Referrals to the Wills Eye Institute Cornea Service after laser in situ keratomileusis: reasons for patient dissatisfaction.

PURPOSE: To review the symptoms, findings, and management options in patients referred to the Cornea Service who were unsatisfied with results after laser in situ keratomileusis (LASIK). SETTING: Cornea Service, Wills Eye Institute, Philadelphia, Pennsylvania, USA. METHODS: A retrospective chart review was conducted of all patients seen for consultation between January 1, 2004, and December 31, 2006, who had LASIK performed elsewhere. The parameters extracted were demographic data, history, symptoms, postoperative best corrected and uncorrected visual acuities, surgical complications, examin

Involvement of the endogenous hydrogen sulfide/Ca(v) 3.2 T-type Ca2+ channel pathway in cystitis-related bladder pain in mice.

BACKGROUND AND PURPOSE: Hydrogen sulfide (H(2) S), generated by enzymes such as cystathionine-γ-lyase (CSE) from L-cysteine, facilitates pain signals by activating the Ca(v) 3.2 T-type Ca(2+) channels. Here, we assessed the involvement of the CSE/H(2) S/Ca(v) 3.2 pathway in cystitis-related bladder pain. EXPERIMENTAL APPROACH: Cystitis was induced by i.p. administration of cyclophosphamide in mice. Bladder pain-like nociceptive behaviour was observed and referred hyperalgesia was evaluated using von Frey filaments. Phosphorylation of ERK in the spinal dorsal horn was determined immunohistoche

Association of cross-sectional area of the rectus capitis posterior minor muscle with active trigger points in chronic tension-type headache: a pilot study.

OBJECTIVE: To investigate whether cross-sectional area (CSA) of the suboccipital muscles was associated with active trigger points (TrPs) in … for 4 wks to record the pain history. TrPs in the suboccipital muscle were identified by eliciting referred pain to palpation, and increased referred pain with muscle contraction. TrPs were considered active if the elicited referred pain reproduced the head pain pattern and features of the pattern seen during spontaneous headache attacks. RESULTS: Active TrPs …

Imaging of the Acutely Injured Hip.

Acute hip pain following injury more commonly originates locally in and around the hip joint rather than being referred from the lumbar spine, sacroiliac joints, groin, or pelvis. Clinical assessment can usually localize the pain source to the hip region. Thereafter, imaging helps define the precise cause of acute hip pain. This review discusses the imaging of common causes of acute hip pain following injury in adults, addressing injuries in and around the hip joint. Pediatric and postsurgical causes of hip pain following injury are not discussed.

Deep brain stimulation for intractable pain: a 15-year experience.

OBJECTIVE: During the past 15 years, we prospectively followed 68 patients with chronic pain syndromes who underwent deep brain stimulation (DBS). The objective of our study was to analyze the long-term outcomes to clarify patient selection criteria for DBS. METHODS: Patients were referred from a multidisciplinary pain clinic after conservative treatment failed. Electrodes for DBS were implanted within the periventricular gray matter, specific sensory thalamic nuclei, or the internal capsule. Each patient was followed on a 6-monthly follow-up basis and evaluated with a modified visual analog s

Onion-skin Hemifacial Dysesthesia Successfully Treated with C2-4 Anterior Cervical Decompression and Fusion: A Case Report.

A 49-year-old man with cervical spondylosis at the C2-4 level presented with onion-skin hemifacial dysesthesia in addition to the right extremities. C2-4 anterior cervical decompression and fusion were performed. Onion-skin hemifacial pain disappeared after surgery. Although we cannot conclude the etiology of the pain was either referred pain or direct injury to the spinal trigeminal nucleus, cervical spondylosis at the middle cervical level has a possibility to present facial pain.

Diode Laser Clinical Efficacy and Mini-Invasivity in Surgical Exposure of Impacted Teeth.

The gold standard to arrange impacted teeth in the dental arch is represented by a surgical approach followed by orthodontic traction force application. In the literature, many surgical approaches are proposed to reach such a scope. The aim of the present study is to demonstrate how laser technique could positively assist surgical approaches.Study population was composed by 16 patients undergoing orthodontic treatment of 20 impacted teeth. In 10 patients (population A) surgical exposure of the impacted teeth was performed using a 980 nm diode laser, while in the other 10 patients (population B

Bilateral Medial Thalamotomy and Anterior Cingulotomy With Gamma Knife Radiosurgery for Refractory Pain: A Case Report.

Medial thalamotomy as well as cingulotomy has been a classical neurosurgical practice for pain, and the latter has also been used for treatment-resistant psychiatric conditions, particularly affective disorders. Here, we present the case of a patient with a seven-year history of progressive refractory neuropathic pain secondary to an intramedullary tumor treated solely with radiotherapy. She had regular emergency room visits for breakthrough pain unresponsive to high-dose opioids and was eventually referred to palliative care. Due to the severe impact of chronic pain on her quality of life, sh

Sensory and biomechanical responses to distension of the normal human rectum and sigmoid colon.

BACKGROUND: Visceral pain is a major clinical problem. The aim of the present study was to compare the pain and biomechanical responses to standardized distension of the human colon. METHODS: The relation between pain intensity and pressure, cross-sectional area (CSA) and tension-strain relations of the rectum and sigmoid colon were studied in 11 normal subjects following standardized distension using impedance planimetry. The bag was inflated stepwise with pressures up to 6 kPa. The subjects, who were blinded for the distension procedure, rated their pain intensity using an aggregate visual

Audit of abdominal pain in general practice.

An audit of 150 consecutive cases of abdominal pain presenting to an urban teaching practice between October 1983 and May 1984 was performed. The median duration of pain prior to presentation was two days. Females predominated in all age groups.Eighty-nine per cent of these patients were managed entirely in the practice and of these, 52 per cent were managed with reassurance and advice alone, while 48 per cent also received a prescription. Only 15 per cent of patients were investigated in any way by the practice. Of the 17 patients (11 per cent) referred, nine were referred as emergencies and

Does pain influence cognitive performance in patients with mild traumatic brain injury?

OBJECTIVES: Pain is still a neglected problem in mild traumatic brain injury (mTBI). In this cross-sectional study, we examined the frequency of musculoskeletal pain in a sample of adult patients with persistent cognitive symptoms after mTBI and whether pain level affected cognition. METHODS: The participants were 23 adult patients aged 18-50 referred to brain injury rehabilitation clinics for neuropsychological assessment after having sustained an mTBI. A non-injured control group (n = 29) was recruited through advertisements. The patients were, on average, assessed 22 months after trauma. A

Oro-facial pain and temporomandibular disorders classification systems: A critical appraisal and future directions.

It is a difficult undertaking to design a classification system for any disease entity, let alone for oro-facial pain (OFP) and more specifically for temporomandibular disorders (TMD). A further complication of this task is that both physical and psychosocial variables must be included. To augment this process, a two-step systematic review, adhering to PRISMA guidelines, of the classification systems published during the last 20 years for OFP and TMD was performed. The first search step identified 190 potential citations which ultimately resulted in only 17 articles being included for in-depth

Neurovascular pains: implications of migraine for the oral and maxillofacial surgeon.

Epidemiologic studies have shown that migraine headaches are a common finding in the general population, often associated with a high degree of disability. Additionally, migraine has a reported comorbidity with other medical conditions, most notably with chronic pains, such as temporomandibular disorders. The pathophysiologic mechanisms involved with migraine are suggestive of an increased and prolonged hyperexcitability to stimuli, especially within the trigeminal distribution. Because migraine is mediated by branches of the trigeminal nerve it has the potential to mimic other types of pains,

Characteristics of patients with low back and leg pain seeking treatment in primary care: baseline results from the ATLAS cohort study.

BACKGROUND: Patients with back pain radiating to the leg(s) report worse symptoms and poorer recovery than those with back pain alone. Robust evidence regarding their epidemiological profile is lacking from primary care, the setting where most of these patients will present and be managed. Our objective was to describe the characteristics of patients with back and leg pain, including sciatica, seeking treatment in primary care. METHODS: Adults visiting their general practitioner with back and leg pain, of any duration and severity, were invited to participate. Participants completed questionn

Three cases of referred sensation in traumatic nerve injury of the hand: implications for understanding central nervous system reorganization.

OBJECTIVE: The aim of this observational study was to explore whether patients with traumatic peripheral nerve injury of the hand perceive referred sensations; sensations that are perceived to emanate from other areas of the body than the part being stimulated. Referred sensations have been reported following amputation, somatosensory deafferentation, local anaesthesia, stroke, brachial plexus avulsion injury, spinal cord injury and complex regional pain syndrome type 1. DESIGN: Ten patients with ulnar or median nerve injuries underwent sensory testing of the face, upper body and legs, involv
