BACKGROUND: Retroperitoneal fibrosis (RPF) is a rare fibroinflammatory disease that leads to hydronephrosis and renal failure. In a case-control study, we have recently shown that asbestos exposure was the most important risk factor for RPF in the Finnish population. The aim of this study was to evaluate the relation of asbestos exposure to radiologically confirmed lung and pleural fibrosis among patients with RPF.
METHODS: Chest high-resolution computed tomography (HRCT) was performed on 16 unexposed and 22 asbestos-exposed RPF patients and 18 asbestos-exposed controls. Parietal pleural plaq…

OBJECTIVE: Retroperitoneal fibrosis (RPF) is a rare condition marked by inflammation and fibrosis affecting the peritoneal and retroperitoneal soft tissues. In recent years, the identification of IgG4-related diseases has brought to light a significant association with fibrous disorders, including RPF, which were once considered independent. In this comprehensive cohort study, we performed a comparative analysis of the demographic, clinical, laboratory, histopathological, and therapeutic characteristics between patients with IgG4-related RPF and those with idiopathic retroperitoneal fibrosis (…

INTRODUCTION AND IMPORTANCE: Retroperitoneal fibrosis is a proliferative disease of fibroblasts with a still unclear appearance and low incidence. The clinical manifestations are nonspecific and appear late, pain is the most common symptom present. Elevated serum IgG4 levels are observed in up to 60 % of the patients and the main goal of treating this condition is to preserve kidney function.
CASE PRESENTATION: We present a case of an asymptomatic 34-year-old man. A poorly defined mass in the pre-aortic, pre-caval and right rim regions with possible malignancy was observed. After 3 biopsies, …

Retroperitoneal fibrosis is a connective disease of the auto-inflammatory/auto-immune type of the retroperitoneum with unknown etiology and pathological mechanism. The manifestations of the pathology can be local or systemic. Amongst the local symptoms, the dull and constant pain in the hips, back or abdomen is the most frequent. We report here a case of a 47-year-old woman, whose pathogenic mechanism could be related to an "IgG4-related disease" disorder as suggested by an increased serum level of this subclass of IgG and the positive immunohistochemistry. The diagnosis is not easy, because t…

A large proportion of patients with retroperitoneal fibrosis (RPF) have been labelled as having idiopathic RPF historically. Recent reports of a subset of these patients having a fibro-inflammatory condition with characteristic histology and good response to therapy has helped in renewed understanding of an old disease. A 40 year old man presented with vague abdominal discomfort of 5 months duration. Ultrasound examination revealed soft tissue thickening with calcification over lower abdominal aorta. CT scan characterized the lesion as a retroperitoneal mass encasing the aorta with an associat…

BACKGROUND: The most frequent complication of retroperitoneal fibrosis (RPF) is an extrinsic compression of the ureter resulting in hydronephrosis. Because of the rareness of this uncommon chronic inflammatory disease, therapeutic results concerning the treatment of the obstructive uropathy are often known only for small study groups.
MATERIAL AND METHODS: The Else Kröner-Fresenius registry is a nationwide register of patients suffering from RPF in which all disease-related data are prospectively recorded. We performed a retrospective analysis of treatment outcome of different therapy strateg…

A 60-year-old man with a history of surgical resection of leiomyosarcoma of the right kidney had a bone scan to evaluate possible metastatic osseous disease. The bone scan showed significantly increased Tc-99m MDP activity in the right abdomen, corresponding to a heterogeneous mass in the right retroperitoneal region on the subsequent abdominal CT scan. These findings were initially interpreted as recurrent malignancy. However, the surgical pathology examination demonstrated that the lesion was retroperitoneal fibrosis, which was not previously known to have increased MDP uptake.

Retroperitoneal fibrosis is an uncommon disease that can be defined as the presence of a fibrous mass, which surrounds vascular structures and ureters. The start of the disease is insidious but can be diagnosed by radiological means such as computerized tomography and magnetic resonance imaging. Steroids and immunosuppressive agents may prevent progression toward advanced renal failure. We report a case of 41-year-old man who presented with bilateral renal colic as the only clinical manifestation of this disease and its favorable response to treatment.
