Self-Care Practices for Puerperal Sepsis Prevention and Associated Factors Among Postnatal Women in Africa: A Systematic Review and Meta-Analysis.

BackgroundPuerperal sepsis remains a preventable contributor to maternal illness. Pregnancy-related sepsis is responsible for ∼10% to 15% of maternal deaths. Despite this burden, no comprehensive review has examined self-care practices related to its prevention. Therefore, this review sought to evaluate self-care behaviors aimed at preventing puerperal sepsis and to explore associated factors among postnatal women in Africa.Data Sources and MethodsWe conducted a systematic review and metaanalysis of 17 eligible studies on the prevalence of self-care practices for puerperal sepsis prevention an

Endometritis: Nontraditional Therapies.

Endometritis is characterized by inflammation of the endometrial lining of the uterus and is a leading cause of subfertility in broodmares. When traditional therapies fall short, nonconventional means can be used either to supplement or in lieu of customary practices to manage endometritis. This article reviews alternative therapies available for use in broodmare practice and provides anecdotal and scientific evidence supporting their use.

An interesting case presentation: peripartum meningococcal meningitis.

Meningococcal disease during pregnancy is extremely rare. A single reported case occurred more than 20 years ago in England. We present the case of a young woman who just hours after delivery of her baby developed fulminant meningococcal meningitis with its classic findings. Our experience illustrates the importance of early diagnosis and appropriate therapy of meningococcal disease in the gravid as well as in the nongravid population.

[Necrotising enterocolitis and pregnancy. One case (author's transl)].

Having had a case of necrotising enterocolitis which occured on the third day after delivery and which was successfully treated by total colectomy with secondary restoration of bowel continuity, the authors discuss the pathogenicity of this condition. It can be secondary to a haemorragic or a septic obstetrical complication. It may equally be the sequel of a functional paralytic ileus which, although rare, is one of the classical complications following labour and Caesarean operations. In this way it can be compared to those cases of enterocolitis which are secondary to ileus, such as are foun

SARS-CoV-2 (COVID-19) and the Teaching of Ignaz Semmelweis and Florence Nightingale: a Lesson of Public Health from History, after the "Introduction of Handwashing" (1847).

Ignác Fülöp Semmelweis (1818-1865) and Florence Nightingale (1820-1910) were two important personalities in the history of medicine and public health. They dealt with the problem of handwashing. Semmelweis is also known as the "father of hand hygiene"; just in 1847 he discovered the etiology and prophylaxis of puerperal sepsis and imposed a new rule mandating handwashing with chlorine for doctors. He also tried to persuade European scientific community of the advantages of handwashing. During the Crimean War, in Scutary (Turkey), Florence Nightingale strengthened handwashing and other hygiene

Bilateral Facial Palsy: A Clinical Approach.

Bilateral facial palsy (BFP) is exceedingly rare, representing only 0.3%-2.0% of facial palsy cases. Unlike unilateral facial palsy, it is often caused by a serious underlying systemic disease and therefore warrants urgent medical intervention. The differential diagnosis is broad, and detailed history, physical examination, and investigations are essential for identifying the etiology. Common acquired causes in existing case series include Lyme disease, Guillain-Barré syndrome, sarcoidosis, trauma, and Bell's palsy. Palsy that develops rapidly is often caused by trauma, infections, or autoimmu

Imaging and diagnosis of postpartum complications: sonography and other imaging modalities.

Postpartum complications can be broadly divided into 4 categories: postpartum hemorrhage, obstetrical trauma, thromboembolic complications, and puerperal infections. Postpartum hemorrhage is most commonly caused by uterine atony, abnormal placentation, or genital tract trauma. Secondary causes of hemorrhage include retained products of conception and, rarely, subinvolution of the placental implantation site. Uterine dehiscence or rupture may be occult on ultrasound examination and may be better visualized on sagittal computed tomography or magnetic resonance imaging. Obstetric trauma during pr…

Safety of peripherally inserted central catheters during pregnancy: a retrospective study.

PURPOSE: We investigated the incidence of complications associated with peripherally inserted central line catheters, inserted using a standardized technique, during pregnancy and the postpartum period. MATERIALS AND METHODS: A retrospective case series was performed that included all pregnant and postpartum women who received peripherally inserted central catheters (PICCs) at a single institution between 2006 and 2014. Patient demographics and data on infectious, mechanical and thrombotic complications were collected. Some patients required more than one line insertion during the same pregna

Mode of delivery and postpartum morbidity in Latin American and Caribbean countries among women who are infected with human immunodeficiency virus-1: the NICHD International Site Development Initiative (NISDI) Perinatal Study.

OBJECTIVE: The purpose of this study was to test whether cesarean delivery before labor and before ruptured membranes is associated with a higher risk of postpartum morbidity than vaginal delivery among women who are infected with human immunodeficiency virus-1 in Latin America and the Caribbean. STUDY DESIGN: Data from a prospective cohort study (National Institute of Child Health and Human Development International Site Development Initiative Perinatal Study) were analyzed. The study population consisted of women who were followed for > or = 6 to 12 weeks after delivery, who had singleton i

Caesarean section: a 15-year review of changing incidence, indications, and risks.

During 15 years (1965 through 1979), 9,718 cesarean sections were performed at Hutzel Hospital. This report concerns the indications, incidence, morbidity, and mortality. Dystocia, fetal distress, and breech presentations were the most common indications for primary cesarean section. The incidence rose from 9.8% to 16.8% during this study. Forceps deliveries dropped from 47.5% to 12.0% and vaginal breech deliveries decreased from 86% to 35%. Spinal anesthesia was used in 90%. Caesarean section hysterectomies were performed in 128. The most common indications were carcinoma in situ of the cervi

Histophilus somni as a Unique Causative Agent of Puerperal Metritis (PM) in a Third-Lactation Holstein Cow.

This manuscript aims to report the clinical and laboratory diagnosis of puerperal metritis (PM) in a dairy cow, caused by H. somni as a unique pathogen. The cow showed signs of systemic illness, including a sudden drop in milk production, a rectal temperature of 40.4 °C, tachypnea, dehydration, and completely fluid, brown, and fetid uterine discharge. Pure cultures of H. somni were identified and submitted to the Kirby-Bauer disc diffusion method for antibiotic sensitivity. The study showed that H. somni was resistant to tetracyclines and cephalosporins (Ceftiofur), antibiotics commonly used t

Peripartum infections and associated maternal mortality in rural Malawi.

OBJECTIVE: To assess associations between maternal mortality and severe morbidity and human immunodeficiency virus (HIV) infection, uptake of antiretroviral therapy, obstetric infections, and nonobstetric infections in a rural Malawian district, where the estimated HIV prevalence is 21%. METHODS: We studied the incidence and outcomes of maternal peripartum infections between September 2007 and September 2009 at the district hospital. We used a facility-based prospective cohort study design, including all cases of severe maternal peripartum infection up to 42 days postpartum, and recorded mate

The burden of group A Streptococcus (GAS) infections: The challenge continues in the twenty-first century.

Streptococcus pyogenes is a Gram-positive bacterium, also known as Group A Streptococcus (GAS), that has become a significant threat to the healthcare system, infecting more than 18 million people and resulting in more than 500,000 deaths annually worldwide. GAS infection rates decreased gradually during the 20th century in Western countries, largely due to improved living conditions and access to antibiotics. However, post-COVID-19, the situation has led to a steep increase in GAS infection rates in Europe, the United States, Australia, and New Zealand, which triggers a global concern. GAS in
