Medical Team Communication on Family-Centered Rounds and Caregiver Outcomes.

BACKGROUND: Family-centered rounds (FCR) offer opportunities to partner with families but can be inequitable with variable outcomes. Defining teachable communication practices associated with optimal FCR may improve family experience and engagement. OBJECTIVE: To evaluate associations between clinician-team communication during FCR and caregiver outcomes in a racially and ethnically diverse population. METHODS: We audio recorded and transcribed FCR. We double coded clinician-communication behaviors, assigned global communication ratings, and counted caregiver participatory behaviors (partici

Building Therapeutic Alliances in Aphasia Rehabilitation: Developing Relational Competencies Through Narrative Practice.

OBJECTIVE: This study explored how graduate clinicians developed relational competencies while facilitating the My Story Project, an evidence-based narrative intervention in which clinicians support people with aphasia to co-construct personal life stories. METHODS: Using reflexive thematic analysis, focus group data from 11 graduate students in speech-language pathology who served as story coaches were analyzed. Each coach facilitated weekly virtual story co-construction sessions with one person with aphasia during a clinical practicum. Coaches received training in the relationship between n

Microprocesses predicting early emotional bond in videoconferencing and face-to-face psychotherapy.

Research on microprocesses underlying alliance formation remains limited compared to outcome-based studies, particularly in videoconferencing psychotherapy (VCP). This study examined therapist and client interaction patterns predicting early emotional bond in VCP and face-to-face therapy. A total of 56 early therapy sessions from 28 therapist-client dyads were coded with the Therapeutic Relationship Coding System, yielding 35,715 behaviors. Alliance strength was determined through an extreme-groups design based on the Bond subscale of the Working Alliance Inventory-Patient version. Bayesian lo

Beating an undead horse: Toward a conceptualization of nuisance in the analytic process.

Winnicott (1945a) suggested that some types of aggressive behaviour of the child returning from sustained separation from their parents may be regarded as an expression of hope, one in which they can yield their forms of defensive self-sufficiency to trust a parent again. The author parses Winnicott's later various approaches to understanding both instinctual and reactive aggression and how applying his later views can obfuscate the meaning of aggression in nuisance-making behaviour. The author offers a specific definition of nuisance in the analysis of adults and how it manifests itself in th

Childhood abuse, dissociation and hypnosis: Trauma-related transference.

This article presents how to apply psychodynamic theory of transference in the treatment of traumatized patients who have endured repeated early life trauma and a lack of relational safety. To make hypnosis safe, the author adapts her interventions to each patient's unique needs. Clinically relevant and experientially informed perspectives are provided, designed to support therapists in maintaining a safe hypnotic approach when patients relate to the therapist with negative transference, such as anger, hate, fear and distrust. In contrast to the popular focus on techniques, goals and treatment

Care-full negotiation of hospital discharge and end-of-life care in an Indonesian palliative care unit.

How do palliative care professionals negotiate end-of-life care with family members when prognosis and dying are not openly discussed? Based on ethnographic fieldwork in an Indonesian palliative care unit, I argue that palliative care professionals employ implicit, ambiguous and culturally sensitive communication to carefully negotiate hospital discharge and discuss end-of-life care. I focus on listening to what is said and what remains unspoken in the embodied communicative practices about end-of-life care in family meetings to discuss hospital discharge. I show how palliative care profession

Initiating Analysis.

Psychoanalysis is defined in this paper as a process that initiates in the analyst's mind with the framing of the patient's material in terms of transference and resistance. Once the analyst is able to do this, a first level of transformation of experience is effectuated that then must be consummated through interpretation to the patient of what is occurring in their mind as it is lived out in the experience with the analyst. For this author, Bion's model of container-contained complements Freud's transference and resistance model; it also offers an example to his thesis that only within a cle

Development and validation of an 8-item version of the Real Relationship Inventory-Client form.

OBJECTIVE: To develop and validate a very brief version of the 24-item Real Relationship Inventory-Client (RRI-C) form. METHOD: Two independent samples of individual psychotherapy patients (Nsample1 = 700, Nsample2 = 434) completed the RRI-C along with other measures. Psychometric scale shortening involved exploratory factor analysis, item response theory analysis, confirmatory factor analysis (CFA), and multigroup CFA. Reliability and convergent and discriminant validity of the scale and subscales were also assessed. RESULTS: The 8-item RRI-C (RRI-C-SF) preserves the two-factor structure: G

Triplet pregnancy: What do we tell the prospective parents.

Experience managing triplet pregnancies has increased over the past few decades as the incidence has changed related to assisted reproductive practices. Physicians caring for women carrying triplets cannot predict an individual outcome or pregnancy course but must educate patients about the challenges related to these high risk pregnancies. Obstetric providers can describe the wide range of risks associated with triplet gestations, and the general plan for management, but ultimately parents must make decisions with potentially lifelong consequences. Here, we present the diagnostic criteria, co

The Emotional Burden of the Correctional Health Care Advanced Practice Nurse.

This state-of-the-science review highlights the unique experiences of the correctional health care advanced practice nurse and the risks involved with repeated exposure to violent behavior, verbal assaults, and other rigors of working within a correctional facility. This article also investigates the outcomes of these risks such as compassion fatigue, vicarious trauma, and burnout. As each of these terms is dissected, insight into the correctional health care advanced practice nurse's daily experience emerges. Very little literature exists on the topic of correctional nursing and the pitfalls

A new therapy for each patient: Evidence-based relationships and responsiveness.

In this study, we introduce the journal issue devoted to evidence-based responsiveness and frame it within the work of the third interdivisional APA Task Force on Evidence-Based Relationships and Responsiveness. We summarize the meta-analytic results and clinical practices on the adaptations of psychotherapy to multiple transdiagnostic characteristics of the patient, including attachment style, culture (race/ethnicity), gender identity, coping style, therapy preferences, reactance level, religion and spirituality, sexual orientation, and stages of change. We then discuss the clinical and resea

Toward an Emerging Role for Motivational Interviewing in Primary Care.

PURPOSE OF REVIEW: Implementing Motivational Interviewing (MI) in primary care settings has been problematic due in part to persistent gaps in knowledge. Examples include poor understanding of how to effectively train persons to conduct MI, or of which aspects of MI-related communication are associated with better outcomes for patients. This review describes how recent research findings addressing the knowledge gaps support a growing role for MI in primary care. RECENT FINDINGS: Two trials of MI training combined classroom time with ongoing coaching and feedback, resulting in enhanced MI abil

"I can't bend it and it hurts like mad": direct observation of gout consultations in routine primary health care.

BACKGROUND: Gout is the most common form of inflammatory arthritis and is associated with considerable co-morbidity. It is usually managed in the primary care setting with a combination of lifestyle modification and pharmacological therapy. This study describes patterns of communication about gout observed in interactions between patients and primary care practitioners during routine consultations. METHODS: Secondary analysis of video-recordings of individual healthcare consultations between patients and a range of primary care practitioners (including general practitioners, practice nurses,

Disclosures of sexual abuse: what do you do next?

BACKGROUND: Sexual abuse, especially in childhood, often has severe and long-lasting clinical consequences, both physical and psychological. OBJECTIVE: This article outlines an approach to use when patients disclose a history of sexual abuse, and provides some resources to call on in those circumstances. DISCUSSION: This article addresses questions one might ask to uncover a history of sexual abuse, possible responses to disclosures of sexual abuse, and documentation of these disclosures.

Rules of engagement: qualitative experiences of therapeutic alliance when receiving in-patient treatment for anorexia nervosa.

Recent research has emphasised the importance of therapeutic alliance to treatment outcomes for anorexia nervosa. This study aimed to examine the experiences of service users in developing therapeutic alliance whilst in treatment for their eating disorders. This qualitative study, using purposive sampling, recruited a sample of service users receiving treatment at a national eating disorders service. In-depth interviews were audiotaped and transcribed, with transcriptions being subject to interpretative phenomenological analysis. Participants were eight adult women receiving tertiary level eat

Time, space and touch at work: body work and labour process (re)organisation.

With 'efficiency savings' the watchword for health and social care services, reorganisation and labour rationalisation are the order of the day. This article examines the difficulties involved in (re)organising work which takes bodies as its object, or material of production. It shows that working on bodies ('body work') systematically delimits possibilities for labour process rationalisation which, in turn, constrains reorganisation of the health and social care sector. It does this in three main ways. First: rigidity in the ratio of workers to bodies-worked-upon limits the potential to incre

Helpful and unhelpful therapy experiences of LGBT clients.

The purpose of this study was to identify a broad range of variables that characterize the helpful and unhelpful therapy experiences of lesbian, gay, bisexual, and transgender (LGBT) individuals. Interviews were completed with a diverse sample of 42 LGBT individuals who have been in therapy, and a content analysis was conducted. Results indicated that basic counseling skills and relationships were key determinants of the quality of LGBT clients' therapy experiences. Also important to the helpfulness of the therapy experience were therapist variables such as professional background and attitude

Silent scream: the cost of crucifixion--working with a patient with an eating disorder.

In working with individuals who have experienced extreme deprivation in early relationships, the pain is often depicted silently and repetitively. There can be a tension between the need for validation and an avoidance of growth. The analyst, then, can find herself caught between her empathic resonance to the silent scream and the need to keep in mind the "cost of crucifixion." The latter has become my inner way of fixing in my own mind the type of dilemma that ensues when the individual truly feels helpless and at the mercy of outside sources and also resents and fears the other's power.
