The effect of phlebotomy on 44 patients (33 men and 11 women) with porphyria cutanea tarda (PCT) is described. All had typical biochemical and clinical signs of PCT. The patients were bled until slight anemia and signs of iron deficiency. In patients who were bled frequently and who were not given iron supplements in conjunction with phlebotomy, urinary uroporphyrin excretion decreased to 1 mg (or 1.2 mumol) a day or less in 1-8 months (mean 4 months) and then continued to decrease long after the treatment had been stopped and became normal in many patients. Revision of 167 bioptic cases of porphyria cutanea tarda failed to confirm porphyrin crystals in diseased hepatocytes. The only peculiar finding was a fine honey-comb microvacuolisation of hepatocytes corresponding to dilated sacs of degranulated endoplasmatic reticulum in electron microscopy. The lesion was closely connected with regressive changes of mitochondria which often were giant and sometimes unspecifically paracrystalline in their inner structure. Steatosis and haemosiderosis were frequent and taken for secondary as well as the hepatic proliferation in terminal fibrosis or cirrhosis.

A patient who developed porphyria cutanea tarda and disturbed liver function tests following treatment with rifampicin and isoniazid for a tuberculous psoas abscess is reported. The patient had normal liver function tests prior to receiving anti-tuberculosis chemotherapy, but was subsequently demonstrated to have cholelithiasis. Six patients with skin changes and urinary porphyrin excretion patterns characteristic for porphyria cutanea tarda were treated with hydroxychloroquine sulfate therapy. During treatment periods ranging from five to 13 months, cutaneous symptoms disappeared and urinary porphyrin excretion abnormalities were completely or almost completely reversed. In three subjects, hydroxychloroquine therapy was accompanied by changes in the urinary excretion of iron. Variegate porphyria (VP) is a rare disease infrequently diagnosed in the United States. The typical cutaneous signs of VP, including fragility, bullae, scarring, hypertrichosis, and photosensitivity, are indistinguishable from those of porphyria cutanea tarda (PCT). The dangerous life-threatening drugs have been identified in 153 acute attacks in 138 porphyric patients by establishing a consistent temporal relationship between the administration of a specific drug and the development of an acute attack. The drugs so identified have been supplemented by a number of others found to be porphyrogenic by testing with a recently devised rat model. Lists of the dangerous and the safe drugs have been compiled.

Porphyria cutanea tarda (PCT) has a known increased incidence of diabetes mellitus and hepatic involvement. We investigated glucose tolerance and glucoregulatory hormone alterations in seven patients with PCT and correlated these results with hepatic histology by percutaneous liver biopsy. Abnormal glucose tolerance was observed in six of the seven patients (87%).
