Incident ischaemic heart disease in persons with Alzheimer's disease in a Finnish nationwide exposure-matched cohort.

BACKGROUND: Previous studies on the association between cardiovascular diseases and Alzheimer's disease (AD) have been inconsistent despite the overlapping risk factor profile. We assessed whether the incidence of ischaemic heart disease (IHD) and revascularisation procedures are different in persons with AD than in the matched population without AD. METHODS: We conducted a nationwide exposure-matched cohort study including all 28,093 community-dwelling individuals with clinically verified diagnosis of AD, residing in Finland and alive on December 31, 2005. Participants were identified from t

Adverse events among nursing home residents with Alzheimer's disease and psychosis.

PURPOSE: The objective of this study was to quantify rates of adverse events in a high-risk multi-morbid population of institutionalized patients with Alzheimer's disease (AD). METHODS: We conducted a retrospective cohort study among nursing home residents diagnosed with AD and psychosis during January 1998 to October 1999. Using the Medicare Minimum Data Set (MDS) and Medicare inpatient claims (ICD-9 codes), 7728 nursing home residents aged 55-95 years with AD and psychosis were identified for study. Potential adverse events of interest were identified from the MDS and Medicare inpatient cla

Borderline personality features influence treatment response to suicide prevention.

BACKGROUND: Suicide is a notable risk for individuals with features of borderline personality disorder. Given the centrality of interpersonal difficulties in this disorder, we proposed that the negative interpersonal cognitions (perceived burdensomeness and thwarted belongingness) identified by the Interpersonal Theory of Suicide (IPTS) may explain the associations between suicidal ideation and borderline personality features. METHOD: Participants were 322 suicidal youth (74% girls) aged 11-18 years (M, SD = 14.74, 1.6) in an intensive outpatient program in the southwest United States. Youth

The anatomy of a personal health agent.

Large language models are enabling a new generation of health agents, yet their ability to address the diverse needs of individuals in everyday health settings remains underexplored1,2. Here we developed a comprehensive personal health agent that reasons over multimodal data from consumer wellness devices and medical records to provide personalized health support. Through analysis of web search and health forum queries, together with user-centred research involving end users and health experts, we identified four key user journeys: general health knowledge, personal data interpretation, wellne

Combining moderators to identify clinical profiles of patients who will, and will not, benefit from aripiprazole augmentation for treatment resistant late-life major depressive disorder.

Personalizing treatment for late-life depression requires identifying and integrating information from multiple factors that influence treatment efficacy (moderators). We performed exploratory moderator analyses using data from a multi-site, randomized, placebo-controlled, double-blind trial of aripiprazole augmentation. Patients (n = 159) aged ≥60 years had major depressive disorder that failed to remit with venlafaxine monotherapy. We examined effect sizes of 39 potential moderators of aripiprazole (vs. placebo) augmentation efficacy using the outcome of percentage reduction in depressive sy

MeDeMSA care study protocol: developing personalized best medical care with integrated mobile palliative and telemedicine support for individuals with multiple system atrophy.

Multiple system atrophy (MSA) represents a management challenge due to its variable clinical presentation, and lack of established multidisciplinary care protocols. In advanced stages, physical barriers often prevent access to specialized care, leaving patients and caregivers to face complications and fear alone. This 18-month, monocentric, open-label study evaluates the impact of a personalized, multidisciplinary treatment, integrating mobile palliative care, on the quality of life (QoL, measured by EQ-5D-5L indices) of MSA individuals compared to a matched historical European MSA cohort. For

CDH1 pathogenic variants and cancer risk in an unselected patient population.

CDH1 pathogenic variants confer a markedly elevated lifetime risk of developing diffuse gastric cancer (DGC) and lobular breast cancer (LBC). The aim of this study was to evaluate the prevalence and clinical impact of CDH1 pathogenic variants in the unselected and ancestrally diverse BioMe Biobank. We evaluated exome sequence data from 30,223 adult BioMe participants to identify CDH1 positive individuals, defined as those harboring a variant previously classified as pathogenic or likely pathogenic or a predicted loss-of-function variant in CDH1. We reviewed electronic health records and BioMe

Compulsive buying disorder clustering based on sex, age, onset and personality traits.

UNLABELLED: In spite of the revived interest in compulsive buying disorder (CBD), its classification into the contemporary nosologic systems continues to be debated, and scarce studies have addressed heterogeneity in the clinical phenotype through methodologies based on a person-centered approach. OBJECTIVES: To identify empirical clusters of CBD employing personality traits, as well as patients' sex, age and the age of CBD onset as indicators. METHODS: An agglomerative hierarchical clustering method defining a combination of the Schwarz Bayesian Information Criterion and log-likelihood was

The standpoint of persons with hearing loss on work disparities and workplace accommodations.

OBJECTIVES: The perspectives of persons who live and work with hearing loss were sought to examine workplace accommodation challenges and strategies. PARTICIPANTS: A convenience sample of seven older adults with hearing loss participated in in-depth interviews. METHODS: A systematic grounded theory approach informed the study design and analysis. Categories of facilitators and challenges in the data were identified through axial coding and clustering. Core categories of social processes emerged through constant comparison and theoretical sampling of the data to reveal the actions and interac

Healthcare complexities during community crises: Recommendation for access to healthcare for Australians with multiple sclerosis.

BACKGROUND: In 2019 and 2020, over 17 million hectares of Australia burned, and half of the Australian population was affected by toxic bushfire smoke. Then in 2020, restrictions designed to curtail the spread of COVID-19 resulted in significant changes to healthcare access. There is no Australian emergency management standard for persons with disabilities, including those with multiple sclerosis (MS). Persons with MS often require multidisciplinary and complex care, with continuity of treatment essential to prevent disease progression. OBJECTIVE: To identify limitations in access to healthca

Preventive screening for intracranial aneurysms.

BACKGROUND: Subarachnoid hemorrhage from rupture of an intracranial aneurysm (aneurysmal subarachnoid hemorrhage) is a devastating subset of stroke. Since brain damage from the initial hemorrhage is a major cause for the poor outcome after aneurysmal subarachnoid hemorrhage, prevention of aneurysmal subarachnoid hemorrhage has the highest potential to prevent poor outcome from aneurysmal subarachnoid hemorrhage. AIM: In this review, we describe the groups at high risk of aneurysmal subarachnoid hemorrhage who may benefit from preventive screening for unruptured intracranial aneurysms followed

Effects of alexithymia on the activity of the anterior and posterior areas of the cortex of the right hemisphere in positive and negative emotional activation.

The aim of the present work was to assess the effects of alexithymia (a psychoemotional disorder associated with difficulties in identifying and describing personal feelings) on regional activity in the cerebral cortex during positive and negative emotional activation in a non-clinical set of subjects. EEG (62-channel) recordings were made during presentation of special emotional video clips to alexithymics (17 subjects) and controls (27 subjects). Alexithymia was found to influence EEG activity in the theta-2, alpha-1, and alpha-2 ranges; alexithymics showed greater reactivity in the anterior

Drinking, cannabis use and mental health as predictors of within-person changes in aggression over eight weeks among youth involved with binge drinking.

INTRODUCTION: Sequential, multiple assignment, randomized trials (SMARTs) are used to deliver behavior interventions to adolescents and emerging adults (A/EAs) involved with alcohol use and aggression. We examine with-in person markers of aggression from a recent SMART to inform future interventions for this population. METHODS: A/EAs (n = 400; ages 14-20; 22 % male) in the emergency department who reported binge drinking and aggression were enrolled in the SMART, completed a baseline survey, received a brief behavioral health intervention (SafERteens), were randomized to booster conditions o

Construct validity of the MMPI-2 College Maladjustment (Mt) Scale.

The construct validity of the MMPI-2 (Minnesota Multiphasic Personality Inventory-2) College Maladjustment (Mt) Scale was examined using 376 student clients at a university psychological clinic. A principal components analysis and correlations of Mt scale scores with clients' and therapists' ratings of symptoms and functioning showed that the Mt scale identifies the presence of maladjustment as defined in terms of depressive and anxious symptoms. There is no evidence to show that the scale is specific to college students or that it is sensitive to severe psychological disturbance. The Mt scale

Unhappiness and dissatisfaction in doctors cannot be predicted by selectors from medical school application forms: a prospective, longitudinal study.

BACKGROUND: Personal statements and referees' reports are widely used on medical school application forms, particularly in the UK, to assess the suitability of candidates for a career in medicine. However there are few studies which assess the validity of such information for predicting unhappiness or dissatisfaction with a career in medicine. Here we combine data from a long-term prospective study of medical student selection and training, with an experimental approach in which a large number of assessors used a paired comparison technique to predict outcome. METHODS: Data from a large-scale

Consumer and Health Care Professional Perspectives on a Digital Platform to Support Preventive Antenatal Care: Qualitative Descriptive Study.

BACKGROUND: Modifiable health risks in pregnancy are common and contribute to adverse maternal and infant outcomes, yet preventive care addressing these risks can be difficult to deliver within time-limited antenatal appointments. Hybrid models combining in-person care with digital platforms offer a promising solution; however, there is limited evidence regarding consumer and health care professional perspectives on the preferred content and functionality of such platforms to inform their design. OBJECTIVE: This study aimed to explore consumer and health care professional perspectives on the

Eating Disorders and Therapist Emotional Responses.

The aims of this study were to identify (a) patterns of clinicians' emotional responses to patients with eating disorders (ED); (b) patient, clinician, and treatment variables associated with therapist emotional responses; and (c) the influence of patient personality on therapist emotional responses. A random national sample of psychodynamic and cognitive-behavioral psychotherapists (N = 149) was asked to examine one patient (>18 years old) with an ED. Clinicians completed the SWAP-200, the Therapist Response Questionnaire, and the Clinical Questionnaire-Eating Disorder Form to provide general

Childhood maltreatment and personality disorders in the USA: specificity of effects and the impact of gender.

Childhood maltreatment increases the risk for adult personality disorders (PDs), but several PDs or maltreatment types co-occur. Specificity of maltreatment-personality associations is poorly understood. Using a representative US population sample, we identified specific associations between maltreatment types (sexual, physical and emotional abuse and physical and emotional neglect) and PDs after controlling for basic demographics, parental psychopathology, co-occurring maltreatment types and comorbid PD. We then examined interactions of gender and maltreatment in predicting PDs. Each maltreat

Diagnostic validity of basic symptoms.

Although the Bonn Scale for the Assessment of Basic Symptoms (BSABS) [13] has come into use in several European countries, its diagnostic validity has not yet been sufficiently examined. That is why we have assessed BSABS items on a sample of 243 consecutive admissions to the Department of Psychiatry at the RWTH University, Aachen, and 79 psychologically healthy persons. Then, a cluster analysis was calculated to identify the empirical item-grouping. Five well-interpretable BSABS subsyndromes were found. In addition, uni- and multivariate analyses were computed to evaluate the diagnostic valid
