within the legitimized system. To this end, the study sought to answer how female football observers are constructed in a marginalized position within the male-dominated football culture through sources of social pressure, and how they accommodate the acc

The dimensionality of vaccination intentions: One strain or multiple strains?

OBJECTIVE: People likely have different attitudes toward different vaccines (e.g., they may hold a positive attitude toward the measles, mumps, and rubella-vaccine while simultaneously hold a neutral attitude toward the flu shot). To examine the dimensionality of vaccination intentions, we measured vaccination intentions toward 16 different diseases. We hypothesized that people differentiate between child-directed vaccination intentions and self-directed vaccination intentions. Furthermore, we hypothesized that some commonly studied factors (e.g., trust in authorities and fear of needles) migh

Pathways to the therapist paragon: a decolonial grounded theory.

INTRODUCTION: While many professional associations within clinical and counseling psychology have made an aspirational call for clinician awareness of social position, there is a lack of research into how socially-conferred privilege impacts psychotherapy. Specifically of interest is the differences in race and gender within the therapeutic dyad, in which there is a BIPOC (Black, Indigenous, and Persons of Color)/white or male/female-identified dynamic. METHOD: The authors utilized a Grounded Theory approach to analyze qualitative interviews with practicing psychologists to construct a proces

Development and psychometric analysis of the PROMIS pain behavior item bank.

The measurement of pain behavior is a key component of the assessment of persons with chronic pain; however, few self-reported pain behavior instruments have been developed. We developed a pain behavior item bank as part of the Patient-Reported Outcome Measurement Information System (PROMIS). For the Wave I testing, because of the large number of PROMIS items, a complex sampling approach was used where participants were randomly assigned to either respond to two full-item banks or to multiple 7-item blocks of items. A web-based survey was designed and completed by 15,528 members of the general

How do speech and language therapists enact aphasia psychosocial support in Ireland? A cross-sectional online survey informed by normalization process theory.

BACKGROUND: Supporting psychosocial well-being in aphasia is necessarily person-centred, interdisciplinary and coordinated. Shortcomings in such support are described in Ireland and elsewhere. Speech and language therapists (SLTs) are integral; and describing current practice and barriers they experience is important for enhancing service delivery. AIMS: To understand how SLTs enact, and are supported to enact, aphasia psychosocial care in Ireland. METHODS & PROCEDURES: This is a cross-sectional, online, self-administered clinician survey targeting SLTs working minimally 1 year post-qualific

Toward a multidimensional fitness paradigm: a critical review and agenda for science, practice, and policy.

This study aims to critically re-evaluate classical definitions of physical fitness proposed by leading institutions such as the American College of Sports Medicine (ACSM), the World Health Organization (WHO), and the National Academy of Sports Medicine (NASM). Despite their physiological robustness, these definitions no longer capture the multidimensional determinants of human functionality in modern life. The paper introduces a multidimensional fitness paradigm that reframes fitness as a biopsychosocio-emotional capacity integrating physical, mental, social, occupational, and digital dimensi

Heterogeneity in health anxiety among medical students and HPV testing acceptability: the mediating role of eHealth literacy from a person-centered perspective.

BACKGROUND: Medical students, as future healthcare professionals, play a critical role in promoting preventive attitudes such as Human Papillomavirus (HPV) testing. However, their own preventive intentions may be influenced by psychological factors. This study aimed to identify latent profiles of health anxiety (HA) among medical students and examine how these profiles relate to HPV testing acceptability through the mediating role of eHealth literacy (eHL), informed by selected constructs from Protection Motivation Theory (PMT). METHODS: A cross-sectional survey was conducted among 887 medica

Model Development on Adherence to Aspirin in Pregnancy: A Co-Produced Qualitative Systematic Review and Meta-Ethnography.

OBJECTIVE: Low-dose aspirin is proven to reduce the incidence of pre-eclampsia, yet adherence remains low. This systematic review synthesises qualitative evidence on barriers and facilitators to low-dose aspirin adherence in pregnancy and uses a co-production approach to develop an explanatory model of this complex behaviour. The overall aim is to contribute to the development of a theory of adherence, targeting this health issue. METHODS: Data sources: A search of electronic databases (Medline, Web of Science, CINAHL, PsycINFO, Embase, Scopus, Open Grey, Google Scholar, Prospero), charity an
