Hereditary hypophosphatemic rickets with hypercalciuria: report of a new kindred.

We report a new kindred of hereditary hypophosphatemic rickets with hypercalciuria. The symptomatic child and several relatives had increased renal phosphate clearance leading to hypophosphatemia, hyperabsorptive hypercalciuria, low PTH and increased 1,25-(OH)2D serum level. However, association with vitamin D deficiency and normal urinary excretion of cyclic AMP might suggest another tubular defect in phosphate transport.

Autosomal dominant hypophosphataemic rickets is associated with mutations in FGF23.

Proper serum phosphate concentrations are maintained by a complex and poorly understood process. Identification of genes responsible for inherited disorders involving disturbances in phosphate homeostasis may provide insight into the pathways that regulate phosphate balance. Several hereditary disorders of isolated phosphate wasting have been described, including X-linked hypophosphataemic rickets (XLH), hypophosphataemic bone disease (HBD), hereditary hypophosphataemic rickets with hypercalciuria (HHRH) and autosomal dominant hypophosphataemic rickets (ADHR). Inactivating mutations of the gen…

The development of rickets as a complication of chemotherapy for the treatment of Wilms' tumor.

Musculoskeletal complaints were the presenting symptoms in four of 44 children (9%) treated for relapsed Wilms' tumors with ifosfamide, a derivative of cyclophosphamide. Subsequent radiologic examination revealed rachitic bone and joint changes. A fifth child manifested clinical and radiographic signs of rickets after presenting with hypophosphatemia, hypouricemia, and glycosuria. Recognition of a potential drug-induced Fanconi syndrome is important when managing pediatric oncology patients previously treated with ifosfamide. Early replacement therapy is recommended when proximal renal tubular…

Familial glucocorticoid deficiency in a girl with familial hypophosphatemic rickets.

Familial glucocorticoid deficiency is a rare multisystem disorder characterized by glucocorticoid deficiency with normal mineralocorticoid activity, achalasia of the cardia, and alacrima. Familial hypophosphatemic rickets is characterized by selective renal phosphate wasting with subsequent hypophosphatemia and an inappropriately low 1,25-dihydroxyvitamin D concentration for the degree of hypophosphatemia. A 6-year-old girl with both disorders is described. A biochemical relationship between familial glucocorticoid deficiency and familial hypophosphatemic rickets could not be defined; the infl…

Primary hyperparathyroidism and rickets. A case report and review of the literature.

A case of primary hyperparathyroidism with clinical signs of rickets in a 15-year-old boy of South Morocco is presented. X-ray findings include a diffusely osteoporotic skeleton with areas of subperiostal resorption, cysts and metaphyseal rachitic changes. Hypercalcaemia, hypophosphataemia, increased alkaline phosphatase are found together with low calcidiol and high calcitriol plasma levels. The surgical removal of a chief-cell adenoma of a parathyroid gland leads to very rapid bone healing as well as normalization of blood chemistry. Reviewing the literature shows that 10 similar cases have …

[A sporadic case of vitamin D-resistant rickets in an adult].

A case of probably sporadic vitamin D-resistant rickets is reported. The patient had suffered from rickets of average intensity in childhood, and the signs had persisted into his adult life. Serious symptoms due to phosphate depletion and marked osteomalacia, however, did not appear until he was 56. A brief description of the differential diagnosis and a summary of the literature on this uncommon form are followed by an account of the gradual biochemical and clinical improvement observed after treatment with phosphates and vitamin D.

[Study of proteins of rat skeletal muscles in experimental rickets].

In dynamics of experimental rickets proteins of rat sceletal muscles were studied after and before physical exercises. Occurrence of rickets was estimated by following concentration of inorganic phosphorus and calcium in blood and by X-ray examination of knee joint. An increase in total amount of sarcoplasmic proteins was observed within the first week of development of the disease. After physical exercices the effect was noticed also within the second week of the development of rickets. Content of fractions of the sarcoplasmic proteins was similar in all animals and within all the periods stu…

X-linked hypophosphatemic rickets: case report with perspectives and challenges.

X-linked dominant hypophosphatemic rickets (XLH) is a rare genetic disorder caused by mutations in the PHEX gene and characterized by chronic hypophosphatemia due to dysregulation of fibroblast growth factor 23 (FGF23). This condition leads to bone deformities, growth impairment, and chronic pain. Conventional treatment with oral phosphate and calcitriol often has limited efficacy and is associated with adverse effects such as hyperparathyroidism and nephrocalcinosis. This case report describes the therapeutic outcomes of three pediatric patients with XLH treated with burosumab, a monoclonal a…

Case Report: Atypical presentation of rickets with hypocalcemia-related emesis.

BACKGROUND: Nutritional rickets, primarily resulting from vitamin D and/or calcium deficiency, is a well-recognized cause of skeletal and extraskeletal manifestations in children. However, gastrointestinal (GI) symptoms, such as vomiting, are not commonly reported as primary manifestations associated with hypocalcemia at the onset. CASE PRESENTATION: We describe a case of a 9-month-old male infant of Afghan origin who presented to the Pediatric Emergency Department with a 7-day history of isolated postprandial vomiting. Physical examination revealed a large anterior fontanel, but no significa…

Tumor-induced rickets/osteomalacia (TIO): diagnostic pitfalls and therapeutic options.

A 10-year-old girl developed bilateral gonalgia and waddling gait. Biochemical and imaging studies showed hypophosphatemia with impaired proximal tubular phosphate reabsorption and high intact fibroblast growth factor 23 (FGF23) level, indicating FGF23-related hypophosphatemic rickets/osteomalacia. However, genetic analyses for hereditary FGF23-related hypophosphatemic rickets were negative. 111In-pentetreotide scintigraphy and 18F-fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (PET/CT) which were available at that time, failed to find a tumor associated with tumor-i…

Burosumab Treatment for Autosomal Recessive Hypophosphatemic Rickets Type 1 (ARHR1).

CONTEXT: Autosomal recessive hypophosphatemic rickets (ARHR) are rare, heritable renal phosphate-wasting disorders that arise from overexpression of the bone-derived phosphaturic hormone fibroblast growth factor 23 (FGF23) leading to impaired bone mineralization (rickets and osteomalacia). Inactivating mutations of Dentin matrix protein 1 (DMP1) give rise to ARHR type 1 (ARHR1). Short stature, prominent bowing of the legs, fractures/pseudofractures, and severe enthesopathy are prominent in this patient population. Traditionally, treatment consists of oral phosphate replacement and the addition…

Hereditary Vitamin-D Dependent Rickets Type II: A Case Report.

Hereditary vitamin D dependent rickets type II is a rare genetic disorder in children characterized by early onset of rickets and deranged biochemical parameters. Low serum calcium level, high alkaline phosphatase, high parathyroid hormone, and high values of 1,25-dihydroxy vitamin D are characteristic biochemical findings. We are reporting a rare case of Vitamin D Dependent Rickets and subsequent improvement after addition of cinacalcet. This is a case report of a 2.5-year-child with Hereditary Vitamin D Dependent Rickets type II receiving cinacalcet as adjunct to oral calcium and calcitriol.…

[Periodontal status evaluation in adolescents with hereditary rickets-like diseases].

BACKGROUND: Hypophosphatemic rickets (HPPR) and hypophosphatasia (HPP) are hereditary pathologies accompanied by phosphorus metabolism problems and rickets deformities of the skeleton. Periodontal diseases, their diagnostic methods and factors affecting the progress of periodontal tissue damage are the least studied among the dental manifestations of HPPR and HPP. OBJECTIVE: The aim of present cohort study was evaluation of periodontal tissue status in adolescents with HPPR and HPP. MATERIALS AND METHODS: A dental examination of 20 adolescents aged 12 to 18 years with genetically and biochem…
