BACKGROUND: Carotid endarterectomy is usually delayed for 2 months following an acute stroke, but the stroke may progress or a further stroke may occur. A randomized pilot study of urgent carotid surgery for acute stroke was undertaken to assess the feasibility of a definitive multicentre trial.

In a pilot open-labeled study 10 subjects with AIDS dementia complex (ADC) were treated with didanosine. Only half of the subjects were able to complete the trial as a result of side effects. Five subjects exhibited improved performance on neuropsychological testing, but the mean change in performance in this small group was not statistically significant. The study suggests that this drug may have some value in ADC patients unable to tolerate other therapies, but that further study is needed to establish this firmly.

OBJECTIVES: To determine the effectiveness of azithromycin, an azalide antibiotic with long tissue half-life, in a pilot study of patients with genital donovanosis in the Northern Territory, Australia.
DESIGN: Patients with histologically confirmed donovanosis were randomised to receive one of two open-label azithromycin dosage regimens: Regimen A--1.0 g once weekly for 4 weeks; or Regimen B--500 mg daily for 7 days. Sixteen patients with advanced metastatic malignant melanoma were treated with a high-dose infusion of interleukin-2 (IL-2; 18 x 10(6) IU/m-2 day-1) together with daily subcutaneous (s.c.) injections of interferon alpha (IFN alpha; 3 x 10(6) U/m-2 day-1) in 5-day cycles. We evaluated fluoxetine in six patients with poorly controlled cataplexy by beginning patients on a single 20 mg dose without any change in their current medications. Once benefit occurred, we discontinued or reduced other anticataplectic medication. Reduction in the number of cataplectic episodes per week after 20 weeks of treatment constituted the basis for efficacy determination. The efficacy of a single dose of MCNU 150 mg was evaluated in nine symptomatic patients with essential thrombocythemia (ET). One hundred and ten radiographs of calcaneum were reviewed for postmenopausal osteoporosis in Saudi women. These patients had presented to the emergency room with unrelated complaints. The mean (+/- SD) age of this group was 58 (+/-8.9) years with a range of 45 to 80 years. Of these eighty-six (76%) patients had osteoporosis; of these, 42 had mild osteoporosis, 31 had frank osteoporosis, and 11 were diagnosed to have sever osteoporosis. Twenty-eight patients with ICD-10 bulimia nervosa or atypical bulimia nervosa were given a cognitive-behavioral treatment handbook and were reassessed 4-6 weeks later. On a clinician-rated global improvement scale 12 patients had much improved and 8 patients had somewhat improved. Twenty-seven wives who care for their spouses suffering from dementia completed the Anticipatory Grief Scale (AGS) and the Hopkins Symptom Checklist--SCL-90-R. The alpha coefficient for the AGS was 0.84, indicating good internal consistency. The AGS was also positively and significantly correlated (p less than 0.001) with the depression, anxiety, and hostility dimensions of the SCL-90-R, demonstrating good validity. The study explores bereavement issues of spouses of patients with dementia and provides preliminary findings on a new instrument to measure anticipatory grief.

A vascularized tube flap of tunica vaginalis was used to replace the penile urethra in 12 rabbits. Ten flaps were available for assessment, 2 flaps became necrotic as a result of compression of the vascular pedicle and 1 rabbit had a proximal anastomotic stricture. Mechanical retinal fixation is a useful tool in the armamentarium of vitreoretinal surgeons. Unfortunately, the use of metallic retinal tacks on a permanent basic mandates a retained intraocular foreign body that poses some potential for long-range toxic effects. The aim of the study was to discover the relative effectiveness of four methods of treating patients suffering from hyperventilation attacks. The methods used were: breathing and relaxation therapy, hyperventilation provocation training, treatment based on influencing the factors which lead to the development of the syndrome, and drug treatment. The first three methods were demonstrated to be effective, whilst the fourth showed no signs of effectiveness. The condition of patients in the control group remained the same or deteriorated.
