Spondylocostal dysostosis with perinatal death and meningomyelocele.

A preterm black girl was born at 35 weeks of gestation to a healthy nonconsanguineous couple. She had a very short trunk with disproportionately long extremities, mild prognathism, low-set ears, thoracolumbar meningomyelocele, and imperforate anus. She died 45 min after birth. Roentgenograms revealed hemivertebrae, block vertebrae, severe thoracic lordosis, absent sacrum, posterior fusion of some ribs with greater distance among them in the anterior thorax, and relatively long extremities. Internal examination showed an intact meningomyelocele extending from the first thoracic vertebra to the …

[The Buckley syndrome: recurring, severe staphylococcal infections, eczema and hyperimmunoglobulinemia E. (author's transl)].

Fifteen patients aged between three and 27 years were examined clinically and immunologically. Common to all patients were severe recurring cutaneous and pulmonary staphyloccal infections, chronic eczema, eosinophilia and an extremely elevated serum IgE level. Eight of the patients had in addition facial dysplasia characterised by coarse features, prognathism and poorly formed external ears. Marked osteoporosis, particularly of the vertebral bodies, was observed in eight patients. A constant defect of granulocyte chemotaxis was found in only three patients; fluctuating or constantly normal che…

An Investigation of Tongue-Palatal Contact Changes in Patients With Skeletal Mandibular Lateral Deviation After Sagittal Split Ramus Osteotomy.

This study aimed to investigate the changes in tongue-palatal contact patterns in patients with mandibular lateral deviation by electropalatography (EPG) before and after sagittal split ramus osteotomy (SSRO). Ten mandibular asymmetry patients who underwent SSRO participated in the study. Tongue-palatal contact patterns for the production of /t/ and /s/ sounds were observed using EPG before surgery and 3 months after surgery, and the changes in EPG pattern were examined. The number of electrode contacts in the 2 vertical columns of the EPG plate was calculated both in the mandibular deviation

[The comparative characteristics of the parameters of the temporomandibular joint and skull in children with bite anomalies].

The authors compare the mean values of 5 parameters of the temporomandibular joint and 19 skull parameters, analyzing 155 tomograms of the joint and 246 teleroentgenograms of the head of children with sagittal and vertical occlusion abnormalities during change of teeth and in permanent occlusion. This comparison has revealed a relationship between the parameters of the temporomandibular joint and the skull. The correlations were the most marked between the height of the mandibular head, height of the mandibular fossa, on the one hand, and anterosuperior and total posterior heights of the face,

Precocious puberty in a boy with a widespread linear epidermal naevus.

A 7-year-old boy with a systematized verrucous epidermal naevus, multiple pigmented naevocytic naevi and precocious puberty is described. The possible basis for this previously unreported association of abnormalities is discussed.

Genioplasty.

In 1988, I performed 15 chin procedures, seven osteotomies, and eight implants. During that same period, 38 rhinoplasties were done, but only five patients (15 per cent) had a simultaneous genioplasty. While the chin is important in nasal surgery, correction is also indicated for other aesthetic problems. Genioplasty, whether by osteotomy or implant, continues to present a unique opportunity to improve facial aesthetics with minimal risk. The current challenge is to define patient selection better, refine the technique (for example, bone substitutes and fixation hardware), and, most important,

Uninherited dentinogenesis imperfecta.

The rare hereditary disease, dentinogenesis imperfecta, is a disturbance of dentin formation in both the deciduous and permanent dentitions. It may be associated with osteogenesis imperfecta, though it is probably that the two diseases are carried by different genes. This association was recognized in a 19-year-old man. Dentinogenesis imperfecta had been diagnosed at the age of 6 and had been regarded as a mutation; 11 years later, an atypical form of osteogenesis imperfecta developed. The case is atypical because of the apparent absence of dentinogenesis imperfecta in the patient's family. Th

The Early Intervention of a Class III Malocclusion With an Anterior Crossbite Using Chincup Therapy: A Case Report.

Class III malocclusions with anterior crossbites pose significant challenges in orthodontic treatment, especially in growing children. This case report details the early intervention of a 12-year-old patient presenting with a Class III malocclusion characterized by an anterior crossbite and mandibular prognathism. A chincup was employed to inhibit mandibular growth and encourage maxillary development. Consistent use of the chincup, with regular follow-ups and adjustments, led to significant improvements. The anterior crossbite was corrected, resulting in a Class I molar relationship and an imp…

A new rationale for preservation of the mandibular third molar in orthognathic patients with missing molars.

Controversies exist regarding the need for prophylactic extraction of mandibular third molars in patients who plan to undergo orthognathic surgery. An 18-year-old male patient was diagnosed with mandibular prognathism and maxillary retrognathism with mild facial asymmetry. He had a severely damaged mandibular first molar and a horizontally impacted third molar. After extraction of the first molar, the second molar was protracted into the first molar space, and the third molar erupted into the posterior line of occlusion. The orthognathic surgery involved clockwise rotation of the maxillomandib…

Nonsurgical correction of a Class III skeletal anterior open-bite malocclusion using multiple microscrew implants and digital profile prediction.

A 20-year-old woman had a severe anterior skeletal open bite and a moderate skeletal Class III relationship with a prognathic mandible and a straight profile. She declined surgery. However, molar intrusion in a Class III patient with a straight profile can cause forward mandibular rotation and deterioration of the profile to a concave pattern. We used digital facial profile prediction software to determine whether the orthodontic compensation treatment would be acceptable to the patient. The final treatment plan consisted of extraction of the third molars, maxillary molar intrusion, and total

A complete skull from Dmanisi, Georgia, and the evolutionary biology of early Homo.

The site of Dmanisi, Georgia, has yielded an impressive sample of hominid cranial and postcranial remains, documenting the presence of Homo outside Africa around 1.8 million years ago. Here we report on a new cranium from Dmanisi (D4500) that, together with its mandible (D2600), represents the world's first completely preserved adult hominid skull from the early Pleistocene. D4500/D2600 combines a small braincase (546 cubic centimeters) with a large prognathic face and exhibits close morphological affinities with the earliest known Homo fossils from Africa. The Dmanisi sample, which now compri

Dental care management of a young patient with extensive lymphangioma of the tongue: a case report.

Lymphangiomas are benign tumors resulting from a congenital malformation of the lymphatic system. Relatively uncommon, lymphangiomas are usually diagnosed at birth and develop within the first years of life. When these tumors occur in the oral cavity, the tongue is the most frequently affected site. Lymphangioma of the tongue is a common cause of macroglossia in children, which may lead to a dry/cracked tongue with ulcerating secondary infections, difficulty in swallowing and mastication, speech disturbances, exclusive nasal breathing, airway obstruction, mandibular prognathism and other possi…

Major osteotomies of the cranium, facial bones, and mandible.

This paper describes the present status of osteotomies of the facial skeletal framework, including craniofacial surgery, in Japan based on patients seen at the Tokyo Metropolitan Police Hospital. It presents our experience with the correction of craniofacial dysostosis, orbital hypertelorism, orbital hypotelorism, untreated plagiocephaly, craniostenosis, cleft lip deformity, prognathism, and asymmetry of the mandible. There are a few departments skillfully performing such osteotomies in Japan, but it cannot yet be said that the procedures are well developed here. I myself have applied them in …

[Crouzon's craniofacial dysostosis].

A brief account of craniofacial synostosis is presented. The cranium is acrocephalic owing to the early closure of the coronal and lambdoid sutures. The crown is typically rudimentary where the anterior fontanel is still open. Cranial radiography shows digitate markings. Skeletal changes lead to hypertelorism, exophthalmos and strabismus. The nose is of the parrot's beak type and the hypoplasic upper jaw is in contrast with the lower jaw, resulting in pseudo-prognathism. These basic malformations are accompanied by several other deformities. The disease may appear as a hereditary or as a spora…

Craniofacial and dentoalveolar morphology in individuals with Prader-Willi syndrome: a case-control study.

BACKGROUND: Prader-Willi syndrome (PWS) is a complex multisystem genetic disorder with distinct genetic and clinical features. Among other clinical symptoms, PWS is characterized by severe infantile hypotonia with feeding problems, childhood onset hyperphagia, obesity, scoliosis, short stature combined with growth hormone deficiency and developmental delay. PWS is associated with facial dysmorphology, orofacial dysfunction, oral abnormalities, low salivary flow and subsequent severe tooth wear. Little is known about the craniofacial growth direction or dental and skeletal relationships in indi
