An extraanatomic bypass method alternative to the obturator bypass.

Obturator bypass is the preferred method when infectious groin problems are challenging. However, this method can not be applied either in some special settings including the dissemination of the infection towards retroperitoneum and some technical difficulties in handling obturator bypass area (like fibrosis and inability to expose the obturator foramen). We present an alternative bypass method in two cases.

A case of prosthetic mitral valve endocarditis caused by Brucella abortus.

Endocarditis is a rare but serious complication of brucellosis. We report here a case of Brucella abortus endocarditis occurring on a prosthetic mitral valve. The diagnosis was established by positive serology, the presence of vegetation on the mitral prosthesis and isolation of B. abortus from the blood and valve cultures. The patient was successfully treated with combined medical and surgical therapy.

[Coxiella burnetii endocarditis on a mechanical valvular prosthesis. Apropos of 2 cases].

The authors report two cases of prosthetic valve endocarditis due to Coxiella burnetii. The histories were chronic and complex suggesting an auto-immune disease: prolonged recurrent fever despite antibiotic therapy with a biological inflammatory syndrome whilst blood cultures remained negative. The first patient presented with prosthetic valve dehiscence and acute glomerulonephritis. The second patient had coagulation defects with prosthetic valve thrombosis, mesenteric adenopathy and congestive cardiac failure without prosthetic valve dysfunction. In suspected endocarditis with negative blood

Clinical outcome and echocardiographic findings of native and prosthetic valve endocarditis in the 1990's.

Prosthetic valve endocarditis is considered to be associated with a more severe prognosis than native valve endocarditis. Among other factors, inappropriate visualization of vegetations in prosthetic valve endocarditis by transthoracic echocardiography is responsible for this observation. Since the introduction of transoesophageal echocardiography into clinical practice the diagnostic sensitivity and specificity of the detection of vegetations located on prosthetic valves have been enhanced. Therefore we aimed to determine and compare the prognosis of prosthetic valve endocarditis and native v

Distal Corporal Anchoring Stitch: A Technique to Address Distal Corporal Crossovers and Impending Lateral Extrusions of a Penile Prosthesis.

BACKGROUND: Unidentified distal crossovers, delayed distal crossovers, and impending lateral extrusion are complications of penile prosthesis implant insertion but are not as common as prosthesis infection or mechanical failure. AIM: To evaluate results of a surgical technique, the distal corporal anchoring stitch, that addresses fixation of the penile prosthesis in patients with these complications. METHODS: A lateral sub-coronal incision is used on the side where the crossover or laterally extruding cylinder should be positioned. Dissection is carried through the Buck fascia, followed by a

Late onset periprosthetic joint infection of the knee caused by Streptococcus anginosus. Case presentation and literature review.

Periprosthetic joint infection (PJI) is one of the most dramatic complications of joint arthroplasty. Although streptococcal bone and joint infections are less common than staphylococcal cases, their role as causative agents of bone and joint remains significant accounting for at least 10% of PJIs. Streptococcus anginosus group (SAG) bacteria are usually found in the normal flora of the urogenital tract, intestinal tract and oropharynx and could cause pyogenic infections to affect brain, lungs and liver. SAG bacteria are uncommonly reported as a cause of osteomyelitis and the involvement of a

Osseointegrated Percutaneous Prosthetic System for the Treatment of Patients With Transfemoral Amputation: A Prospective Five-year Follow-up of Patient-reported Outcomes and Complications.

INTRODUCTION: Direct skeletal attachment of prostheses has previously been shown to improve patient-reported outcome (PRO) measures of individuals with transfemoral amputation (TFA) at 2-year follow-up. This prospective study reports the outcomes at 5-year follow-up. METHODS: A total of 51 patients (55 legs) with TFA were included in a prospective study. Complications, success rate, and PRO measures were followed for 5 years. RESULTS: The cumulative fixture survival rate at 5 years was 92%, and the revision-free survival rate was 45%. Thirty-four patients had 70 superficial infections. Eleve

[Aortic valve replacement using the Björk-Shiley valve. Long-term results (268 patients)].

From 1974 till November, 1986, 268 adult patients aged from 15 to 84 years (11 p. cent over 70) underwent isolated aortic valve replacement by Björk-Shiley prosthesis. 81 p. cent of the prosthetic valves implanted were size 21 or over. The initial lesion was aortic stenosis (AS) in 40 p. cent, aortic regurgitation (AR) in 35 p. cent and aortic disease (AD) in 25 p. cent of the cases. The aortic valve lesions were degenerative (46 p. cent), rheumatismal (18 p. cent), congenital (12 p. cent), infective (19 p. cent including acute infective endocarditis) or dystrophic (7 p. cent) in origin. Research progress on debridement, antibiotics, and implant retention (DAIR) for the treatment of periprosthetic joint infection after artificial joint replacement.

Periprosthetic joint infection (PJI) is one of the most serious complications following artificial joint replacement, posing significant treatment challenges and a heavy clinical burden. Debridement, antibiotics, and implant retention (DAIR) has become the preferred treatment strategy for acute PJI due to its advantages of minimal invasiveness, functional preservation, and cost-effectiveness. This article reviews the latest advances in optimizing the indications, timing of intervention, key technical details, and multimodal anti-infection strategies for DAIR. It focuses on discussing patient s

Comparative evaluation of treatment strategies for femoral metastases: endoprosthesis reconstruction versus intramedullary nailing.

INTRODUCTION: Advances in cancer treatment have increased survival rates, leading to a higher incidence of femoral metastases, affecting approximately 20,000 patients annually. Surgical management typically involves intramedullary nailing (IMN) or endoprosthesis (EP). This meta-analysis compares IMN and EP outcomes regarding infection rates, reoperations, and implant failures across short- and intermediate-term follow-up periods. METHODS: A systematic review following PRISMA guidelines identified studies from PubMed evaluating IMN and EP for metastatic femoral disease. Eligible studies, in En

Listeria monocytogenes prosthetic joint infection: Two clinical cases and a review of the literature.

INTRODUCTION: Most infections caused by Listeria monocytogenes occur in individuals with predisposing conditions, such as advanced age or the use of immunosuppressive therapies, and primarily manifest as bacteremia or meningoencephalitis. Osteoarticular infections are uncommon and typically affect joint prostheses, with late-onset infections following surgery. Treatment in these cases should include antibiotic therapy along with a surgical strategy, which generally involves the removal of the affected prosthesis. METHODS: Two cases of prosthetic joint infection caused by L. monocytogenes are

Healthcare exposures and associated risk of endocarditis after open-heart cardiac valve surgery.

BACKGROUND: Infective endocarditis (IE) following cardiac valve surgery is associated with high morbidity and mortality. Data on the impact of iatrogenic healthcare exposures on this risk are sparse. This study aimed to investigate risk factors including healthcare exposures for post open-heart cardiac valve surgery endocarditis (PVE). METHODS: In this population-linkage cohort study, 23,720 patients who had their first cardiac valve surgery between 2001 and 2017 were identified from an Australian state-wide hospital-admission database and followed-up to 31 December 2018. Risk factors for PVE

Bacterial comparison study of prosthetic liners.

BACKGROUND: Amputee patients in low-resource settings face a heightened risk of skin infections at the skin-liner interface of their residual limb, which can develop into a variety of serious complications. Most research comparing prosthetic liners has focused on mechanical and aesthetic properties. However, the relative propensity of different liner types to host bacterial growth has yet to be explored. METHODS: To address this question, we performed a bacterial comparison study across a traditional sock-EVA combination liner, an advanced Ossur Iceross silicone liner, and a novel silicone li

Three cases of Cutibacterium avidum prosthetic valve infective endocarditis at a single heart center.

OBJECTIVE: To resolve an exceptional clustering of Cutibacterium avidum prosthetic valve infective endocarditis (IE) at a single heart center. METHODS: During a period of 21 months, three patients experienced C. avidum bacteremia 24-128 days after aortic valve replacement. Operative procedures and electronic prescriptions of antimicrobials were surveyed, and bacterial isolates were genome sequenced. RESULTS: The prosthetic valves were inserted by separate surgical teams. In one case, echocardiographic confirmation of IE was not achieved until 4 months after the first positive blood culture,

Q-Fever endocarditis: Prosthetic valve infection in the absence of zoonotic exposure.

Background: Q-Fever is a rare condition with an often insidious presentation. Endocarditis is a serious complication of up to 5% of Q-Fever cases, with a higher incidence and mortality in patients with prosthetic valves.Case-history: A 67-year-old man presented with a 6-weeks history of breathlessness on a background of previous bioprosthetic aortic valve replacement in 2018. Subsequent echocardiograms showed severe eccentric aortic regurgitation, a 2 cm vegetation, and severely impaired biventricular function. Routine blood cultures were negative, but later PCR testing for non-culturable micr

Two-Stage Revision Total Knee Arthroplasty for Chronic Histoplasma Capsulatum Prosthetic Joint Infection: A Case Report.

CASE: This report describes a histoplasma capsulatum total knee prosthetic joint infection (PJI) in an immunosuppressed patient treated with a 2-stage revision. The diagnosis of PJI was made based on minor criteria, and the causative organism was identified from cultures obtained at the time of explantation. The patient underwent induction with amphotericin B, followed by oral antifungal therapy and a successful 2-stage revision with a hinged prosthesis with an interval of ∼7 months between stage 1 and stage 2. At the most recent follow-up (18 months), she remained clear from infection with pl
