Cerclage wire is widely used in the treatment of fracture internal fixation and is shown effective in clinic. But a report by S.L. has pointed that the wire loop delayed the growth of bone. We have established a radius fracture model to study the possible detrimental effects of cerclage wire on fracture healing and the potential mechanism. By high-resolution CT analysis cerclage wire is found to delay fracture healing, by histological assessment cerclage wire is found to extended the time of hematoma and the marrow cavity appearing, by confocal microscopy cerclage wire decreased the content of

INTRODUCTION: Ionising radiation is a potential risk for potentially exposed personnel. Only a few studies have examined the factors contributing to the emission of radiation in orthopaedic trauma procedures. We hypothesize that the experience of the surgeon and the fracture type influence the emission of radiation intraoperatively.
METHODS: In a retrospective analysis, we examined 100 consecutive distal radius fractures receiving a volar plate osteosynthesis. The following parameters were documented: demographic data, plate system, fracture type, duration of the surgical procedure and durati…

No consensus currently exists on the facture location of dorsally displaced distal radius fractures (DRFs). We present a systematic evaluation of the distal fracture line (DFL) location of DRFs and possible influencing factors. Determining the average location of DRFs provides a basis for developing more sensitive tests to determine bone strength using a variety of imaging techniques and for developing improved biomechanical models to test fracture characteristics and surgical implants. Initial radiographs of 157 DRFs dorsally displaced DRFs in patients aged 40-74 years were identified, patien…

Fractures of the distal radius are common injuries and may result from high-energy or low-energy trauma. Treatment options include non-surgical and surgical management, and although high-level evidence does not support surgical management for all of these fractures, there is a clear increasing trend in plate fixation for fractures of the distal radius. Where there is an indication to manage these surgically, open reduction and internal fixation of distal radius fractures is a core surgical skill that most trauma surgeons possess. However, some of these fractures may present in complex or atypi…

INTRODUCTION: Distal radius fractures (DRFs) are common, with over 14,000 annual cases in Denmark. Most cases are treated non-surgically and may require closed reduction prior to casting. National guidelines for anaesthesia and reduction technique for closed reduction of DRFs are lacking, impacting clinical practices and potentially optimal care. This survey aimed to describe Danish clinical practice for initial treatment and closed reduction of DRFs.
METHODS: Between January and April 2025, the survey was distributed online to the 23 physicians responsible for all 44 emergency departments an…

PURPOSE: To develop a classification system for extra-articular distal radius fractures (DRFs) integrating radiographic parameters of instability, ulnar fractures, distal radioulnar (DRU) joint disruption, and carpal involvement and to evaluate its reliability.
METHODS: A multicenter study across three university-affiliated orthopedic centers identified 1,239 extra-articular DRFs from 3,784 radiographs. Radiographic features were categorized into four domains: (1) DRF instability (radial inclination <15°, radial height loss ≥5 mm, dorsal angulation >15° in dorsally displaced fractures or vola…

PURPOSE: To characterize volar shear fractures of the distal radius using computed tomography (CT) imaging to better evaluate dorsal cortical fracture involvement, articular complexity, and morphologic features to assist in operative planning.
METHODS: A retrospective review of patients with CT-confirmed distal radius fractures was performed. One hundred ten volar shear fractures were identified and classified according to the AO/Orthopedic Trauma Association system, with CT-based characterization guided by previously described morphologic patterns. Volar shear B1 fractures were defined as pa…

IMPORTANCE: Distal radius fractures and distal ulna fractures are common orthopedic injuries treated in emergency departments (ED). Our goal in this narrative review was to present emergency physicians with a single source that covers the evaluation and treatment of distal radius fractures and distal ulna fractures across the lifetime of our patients. There are differences in the treatment algorithms for distal radius fractures between pediatric, young adult, and geriatric patients.
OBSERVATIONS: Pediatric distal radius fractures are defined by their relation to the growth plate and are influ…

PURPOSE: Volar locking plate (VLP) fixation for distal radius fractures has relatively stable clinical outcomes. However, surgeons who assess postoperative outcomes as good may have difficulty fully understanding the perspectives of patients who express dissatisfaction. Central sensitization inventory (CSI) score is now thought to assess psychogenic pain. Therefore, clinical evaluation related to CSI score following volar locking plate fixation for distal radius fractures in this study.
METHODS: Twenty-nine distal radius fracture patients (12 males and 17 females; mean age: 55.2 ± 15.9 years)…

Distal radius fractures are among the most common fractures treated globally. Consideration of the mechanism of injury and careful radiographic assessment contributes to appropriate management of these injuries in isolation or jointly with other fractures or injuries affecting the wrist joint. Treatment options can range from non-operative management to a variety of fixation techniques. Interest in the distal radius fractures was renewed as open reduction and monoblock volar locking plate fixation became the preferred contemporary treatment in the last few decades. Isolated volar lunate facet …

Scapholunate complex injuries are the most frequent lesions associated with distal radius fractures and the treatment algorithm according to the stage of the instability remains controversial. However, there is an admitted consensus around the necessary treatment of the associated high-grade instabilities. They occur frequently in young patients after high energy trauma, and not treated, they can lead to chronic wrist pain and eventually to scapholunate advanced collapse. The routine use of the arthroscopy provides an accurate intraoperative staging of the lesions and allows a tailored treatme…

Distal radius fractures are the most common skeletal injuries requiring intervention in children. These injuries are classified by fracture pattern, location, displacement, and angulation. While each unique fracture pattern warrants slightly modified treatment plans and follow-up, the goals of treatment remain constant. Successful outcomes depend on restoration of motion and function, and attaining acceptable sagittal and coronal alignment is a necessary first step. For displaced fractures, closed reduction is often necessary to restore alignment; well-molded cast application is important to m…

INTRODUCTION: "Distal end radius fractures" account for almost 16% of all fractures and are the most common kind of upper limb fractures treated in hospital emergency departments. "Distal end radial fractures" occur more often in younger people due to high-energy trauma than in older people due to low-energy trauma. When comparing women and men of the same age, proximal end radii fracture are more prevalent in women than men owing to significant osteoporosis and a higher tendency for accidents in older women.
METHODOLOGY: This was a prospective interventional study in which participants were …

BACKGROUND: While complexity of distal radius fractures varies, volar plating is the most prevalent surgical option in adult injuries. The time between date of injury and surgical intervention varies according to several factors, including the timing of presentation and the surgeon's availability. This study aims to understand the impact of a delay in surgical intervention on operative time, patient-reported outcomes, and reoperation rates.
METHODS: A retrospective review was performed on patients treated with volar plating of distal radius fractures from 2017 to 2020 at a single institution …

Following low-energy distal radius fractures (DRF) patients rarely receive a bone health evaluation. The purpose of this survey was to investigate the attitudes and practices of American Society for Surgery of the Hand (ASSH) members regarding osteoporosis and low-energy DRF. An electronic survey was sent to 4,125 members of the ASSH. Physicians were asked about referral patterns, comfort level with labs and imaging related to bone health, and barriers. There were 475 responses (response rate 11.5%). Most respondents always (33.1%) or often (32.8%) talk about osteoporosis risk after low-energy…

Ultrasonography (US) is a noninvasive examination modality that is devoid of risk, both for the patient and the surgeon, compared with fluoroscopy. The principle is the same for distal radius and finger fractures: replace the fluoroscopy checks with US checks to reduce the patient's, surgeon's, and surgical team's exposure to radiation. In this article, the authors report their experience of the effectiveness of ultrasound imaging during the fixation of a distal radius and long finger fracture. They also describe equipment needed and surgical procedure.

Deep learning algorithms can be used to classify medical images. In distal radius fracture treatment, fracture detection and radiographic assessment of fracture displacement are critical steps. The aim of this study was to use pixel-level annotations of fractures to develop a deep learning model for precise distal radius fracture detection. We randomly divided 3785 consecutive emergency wrist radiograph examinations from six hospitals to a training set (3399 examinations) and test set (386 examinations). The training set was used to develop the deep learning model and the test set to assess it
