Adult granulosa cell tumor presenting as metastases to the pleural and peritoneal cavity.

Ovarian tumors of sex-cord stromal derivation rarely exfoliate malignant cells in serous effusions. Adult granulosa cell tumor (AGCT) often presents as a stage I disease and association with ascites is about 10%. AGCT is rarely known to exfoliate tumor cells in the ascitic fluid. Exfoliation of malignant cells in the pleural fluid is rarer and has not been described previously in literature to the best of our knowledge. We report a case of AGCT presenting as stage IV disease with exfoliation of malignant cells in the pleural and ascitic fluid with exudative effusions. We wish to emphasize on c

Radiographic evaluation of asbestos-related chest disorders.

This paper reviews the radiographic, clinical, pathologic, and epidemiological features of pleural and pulmonary parenchymal disorders which have been related to asbestos exposure. In particular, the following are discussed: (1) pleural plaques--radiographic detection by plain films and computed tomography, normal and abnormal densities which may mimic plaques, the 1980 ILO U/C classification, recent epidemiological data on plaques including their relationship to carcinoma and mesothelioma; (2) diffuse pleural thickening; (3) benign asbestos pleural effusions; (4) mesothelioma--emphasizing rec

[Pathology of primary tumours of the pleura].

In the WHO classification, pleural tumours of mesothelial and mesenchymal origin as well as lymphoproliferative disorders are distinguished, with malignant mesotheliomas forming the most common primary pleural tumour. Histologically, a prognostically relevant differentiation between epitheloid (40-60 %), sarcomatoid (20-30 %), and biphasic mesotheliomas (20-30 %) is made. Besides other rare primary tumours of the pleura, the differential diagnoses of malignant mesotheliomas comprise carcinomatous pleuritis of intra- and extrapulmonary malignancies. Especially in early tumour stages, the differ

Clinical implications of unexpandable lung due to pleural disease.

Unexpandable lung due to pleural disease may manifest itself as a hydropneumothorax after pleural drainage procedure or as an inability to completely drain a pleural effusion due to chest pain. The condition is a mechanical complication of a variety of pleural disorders. Of these, malignant lung entrapment and inflammatory lung entrapment are considered complications of active pleural disease, and management is primarily dependent on the nature of the active process. Trapped lung is a sequela of remote inflammation of the pleural space. Trapped lung is usually asymptomatic but may be the cause

[Non-malignant cells in pleural fluid].

Insufficient attention has been paid to the cytological examination of cellular elements of the pleural fluid besides or in the absence of tumour cells. 108 cases studied due to pleural fluid were selected. They were characterized by the following: 1. each had histological diagnosis established by either pleuroscopy or by pleural needle biopsy and 2. neither of the samples contained tumour cells. Relationship of the dominant cell type and the ethiology of the pleural fluid was analysed. In case of punctates rich in lymphocytes further cytopathological differentiation is possible, e.g. in case

[Gamma interferon and adenosine deaminase in pleuritis].

The concentration of gamma interferon (IFN gamma) and adenosine deaminase (ADA) activity were measured in the pleural fluid of 162 patients to compare their diagnostic significance and to establish a possible correlation between both tests. The IFN gamma levels in tuberculous pleural effusions were quite variable, with a mean of 93 U/ml and a median of 48 U/ml. They were higher than 2 U/ml in all cases, whereas no case of nontuberculous effusion showed higher values. ADA activity was higher than 43 U/l in all tuberculous effusions, with a mean value of 80 U/l, higher than in any other group ex

Pleural effusion as a sample matrix for laboratory analyses in cancer management: a perspective.

Malignant effusions, pleural effusion or ascites, represent a common problem in cancer patients. Pleural effusion in a cancer patient may be caused also by non-neoplastic conditions, and the diagnosis of malignant pleural effusion is established by the demonstration of tumor cells in pleural fluid. Microscopical detection of tumor cells in pleural fluid often fails, and there is an unmet medical need for more sensitive methods. New approaches, including isolation using magnetic beads coated with monoclonal antibodies targeting antigens expressed on tumor cells not only increase the diagnostic

Pleural lymphoma combined with pleural sarcoidosis.

A patient with sarcoidosis was found to have a massive left pleural effusion. Her chest computed tomography showed small nodules in the lung parenchyma and swelling of the hilar lymph nodes, with normal visceral and parietal pleura. Thoracoscopy showed white nodules on the visceral pleura and normal parietal pleura, which were resected. Epithelioid granulomas were seen in the visceral pleura and lung parenchyma. Surprisingly, in the parietal pleura, abnormal cells that were positive for the leukocyte common antigen, CD20, and CD79a were found, leading to the diagnosis of malignant B-cell lymph

[A case of pyothorax-associated pleural angiosarcoma diagnosed by autopsy].

A 76-year-old man with a dry cough visited our hospital in June 2006. A chest X-ray showed opacification of the left hemithorax and CT demonstrated a soft tissue mass with pleural calcification. At first, we considered he had acute bronchitis with an old tuberculous pyothorax. But, his condition deteriorated with the additional complaint of a left chest pain and shortness of breath in September 2006. Consequently, he was admitted to our hospital. CT demonstrated that the soft tissue mass was growing and was invading the left rib and submammary tissue. Neither CT nor sonographically guided fine

Tract seeding in indwelling pleural catheter placement for the drainage of malignant pleural effusions: Incidence and related clinical and imaging factors.

BACKGROUND: The incidence of tract seeding after the placement of indwelling pleural catheter (IPC) for malignant pleural effusion drainage has been variable in the literature. RESEARCH QUESTION: To evaluate the incidence of IPC-related cancer tract seeding and find out related demographic, clinical or imaging factors to the tract seeding. STUDY DESIGN AND METHODS: This retrospective study included 124 consecutive patients seen between January 2011 and December 2021 who underwent IPC placement for malignant pleural effusion drainage. Chest radiographs before IPC placement and serial chest CT

[Pleural biopsies: complications and current value of Abram's needle biopsy. Apropos of 1,000 samples. Value of pleuroscopic biopsy].

The frequency of incidents resulting from Abrams needle biopsy is very low provided certain contraindications are respected : pneumothorax, 3.1% of cases ; hemorrhages, 1.3% ; neoplastic contamination of the bioptic tract, 1.9%. Serious accidents are rare. We only observed 2 serious hemorrhages, one in 1964, the other in 1968. In the latter case, the pleural hemorrhage caused a very serious respiratory insufficiency. The delayed functional reaction is still not well known. However the occurrence of immediate accidents could reflect on radiological sequelae. The value of this examination seems

Gelatinous Pleural Effusion: A Diagnostic Challenge for Pleural Mesothelioma in an 80-Year-Old Man.

BACKGROUND Gelatinous pleural effusion, due to raised hyaluronic acid, can be associated with pleural infection and malignancies, such as tuberculosis, metastatic pleural disease, and mesothelioma. This report is of an 80-year-old man presenting with a gelatinous pleural effusion and diagnosis of pleural mesothelioma. CASE REPORT An 80-year-old man with diabetes mellitus, ischemic heart disease, metastatic prostate cancer, 30-pack-year smoking history, and 5-year history of asbestos exposure (during his 30s), presented with a 4-week history of breathlessness and was found to have right-sided p
