CD8+ T-cell immunity in chronic inflammatory demyelinating polyradiculoneuropathy.

OBJECTIVE: Chronic inflammatory demyelinating polyradiculopathy (CIDP) is a common, but often misdiagnosed disease of the peripheral nervous system with assumed autoimmune pathogenesis. While current concepts of CIDP postulate a pathogenetic role of B cells and (auto)antibodies, the relevance of CD8 T cells present in the biopsies is still elusive. Thus, we asked whether nervous tissue infiltrating and blood-derived lymphocytes in CIDP are clonally expanded to evaluate the involvement of T cells in the pathogenesis of the disease. METHODS: We characterized the clonal composition of the T-cell…

Post-void bladder ultrasound in suspected cauda equina syndrome-data from medicolegal cases and relevance to magnetic resonance imaging scanning.

OBJECTIVE: Post-void residual (PVR) scans of less than 200 ml are increasingly being used to rule out the likelihood of cauda equina syndrome (CES) and to delay emergency MRI scanning in suspected cases. This study was done to review a series of 50 MRI confirmed cases of CES and to test the hypothesis that a PVR of less than 200 ml was unlikely to be present. METHODS: Fifty consecutive medicolegal cases involving CES were audited. Records were reviewed to see if PVR scans were done. MRI scans were reviewed, clinical and radiological diagnosis reviewed, and treatment recorded. RESULTS: Out of

Factors affecting urinary outcome after delayed decompression in complete cauda equina syndrome: "A regression model study".

PURPOSE: To evaluate the recovery of urinary functions and the factors predicting urinary recovery, following delayed decompression in complete cauda equina syndrome (CESR) secondary to Lumbar disc herniation (LDH). METHODS: Retrospective study evaluated 19 cases of CESR due to single-level LDH, all presenting beyond 72 h. Mean delay in decompression was 11.16 ± 7.59 days and follow-up of 31.71 ± 13.90 months. Urinary outcomes were analysed on two scales, a 4-tier ordinal and a dichotomous scale. Logistic regression analysis was used for various predictors including delay in decompression, ag

Cauda equina syndrome: is the current management of patients presenting to district general hospitals fit for purpose? A personal view based on a review of the literature and a medicolegal experience.

There is no universally agreed definition of cauda equina syndrome (CES). Clinical signs of CES including direct rectal examination (DRE) do not reliably correlate with cauda equina (CE) compression on MRI. Clinical assessment only becomes reliable if there are symptoms/signs of late, often irreversible, CES. The only reliable way of including or excluding CES is to perform MRI on all patients with suspected CES. If the diagnosis is being considered, MRI should ideally be performed locally in the District General Hospitals within one hour of the question being raised irrespective of the hour o

Assessing and managing patients with cauda equina syndrome.

Cauda equina syndrome is a surgical emergency that requires prompt assessment and swift surgical intervention. Compression of the cauda equina can result in loss of power and sensation to the lower limbs, back pain and loss of bladder and bowel function. Research regarding timing of surgery and surgical outcome continues to be debated. Nurses have an important role in recognising symptoms of cauda equina syndrome and supporting the patient and effectively managing care based on individual healthcare needs.

Timing of surgical intervention in cauda equina syndrome: a systematic critical review.

OBJECTIVE: Cauda equina syndrome (CES) is a rare but important neurosurgical emergency. Despite being a recognized clinical entity since 1934, there remains significant uncertainty in the literature regarding the urgency for surgical intervention. The past decade has seen the emergence of the much-referred-to 48-hour limit as a possible window of safety. The ramifications of this time point are significant for early patients who may subsequently have urgent treatment delayed, and for litigation cases, after which adverse decisions are more likely to occur. METHODS: A systematic principally qu

Cauda equina syndrome presenting as abdominal pain: a case report.

Cauda equina syndrome (CES) is an uncommon entity. Symptoms include bowel and bladder dysfunction, saddle anesthesia, and varying degrees of lower limb motor and sensory disturbances. The consequences of delayed diagnosis can be devastating, resulting in bowel and bladder incontinence and lower limb paralysis. There is little in literature regarding abdominal pain as a significant feature of the initial presentation of CES. We present the case of a 32-year-old woman with CES who presented to the emergency department with gradually worsening lower abdominal pain.

[Cauda equina syndrome leading to diagnosis of malignant non-Hodgkin lymphoma of the spine].

INTRODUCTION: Malignant non-Hodgkin lymphoma is a lymphoid proliferation. Peripheral neuropathies are observed in only 2-8% of cases. CASE: We report a case in which cauda equina syndrome in a 29-year-old man led to the diagnosis of a painless non-Hodgkin lymphoma of the spine. Combined chemotherapy and radiation therapy produced spectacular improvement. DISCUSSION: Infiltration of the cauda equina roots is rare in malignant non-Hodgkin lymphoma and raises problems for positive diagnosis. The good outcome under treatment underlines the interest in considering this cause in disorders affectin

Cauda equina syndrome due to leptomeningeal carcinomatosis of the ovary.

BACKGROUND: Serous cystadenocarcinoma is the most common malignant ovarian tumor. 85% are associated with extraovarian spread at the time of diagnosis. Cauda equina syndrome due to leptomeningeal ovarian serous cystadenocarcinomatosis is rare. CASE: A 66-year-old female with stage IV ovarian papillary serous cystadenocarcinoma presented with perianal numbness and sphincter dysfunction. On exam she had decreased anal tone with saddle anesthesia. Her MRI did not demonstrate any leptomeningeal involvement. CSF showed malignant cells consistent with metastatic ovarian adenocarcinoma. She received

Primary liposarcoma of the lumbar spine.

BACKGROUND: We describe a rare case of the cauda equina syndrome caused by a primary pleomorphic liposarcoma of the lumbar spine. CASE REPORT: A 35-year-old man presented a 2-month history of back pain and slowly progressive weakness of lower limbs without sphincter dysfunction. It had a laminectomy for a supposed disc herniation three years previously. A diagnosis of recurrence hernia or fibrosis was discussed. RESULTS: Magnetic Resonance Imaging (MRI) and CT scan of the lumbar vertebral column shows diffuse involvement of L4/L5 vertebral bodies, and an intradural lesion surrounding the ner

Cauda equina syndrome secondary to lumbar disc herniation.

The authors conducted a retrospective study of 8 cases of cauda equina syndrome (CES) due to a herniated lumbar intervertebral disc, in order to highlight the clinical presentation and outcome after urgent surgical decompression. Between 1995 and 1999, 8 patients (4 men and 4 women) with ages ranging from 43 to 64 years (mean 53 years) presented with CES. There were two modes of presentation: acute to subacute (4 patients) and insidious (4 patients). In 3 patients the precipitating factor was stooping, with or without heavy lifting. One patient gave a history of stooping and heavy lifting, fol

Peripheral neuropathies associated with HIV and hepatitis C co-infection: a review.

Co-infection with HIV and hepatitis C has become increasingly prevalent. It is a major source of morbidity in HIV-infected populations. Distal symmetric polyneuropathy is the most common form of peripheral neuropathy in HIV as well as hepatitis C mono-infection. There is considerable overlap in the symptoms and signs of HIV and hepatitis C neuropathy. It is not known whether there are additive or synergistic effects on the peripheral nerve by these two viruses. There is a need for studies to further elucidate the mechanisms involved.

Synovial cysts and the lithotomy position causing cauda equina syndrome.

We describe a case of cauda equina syndrome caused by synovial cysts and the lithotomy position. A transurethral resection of the prostate was performed under spinal anesthesia in the lithotomy position. We believe that this is the first case report of facet joint synovial cysts and the lithotomy position causing ischemic neurologic injury to the cauda equina. Other etiologies such as needle trauma, neurotoxicity, hematoma, and abscess were not evident. We believe that positioning the patient in the lithotomy position narrowed the cross-sectional area of the spinal canal in a patient with a co

Cauda equina syndrome as a postoperative complication of lumbar spine surgery.

BACKGROUND: Musculoskeletal disorders are one of the most common causes that lead to disability throughout the world and routinely present with only limited or short-term symptom relief following traditional treatments. Recent developments, such as platelet-rich plasma (PRP) and mesenchymal stem/stromal cell (MSC) interventions, are being used as regenerative biologic adjuncts in musculoskeletal medicine, yet significant variability exists in preparation strategies, activation methods, and dosing protocols. In addition, clinical interpretation remains limited by inconsistent biologic character

Cauda equina syndrome caused by primary and metastatic neoplasms.

Cauda equina syndrome (CES) is defined as the constellation of symptoms that includes low-back pain, sciatica, saddle anesthesia, decreased rectal tone and perineal reflexes, bowel and bladder dysfunction, and variable amounts of lower-extremity weakness. There are several causes of this syndrome including trauma, central disc protrusion, hemorrhage, and neoplastic invasion. In this manuscript the authors reviewed CES in the setting of both primary and secondary neoplasms. They examined the various primary tumor types in this region as well as those representative of metastatic spread. Both su
