BACKGROUND: In 2018, Femoral Neck System (FNS), a dedicated fixator for femoral neck fractures, was introduced. This device has been in increasing use because it provides excellent rotational and angular stability. However, the shortest bolt of FNS is 75 mm long. Thus, it is not usable when the axial length of the proximal femur (ALPF), the distance between the innominate tubercle and the surface of the femoral head, is less than 80 mm. In this study, we investigated the incidence and associated factors of small ALPF (< 80 mm) in femoral neck fracture patients.
METHODS: We measured the ALPF o

PURPOSE: Basicervical femoral neck fractures (BFNFs) are uncommon in the elderly but associated with higher postoperative complications compared to intertrochanteric fractures. Proximal femoral nail antirotation (PFNA) is one of the preferred surgical options for intertrochanteric fracture due to its minimal invasiveness, fewer complications, and better in postoperative functional outcomes. However, limited research addressed the outcomes and the risk factors for implant-related complications after using PFNA.
METHODS: This retrospective study included 149 patients treated with PFNA fixation

BACKGROUND: Impact of concomitant fractures on patients sustaining a proximal femur fracture remains unclear. Rising numbers and patient need for rehab is an important issue. The objective of our study was to investigate the impact of concomitant fractures, including all types of fractures, when treated operatively, for proximal femur fractures on the length of hospital stay, in-house mortality and complication rate.
METHODS: Observational retrospective cohort single-center study including 85 of 1933 patients (4.4%) with a mean age of 80.5 years, who were operatively treated for a proximal fe

AIMS: Trochanteric fractures (TF) are among the most frequent fractures and have major impact on the affected patients and the healthcare system. Cephalomedullary nails (CMN) are widely used for these fractures. Complications remain frequent, either due to comorbidities or concerning the implant. The goal of this retrospective, single-centre study was to compare the complication rates of two CMN from one manufacturer.
MATERIALS AND METHODS: All internal fixations TF performed between 01/2017 and 07/2021 in our institution were analysed retrospectively. Follow-up was at least two years. The CM

Digital mobility outcomes (DMOs) offer unique insights into recovery of real-world mobility after proximal femoral fracture (PFF), but their clinical validity remains to be established. This study assessed construct validity (convergent, divergent, and known-groups) of 24 DMOs measuring walking activity (amount, pattern) and gait (pace, rhythm, bout-to-bout variability) in patients within one year after PFF. Patients were recruited from inpatient and outpatient lists at five European sites, resulting in 505 included participants (66% female), with mean age of 77.6 ± 9.4 years and supervised ga

BACKGROUND Intertrochanteric femoral fractures are common in older adults, and proximal femoral nail (PFN) fixation is widely preferred due to its biomechanical advantages. Lag screw cut-out, defined as the progressive migration of the lag screw through the femoral head with loss of fixation, is one of the most serious mechanical complications after PFN fixation. This complication can result in mechanical failure and require revision surgery. Therefore, this study aimed to identify independent risk factors for lag screw cut-out and evaluate lag screw stability in patients without cut-out. MATE

Background and Objectives: Total hip (TH) and femoral neck (FN) T-scores from the same dual-energy X-ray absorptiometry (DXA) examination often diverge. We evaluated whether the signed TH-FN difference is associated with prevalent proximal femur fracture and whether it has individual-level discriminative value, explicitly separating statistical association from discrimination. Materials and Methods: In a single-center retrospective case-control study (complete case N = 3752; 448 fractures), multivariable logistic regression adjusting for the TH T-score and 12 covariates estimated the associati

INTRODUCTION: Surgery is often delayed in patients with proximal femoral fractures who receive oral anticoagulants, to avoid complications related to perioperative bleeding. However, surgery delay may increase the risk of postoperative mortality. Our primary goal was to understand whether anticoagulated patients benefit from early surgery in terms of survival and perioperative complications.
METHODS: This is a multicenter retrospective cohort study of 581,189 patients with proximal femoral fractures. About 2.0 % (n = 11,385) received direct oral anticoagulants (DOAC) and 1.5 % (n = 8,726) rec…

INTRODUCTION: Proximal femoral nailing (PFN) offers biomechanical benefits for intertrochanteric fractures but can lead to higher complication rates from poor reduction and technique errors, particularly in obese patients. Incorrect entry points may cause reduction loss, iatrogenic fractures, and misplaced lag screws. The study aims to investigate the effect of using an oriented flexible reamer instead of a rigid reamer on clinical and radiological results to obtain a medial entry point and better positioning of the nail in the intramedullary area in obese intertrochanteric fracture patients.

BACKGROUND: The proximal femoral nail anti-rotation (PFNA) is a commonly used internal fixation system for intertrochanteric fractures (IFs) in older adults. Knee osteoarthritis (KOA) is a degenerative lower extremity disease that occurs most frequently in the elderly. Some patients have already had KOA before the IFs. However, whether KOA impacts the postoperative outcome of IFs has not been reported.
OBJECTIVE: This study aimed to investigate the effect of KOA on the fracture side on the outcome after PFNA for IFs in the elderly.
METHODS: Between January 2016 and November 2021, 297 elderly

BACKGROUND: The surgical Apgar score (SAS) is a perioperative risk evaluation score, which considers intraoperative minimum heart rate, minimum mean arterial pressure and estimated blood loss. Although validated in multiple surgical fields, SAS remains quite controversial in the orthopedic one. The main purpose of this study was to investigate if SAS relates with the occurrence of complications during the first 30-days after proximal femoral fracture surgery.
METHODS: Retrospective study including all consecutive patients submitted to proximal femoral fracture surgery between January and July

BACKGROUND Intertrochanteric fractures can be managed using insertion of a proximal femoral nail anti-rotation (PFNA) intramedullary nail inserted into the femur. The helical blade used with the PFNA technique has the advantage of less bone removal and good biomechanical properties, but significant hidden blood loss (HBL) can occur. This study compared postoperative HBL in 219 elderly patients with intertrochanteric fractures using PFNA fixation, stratified by greater trochanter comminution status (comminuted vs non-comminuted). MATERIAL AND METHODS We retrospectively analyzed data on 219 elde

Introduction Pertrochanteric femur fractures comprise approximately half of all hip fractures. The proximal femoral nail anti-rotation 2 (PFNA2), designed by Arbeitsgemeinschaft fur Osteosynthesefragen (AO), is an intramedullary device with a helical blade rather than a screw for better purchase in the femoral head. This modification in the proximal femur nail (PFN) was done to adapt to the Asian population and reduce nail-related complications in the Asian population. Materials and methods The study was conducted in a tertiary health center from November 2022 to November 2024 and included 40
