Daily light exposure habits of youth with migraine: A prospective pilot study.

BACKGROUND: Eighty percent of youth with migraine report photophobia. It is unknown if photophobia leads to light avoidant behavior, and if such behaviors worsen light sensitivity and disrupt sleep. Recently developed wearable, continuous light loggers allow us to address these open questions. We conducted a pilot study to determine the feasibility of measuring light exposure using wearable light loggers in youth with migraine. METHODS: Youth 10 - 21 years old with a headache-specialist confirmed ICHD-3 diagnosis of migraine were recruited from CHOP headache clinics. Each participant recorded…

In Vivo Confocal Microscopy and Anterior Segment Optical Coherence Tomography Features of Corneal Pseudodendritic Lesions in Hereditary Tyrosinemia Type 1.

BACKGROUND Tyrosinemia is a metabolic disorder leading to hepatic, renal, and ocular involvement. Ocular manifestations, such as photophobia and pseudodendritic keratitis, can mimic herpetic keratitis without response to antiviral therapy but improve with metabolic intake and appropriate drugs. Although rare, it poses significant challenges, needing a multidisciplinary approach. CASE REPORT A 14-year-old girl with a diagnosis of type 1 hereditary tyrosinemia was evaluated at our tertiary center for persistent photophobia and bilateral ocular discomfort. Previously, a presumptive diagnosis of r…

A rare complication of topical atropine for treatment of eccrine hidrocystoma: Pharmacological fixed dilated pupils.

Multiple eccrine hidrocystoma (EH) has been treated with topical atropine with variable results. However, in rare cases, anticholinergic side effects have been seen after the use of the topical form of this drug. We presented a 50-year-old woman who developed recent onset of visual disturbance and photophobia from 2 weeks prior. The diagnosis of topical atropine-induced bilateral mydriasis was made. We reported a recognized but often overlooked case of bilateral mydriasis caused by application of topical 1% atropine for treatment of multiple EH.

[Optical coherence tomography in the diagnosis of achromatopsia].

CASE REPORT: The case of a fifty five year-old male with nyctalopia, photophobia, poor colour vision and nystagmus, is presented. The initial suspected diagnoses were achromatopsia and blue-cone monochromatism, since both are clinically indistinguishable. Optical coherence tomography (OCT) showed the characteristic foveal reflectivity pattern of achromatopsia. This diagnosis was subsequently confirmed by genetic study. DISCUSSION: OCT is a non-invasive diagnostic imaging method that allows tissue morphology to be observed with high resolution. Its use might be of great help to distinguish cli…

[Hydroxypropylmethyl-containing eyedrops in the treatment of sicca syndrome].

The author reports on 16 patients (9 with keratoconjunctivitis sicca, 7 with Sjögren's syndrome) treated with Isopto-Naturale eye drops for 10 months. Reduction of photophobia, burning, dryness and foreign-body sensation were achieved by local administration of the drug 2 to 5 times a day. The lesions in the corneal epithelium also improved significantly during the treatment period. Thus, Isopto-Naturale eye drops have proved to be more effective than 1% methylcellulose or 1.4% PVA artificial tear eye drops in the treatment of dry-eye syndrome.

Aversive light sensitivity and aging: Implications for sleep and health in older adults.

The current digital survey study investigated the role of aversive visual light sensitivity (AVLS) in sleep and health in a sample of 425 community-dwelling older adults (75.5 % female, mean age = 68.9 ± 5.6). Respondents who reported photophobia or eye diseases/surgery were excluded. The Visual Sensitivity Scale Questionnaire (VLSQ-8) was used to quantify the AVLS symptoms. The distribution of the VLSQ-8 scores indicated prevalence of mild AVLS symptoms in the sample, with median close to the lower end of the scale (median = 14, IQR = 9, scale range 8-40). Females reported significantly highe…

Understanding the subtypes of visual hypersensitivity: Four coherent factors and their measurement with the Cardiff Hypersensitivity Scale (CHYPS).

Subjective visual sensitivity or discomfort has been reported in many separate literatures, and includes a wide range of visual triggers (e.g., repeating patterns, bright lights, motion, flicker) across a wide range of neurological, psychiatric, mental health, and developmental conditions and areas of neurodiversity (e.g., migraine, traumatic brain injury, functional neurological disorder, PPPD, PTSD, anxiety, depression, anorexia, OCD, autism, ADHD, dyslexia, dyspraxia, synaesthesia). To unite this research across disciplines and to allow progress in mechanistic understanding, we aimed to pro

Complicated corneal ulcer. Case report.

Corneal ulcers are considered an ophthalmologic emergency because of their potential to permanently impair vision or perforate the eye. The therapeutic management includes medical therapy and in case of failure, surgical care such as amniotic membrane transplantation. We present the case of a 76-year-old male, admitted for sudden visual loss in the left eye, associated with ocular pain, tearing, and photophobia. The patient was diagnosed with superficial ulcerative keratitis with hypopyon and acute exogenous anterior uveitis for which he underwent medical treatment, both general and topic, wit…

Headache associated with moyamoya disease: a case story and literature review.

Headache associated with moyamoya disease (HAMD) is common in moyamoya disease. However, the characteristics and classification of HAMD are largely unknown. We present a case of a 39-year-old woman with HAMD. To characterize and classify the features of this syndrome, the patient was asked to complete a 4-month diagnostic headache diary. There was a total of 15 ictal days. All episodes were without aura. The headache was more commonly pressing (10/15), mild to moderate in severity (14/15), unchanged by physical activity (11/15), and associated with photophobia (10/15). The International Headac…

Progressive cone dystrophy and sensorineural hearing loss.

A 39-year old man presented 13 years ago with a history of progressive loss of vision and photophobia. A full ophthalmological and ENT work-up during several years of follow-up, including psychophysical as well as electrophysiological tests, revealed a progressive cone dystrophy in combination with sensorineural hearing loss. His younger sister presented with very similar features and underwent the same work-up. A novel syndrome of progressive cone dystrophy and sensorineural hearing loss is described in both siblings. Both also suffered from non-ocular disease possibly related to ciliary dysf…
