Sickle cell disease is a very common entity in central India, especially in Maharashtra. Patients usually present with recurrent haemolytic and/or vaso-occlusive crises. Although unusual, Priapism i.e. spontaneous painful erection of penis can be a debilitating manifestation of crisis. Stuttering priapism is a clinical phenomenon that occurs commonly in certain patient populations, including sickle cell anemia and other hematologic dyscrasias. Although the mechanism is still not completely understood, treatment is focused on prevention of recurrence in the outpatient setting, and immediate detumescence and minimizing corporal fibrosis in the acute setting. We present a case of stuttering priapism in a 44 year-old male with hereditary spherocytosis and discuss the pathophysiology and clinical management of this entity.

We present the first case of priapism following radical prostatectomy. A 66-year-old man with normal erections underwent radical retropubic prostatectomy with unilateral nerve sparing. Pathology showed a pT2c pN0 Gleason score 3 + 3 = 6 prostate cancer and the postoperative course was uneventful. Ten days after surgery he recognized a spontaneous painful penile erection without sexual stimulation which occurred in a standing position and disappeared in a supine position. A patient who presented with priapism after ingestion of alfuzosin is described. He presented with recurrent priapism, which was relieved temporarily after physical exercise. It did not subside with pharmacologic management and was managed surgically using Winter's procedure. It remained quiescent for a short period, only to reappear later. Priapism is a rare disease that can be secondary to a clotting disorder. The authors report the first published case of priapism due to a heterozygous mutation of factor V Leiden responsible for activated protein C resistance in a patient treated with low molecular weight heparin. A similar case has already been reported, but with a homozygous mutation. Priapism must therefore be considered to be a thromboembolic event and the presence of a clotting disorder should be systematically investigated in the absence of an obvious aetiology.

Priapism is an undue and sustained erection in the absence of sexual stimulus. In the elderly population drug induced, thromboembolic and malignant aetiologies are common. This report demonstrates that in elderly demented patients, penile prosthesis should be considered in the differential diagnosis of such a condition. A plain X-ray of the pelvic region incorporating the penis could prove very useful, particularly in the absence of a proper history.

A case is reported of a 35 year old man who sustained an injury to the perineum in a cycling accident which resulted in a traumatic priapism. After confirmation of the diagnosis by Doppler sonography and angiography, therapeutic selective arterial embolisation was followed by successful detumescence of the penis and subsequent return of normal erectile function. It is suggested that percutaneous embolisation of the lacerated cavernosal artery is a safe and effective minimally invasive treatment for this uncommon condition.

We report on the case of a 19-year-old man with post-traumatic high-flow priapism that appeared after riding on a mountain bike. Laboratory test (cavernous blood gas measurement) and image studies (color flow Doppler ultrasound and arteriography of the internal pudendal artery) that confirmed the diagnosis are depicted. He was successfully treated with selective percutaneous arterial embolization with autologous blood clot, recovering a normal erectile function.

OBJECTIVES: Priapism in the pediatric population is rare and most commonly occurs secondary to sickle cell disease or hematologic malignancy. We present a case of a 12-year-old boy with priapism who required aggressive surgical therapy for adequate detumescence. This patient had a recent viral upper respiratory infection and titers for Mycoplasma pneumoniae were indicative of infection. A case of priapism following oral intake of papaverine hydrochloride (300 mg) is reported. The priapism persisted after coitus. It resisted conservative treatment, but responded to aspiration of the corpora cavernosa and irrigation with phenylephrine solution. We found no earlier report of priapism induced by oral papaverine. The possible therapeutic role of oral papaverine in impotence merits study.
