Marijuana smoke induces severe pulmonary hyperresponsiveness, inflammation, and emphysema in a predictive mouse model not via CB1 receptor activation.

Sporadic clinical reports suggested that marijuana smoking induces spontaneous pneumothorax, but no animal models were available to validate these observations and to study the underlying mechanisms. Therefore, we performed a systematic study in CD1 mice as a predictive animal model and assessed the pathophysiological alterations in response to 4-mo-long whole body marijuana smoke with integrative methodologies in comparison with tobacco smoke. Bronchial responsiveness was measured with unrestrained whole body plethysmography, cell profile in the bronchoalveolar lavage fluid with flow cytometr

Resolution of pulmonary interstitial emphysema following selective left main stem intubation in a premature newborn: an old procedure revisited.

Pulmonary interstitial emphysema (PIE) contributes to worsening respiratory status in already compromised premature infants. Various treatments for PIE include high-frequency ventilation, postural therapy, pneumonectomy or lobectomy and selective bronchial intubation. While right main stem bronchus intubation is easy to perform at the bedside for treatment of PIE in the left lung, left main stem bronchus intubation for the treatment of right-sided PIE remains challenging and the technique was rarely reported and poorly described. In this case report, we revisit this procedure and describe succ

Multicenter controlled trial comparing high-frequency jet ventilation and conventional mechanical ventilation in newborn infants with pulmonary interstitial emphysema.

One hundred forty-four newborn infants with pulmonary interstitial emphysema were stratified by weight and severity of illness, and randomly assigned to receive treatment with high-frequency jet ventilation (HFJV) or rapid-rate conventional mechanical ventilation (CV) with short inspiratory time. If criteria for treatment failure were met, crossover to the alternate ventilatory mode was permitted. Overall, 45 (61%) of 74 infants met treatment success criteria with HFJV compared with 26 (37%) of 70 treated with CV (p less than 0.01). Eighty-four percent of patients who crossed over from CV to H

Anatomic Pulmonary Lobectomy via Video-Assisted Thoracoscopic Surgery in an Adult Patient With Congenital Lobar Emphysema: A Case Report.

Congenital lobar emphysema (CLE) is a congenital lung malformation characterized by progressive hyperinflation of a pulmonary lobe. We report the case of a 23-year-old male, an active-duty military service member with a history of recurrent pneumonias and lung abscesses since childhood, who presented with a new infectious episode. A chest computed tomography scan revealed an abscess in the right upper lobe, together with air trapping and overdistension of both the apical segment of the right upper lobe and the adjacent right middle lobe, raising suspicion for a congenital lung malformation. Sequential Bilateral Bronchoscopic Lung Volume Reduction for Residual Hyperinflation Following Successful Index Procedure in Pulmonary Emphysema: A Multicenter Study.

BACKGROUND: Bronchoscopic lung volume reduction (BLVR) improves pulmonary function and exercise capacity in select patients with emphysema, but some remain symptomatic with residual hyperinflation. We assessed the safety and effectiveness of bilateral BLVR in such cases. METHODS: We retrospectively analyzed patients across 2 centers who underwent BLVR in the contralateral lung after successful BLVR in the index lung, with residual hyperinflation. RESULTS: Thirteen patients [5 males; mean age 68; interquartile range (IQR)=64 to 73 y] underwent sequential bilateral BLVR. The median interval be

Early Pulmonary Interstitial Emphysema in Preterm Neonates-Respiratory Management and Case Report in Nonventilated Very Low Birth Weight Twins.

Worldwide metabolic dysfunction-associated steatotic liver disease (MASLD) is the leading cause of chronic liver disease. Despite high global prevalence, MASLD is not yet recognized as a noncommunicable disease, although all of the criteria are fulfilled for such a classification. MASLD is strongly associated with type 2 diabetes, cardiovascular disease, chronic kidney disease, and certain extrahepatic cancers. At 65% and 37% the prevalence of MASLD is extremely high in adults and children with type 2 diabetes, respectively. The pathogenesis of MASLD is closely related to that of type 2 diabet

Premedication with Clarithromycin Is Effective against Secondary Bacterial Pneumonia during Influenza Virus Infection in a Pulmonary Emphysema Mouse Model.

Secondary bacterial pneumonia (SBP) during influenza increases the severity of chronic obstructive pulmonary disease (COPD) and its associated mortality. Macrolide antibiotics, including clarithromycin (CAM), are potential treatments for a variety of chronic respiratory diseases owing to their pharmacological activities, in addition to antimicrobial action. We examined the efficacy of CAM for the treatment of SBP after influenza infection in COPD. Specifically, we evaluated the effect of CAM in elastase-induced emphysema mice that were inoculated with influenza virus (strain A/PR8/34) and subs

Correlation between CT-determined emphysema and the severity of acute exacerbation of chronic obstructive pulmonary disease: a cross-sectional study.

BACKGROUND: The high prevalence of chronic obstructive pulmonary disease (COPD) poses a significant social burden. Emphysema is a basic pathological change associated with poor lung function in stable COPD. However, few studies have focused on the potential role of emphysema in acute exacerbation of COPD (AECOPD). The current study aimed to explore the correlation between CT-determined emphysema and other main physiological parameters of severe AECOPD. METHODS: In total 90 hospitalized patients with AECOPD participated in this cross-sectional study. All of them underwent chest CT scans and lu

Automated quantification of interstitial lung abnormalities and emphysema on computed tomography: a predictive marker for postoperative pulmonary complications after esophagectomy.

BACKGROUND: Postoperative pulmonary complications (PPCs), including pneumonia, acute lung injury, and acute respiratory distress syndrome, are common morbidities associated with mortality following esophagectomy. This study aimed to assess the association of chest computed tomography (CT) texture features with PPCs following esophagectomy. METHODS: Between 2016 and 2022, data from 765 patients who underwent upfront esophagectomy were analyzed. Deep learning-based automatic quantifi cation was used to identify interstitial lung abnormalities (ILAs) and emphysema on the preoperative chest CT. L

Generalized emphysema and chronic obstructive pulmonary disease in a farmer, non-smoker - the more we seek, the more we find.

UNLABELLED: Although alpha-1 antitrypsin (A1AT) deficiency represents one of the most common genetically conditioned diseases in the population of Caucasian adult individuals, it is rarely diagnosed. Alpha-1 antitrypsin is an important component of the anti-proteolytic protection in the lungs. Individuals affected by the protein deficiency are exposed to a higher risk of developing chronic obstructive pulmonary disease (COPD), emphysema or liver diseases. A CASE REPORT: A 52-year-old farmer, non-smoker, spraying orchards since the age of 15 years, was admitted to the Department with dyspnea a

Evaluation of clinical characteristics and prognosis of chronic pulmonary aspergillosis depending on the underlying lung diseases: Emphysema vs prior tuberculosis.

PURPOSE: There have been scarce data evaluating the differences of clinical characteristics and prognosis of chronic pulmonary aspergillosis (CPA) depending on underlying pulmonary diseases. We tried to clarify them in CPA patients who had pulmonary emphysema or previous pulmonary tuberculosis. METHODS: We reviewed and evaluated CPA patients diagnosed between 2007 and 2013 with pulmonary emphysema (PE group; n = 29), with previous pulmonary tuberculosis (PT group; n = 47) and with combination of these 2 underlying conditions (CTE group; n = 24). RESULTS: In CT findings, fungus balls were rar…

[Combined pulmonary fibrosis and emphysema: the natural history of the disease. The chronological evolution of clinical features, respiratory function and the CT scan].

INTRODUCTION: The syndrome of combined basal pulmonary fibrosis and apical emphysema (CPFE) is characterised by severe dyspnoea not fully explained by pulmonary function tests that show subnormal lung volumes and expiratory flows while CT imaging reveals varying degrees of emphysema and interstitial fibrosis. CASE REPORT: A man presented with chronic obstructive pulmonary disease associated with severe emphysema. Airflow obstruction was associated with early over-inflation. Five years later he developed interstitial fibrosis with CT appearances compatible with non-specific interstitial pneumo

Elevated platelet activating factor in the tracheal aspirate at birth and signs of intra-uterine inflammation in infants with neonatal pulmonary emphysema.

UNLABELLED: To investigate the pathophysiology of the neonatal pulmonary emphysema, we assayed platelet activating factor (PAF) in the tracheal aspirates of the low birth weight infants. A total of 29 neonates (birth weight <1750 g) who required mechanical ventilation were enrolled. Tracheal aspirates were obtained within 48 h and blood samples collected within 24 h of life. PAF was assayed on the basis of its ability to cause aggregation of washed rabbit platelets. PAF was significantly elevated in four infants who showed pulmonary emphysema within the 1st week of life (median 24 pg/g lipid p…

Airway mucus plugs and ventilatory support during exacerbations, with subsequent outcomes, in patients with chronic obstructive pulmonary disease or emphysema.

BACKGROUND: Airway mucus plugs are associated with airflow obstruction and exacerbations in stable chronic obstructive pulmonary disease (COPD); however, their roles during exacerbations remain unclear. This study aimed to examine whether mucus plugs during exacerbations are associated with ventilatory support requirements and prognosis in patients with COPD. METHODS: This single-center retrospective study included patients with COPD, defined by airflow limitation on spirometry or emphysema on computed tomography (CT), who presented to the emergency department, and were diagnosed with COPD ex
