Personal identity development as a foundation for parenthood in young adulthood: 7-year longitudinal associations with the desire to have children, actual parenthood, and parental identity.

The transition to parenthood is a transformative milestone in young adulthood, profoundly reshaping one's sense of self. Although young adults differ widely in the steps they have taken toward parenthood, research on the developmental foundations of the parental role remains limited. This study examined how personal identity development-both in terms of levels and within-person changes in overall commitment and ruminative exploration-relates to three key indicators of the transition to parenthood: the desire to have children, actual parental status, and parental identity. The analytical sample

MeDeMSA care study protocol: developing personalized best medical care with integrated mobile palliative and telemedicine support for individuals with multiple system atrophy.

Multiple system atrophy (MSA) represents a management challenge due to its variable clinical presentation, and lack of established multidisciplinary care protocols. In advanced stages, physical barriers often prevent access to specialized care, leaving patients and caregivers to face complications and fear alone. This 18-month, monocentric, open-label study evaluates the impact of a personalized, multidisciplinary treatment, integrating mobile palliative care, on the quality of life (QoL, measured by EQ-5D-5L indices) of MSA individuals compared to a matched historical European MSA cohort. For

Development of a Multivariable Model to Predict the Need for Bone Marrow Sampling in Persons With Monoclonal Gammopathy of Undetermined Significance : A Cohort Study Nested in a Clinical Trial.

BACKGROUND: Monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM) are asymptomatic precursor conditions to multiple myeloma and related disorders. Smoldering multiple myeloma is distinguished from MGUS by 10% or greater bone marrow plasma cells (BMPC) on sampling, has a higher risk for progression, and requires specialist management. OBJECTIVE: To develop a multivariable prediction model that predicts the probability that a person with presumed MGUS has 10% or greater BMPC (SMM or worse by bone marrow criteria) to inform the decision to obtain a bone

Detection of latent tuberculosis by the tuberculin skin test and a whole-blood interferon-γ release assay, and the development of active tuberculosis in HIV-seropositive persons.

This study evaluated the QuantiFERON-TB Gold In-Tube (QFT-GIT; Cellestis, Carnegie, Australia) test and the tuberculin skin test (TST) for the detection of latent tuberculosis infection (LTBI) in HIV-infected adults. One hundred thirty-five HIV-seropositive persons and 135 controls underwent TST and QFT-GIT. HIV-infected patients who gave a positive result on either test were offered chemoprophylaxis. The prevalence of LTBI was 6.7% by TST and 9.6% by QFT-GIT (P = 0.3) in HIV-seropositive subjects, and 34.8% by TST and 21.5% by QFT-GIT (P = 0.02) among controls. TST reactivity declined sharply

Development and validation of a clinical prediction rule for hospitalization due to pneumonia or influenza or death during influenza epidemics among community-dwelling elderly persons.

BACKGROUND: Uncertainties among health care providers and patients about the risk of serious influenza-associated complications and the potential benefits of vaccination may contribute to unsatisfactorily low influenza vaccination rates. To quantify the risk of serious outcomes (hospitalization due to pneumonia or influenza or death due to any cause) during influenza seasons, we developed a clinical prediction rule for the probability of hospitalization due to pneumonia or influenza or death among elderly persons. METHODS: We developed the clinical prediction rule using data from linked admin

Building meaningful partnerships: engaging and co-developing a patient advisory council to inform the implementation of person-centred quality indicators in Alberta primary care.

BACKGROUND: Effective primary care improves patient outcomes by providing comprehensive, continuous, and person-centred care. The Patient's Medical Home vision positions primary care as the central entry point to high-quality health care in Canada. In Alberta, Person-centred Care Quality Indicators (PC-QIs) were developed with patients and health system stakeholders, are being implemented across the province to assess and report patient experiences in primary care. To ensure that patients are central to this work, a PC-QI patient Advisory Council (PAC) was created. METHODS: The PC-QI PAC was

Developing a model to support safe medication use in the home for foreign-born-persons with language difficulties: a co-creation participatory action research study.

BACKGROUND: Migration has increased population diversity and created challenges for safe medication use. Evidence remains limited regarding factors influencing medication-related problems among migrants and the support needed to optimise medication use in the home. No studies have addressed support for medication use in the home among migrants or proposed models for such support. The aim was to identify the support needed and to develop a model for support to promote safe medication use in the home and prevent medication errors among foreign-born persons with language difficulties. METHODS: A

Within-person imbalance of reward sensitivity and executive functioning across adolescent development: A longitudinal examination of the dual systems model from childhood to adulthood.

The dual systems model of adolescent development asserts that the neurobiological systems underlying reward/motivational processes and cognitive control mature at different rates, resulting in an "imbalance" during adolescence whereby adolescents are biased toward rewards but unable to exert sufficient executive control in risk-taking contexts. While a hypothesized imbalance between these systems is central to the dual systems model, few studies have investigated longitudinal trajectories within and between each system with age. Therefore, this validation study assessed the developmental traje

Exploring the associations between the biomechanical and psychological mechanistic pathways of lower back pain development amongst persons with lower-limb amputation: A study protocol.

INTRODUCTION: The global rise in lower-limb amputations is mainly due to diabetes and vascular complications. Amputations cause serious physical and psycho-social disabilities which impair locomotion and compromise patients quality of life. Biopsychosocial factors such as altered gait parameters, poor prosthetic fit, depression, fear avoidance behaviours and stigma increase the risk of individuals incurring lower back pain; the leading cause of secondary disability amongst persons with lower-limb amputation. Exploring the potential mechanistic pathways of lower back pain development is importa

Towards a standardized approach of assessing social context of persons receiving home care in Flanders, Belgium: the development and test of a social supplement to the interRAI instruments.

BACKGROUND: Apart from a person's physical functioning, the early identification of social context indicators which affect patient outcomes - such as environmental and psychosocial issues - is key for high quality and comprehensive care at home. During a home care assessment, a person's biomedical and functional problems are typically considered. Harder to define concepts, such as psychosocial well-being or living arrangements, are not routinely documented, even though research shows they also affect functioning and health outcomes. The purpose of this study is to develop and test a concise, i
