BACKGROUND: Surgical stabilization of rib fractures (SSRF) has been associated with improved pain scores, fewer ventilator days, lower rates of ventilator-associated pneumonia and tracheostomy, shorter hospitalization, and reduced mortality. RibScore is a 6-point scoring system using chest wall injury radiographic data to predict adverse pulmonary outcomes (APOs). This study examines the incidence of APOs using RibScore criteria, hypothesizing the incidence of APOs decreases after SSRF.
METHODS: A single-institution retrospective review was performed for adult SSRF patients at a Level I traum…

PURPOSE: Surgical stabilization of rib fractures (SSRF) has re-emerged as an important component of chest wall trauma care, yet its history is often presented as a simple timeline rather than as a source of operative and biomechanical lessons. This literature, problem-oriented historical review re-examines earlier rib fixation strategies in order to identify the mechanical and operative problems they were intended to solve, why many disappeared, and which principles remain relevant to contemporary SSRF practice.
METHODS: A literature review was conducted focusing on historically important fix

BACKGROUND: Surgical stabilization of rib fractures (SSRFs) has become an emerging therapy for treatment of patients with rib fractures. More commonly, it is used in the acute setting; however, delayed SSRF can be utilized for symptomatic rib fracture nonunions. Here, we describe our institution's experience with delayed SSRF, hypothesizing it is safe and resolves patient symptoms.
METHODS: This is a retrospective review of patients presenting to our Level I trauma center to undergo delayed SSRF for symptomatic nonunions from January 2017 to September 2022. Delayed SSRF was defined as SSRF ov

This paper presents 15 cases of deep venous thrombosis of the upper extremity as a complication of previously asymptomatic thoracic outlet syndrome. The diagnosis was based on clinical examination and phlebography. To achieve prompt recanalisation of the vein streptokinase was used. Fibrinolytic treatment was followed by surgical treatment carried out 6 to 8 weeks after resolution of acute symptoms. The surgery consisted of excision of first rib and accessory cervical rib when present through the axillary approach. In 73% of cases complete lysis of the thrombus was achieved which was confirmed

The research upon vital reactions is a hot topic in forensic pathology. However, there are very few studies which focus on the bone tissue a … nsic markers. This study provides the application of proteomics on human bone marrow of traumatized ribs with the purpose to a) define a significant pattern of vital reaction on broken ribs versus undamaged ones; b) evaluate the proteomic changes over different known survival times; c) assess proteomic differences among resuscitation fractures versus o …

BACKGROUND: Traditional rib anatomy and costal margin teaching contends that the costal margin consists of a combined costal cartilage made up of ribs 7 to 10. Variations in 9th and 10th rib anatomy have been observed. We sought to evaluate the variability of interchondral joints and the make-up of the costal margin.
METHODS: Cadaveric dissections were performed to evaluate the anatomy of the anterior ribs and the composition of the costal margin. Experienced chest wall surgeons evaluated this anatomy through a standardized dissection and assessment. Dissection videos were performed to allow …

BACKGROUND: Surgical stabilization in complex chest wall traumas, especially in case of posterior ribs arch destruction and flail chest, still remains a challenge for thoracic and trauma surgeons. Throughout the years different techniques and materials developed to address the complex anatomy of the ribs and reach an adequate fixation. Our aim is to present a prosthetic-free ribs stabilization technique which is easily reproducible and can achieve an efficient mechanical stabilization of the chest wall.
METHODS: This surgical technique can be summarized in three steps: muscle sparing thoracot…

PURPOSE: The aim of this paper is to propose a fracture model for human ribs based on acoustic emission (AE) data. The accumulation of microcracking until a macroscopic crack is produced can be monitored by AE. The macrocrack propagation causes the loss of the structural integrity of the rib.
METHODS: The AE technique was used in in vitro bending tests of human ribs. The AE data obtained were used to construct a quantitative model that allows an estimation of the failure stress from the signals detected. The model predicts the ultimate stress with an error of less than 3.5% (even at stresses …

Thoracic outlet syndrome (TOS) is a rare neurovascular disorder generally caused by the presence of a cervical rib or hypertrophic scalene anterior muscle that can compress the brachial plexus and/or subclavian vessels. In the vascular form, the symptoms are caused by the compression of the artery and/or the subclavian vein. In the first case, the compression is caused by the cervical rib and leads to hypo-perfusion with cooling and cyanosis of the upper limb, while in the second case, the compression is caused by the anterior scalene muscle and leads to congestion, cyanosis, swelling and pain

A 36-year-old male nonsmoker with fever, dyspnea, and cough of 2 weeks' duration presented to the emergency department. He reported left pleuritic chest pain and weight loss for the last 2 months. Chest radiology revealed a left lower lobe mass, a liver lesion, and bony destruction of right sacrum and ribs. Flexible bronchoscopy (FB) revealed an endobronchial (EB) tumorous lesion with cheesy material occluding the left lower lobe. Biopsies and cultures of the lung and rib lesions supported the diagnosis of tuberculosis (TB). Antituberculous therapy was started with rapid clinical improvement. …

The study of the angle of caudal declination of the ribs in 19 children aged 5 years, and in 23 children aged 8 years undergoing pectus excavatum repair, established that children with deformity had significantly wider angle compared to children of the same age without chest deformity. The angle of declination was not significantly changed 3 years after the operation. Thus it was concluded that subperichondral excision of the rib cartilage and osteotomy of the sternum led to fixed chest position. These data can explain persistent pulmonary dysfunction of the restrictive type in spite of succes…
