Tube-feeding decisions in the elderly.

Tube-feeding decisions in the elderly can be most challenging. We begin our decision making with the premise that artificial nutrition is a medical therapy and therefore that its prescription is based on a calculation of net benefits over burdens for the patient to whom it is given. When the burdens of this therapy outweigh the benefits of prolonged life, tube feeding may be ethically withheld or withdrawn. For the cognitively impaired and in the absence of known patient preferences, the ratio of benefits to burdens may best be figured after a time trial of therapy. If restraints are needed to

Network Analysis of Multidimensional Interactions Between Self-Regulatory Fatigue, Decision Conflict, and Quality of Life in Advanced Cancer Patients: Identifying Core Nodes for Precision Intervention.

Objective: To address the heavy burden of ego depletion and decision conflict in patients with advanced cancer, this study employed network analysis to explore their interaction mechanisms and identify key intervention targets, overcoming the limitations of traditional linear studies. Methods: A total of 200 patients with advanced cancer were assessed using the Self-Regulatory Fatigue Scale (SRFS), Decisional Conflict Scale (DCS), and Functional Assessment of Cancer Therapy-General (FACT-G). A Gaussian Graphical Model (GGM) was constructed to identify key nodes. Results: Network analysis revea

The role of non-governmental development organizations in the implementation of lymphatic filariasis programmes.

The Lymphatic Filariasis (LF) Non-governmental Development Organization (NGDO) Network was established to engage in supporting both international and national LF elimination agendas covering areas such as assisting ministries of health as an on-the-ground link between communities and programmes, which additionally gives the Network members an important voice from the field at international meetings; playing key roles in programme evolution (especially helping to both scale up and scale down mass drug administration [MDA] as elimination thresholds are met); having a role in operational research

Trauma and identity predictors of ICD-11 PTSD and complex PTSD in a trauma-exposed Colombian sample.

BACKGROUND: The 11th International Classification of Diseases introduces the diagnosis of Complex PTSD (CPTSD); characterized by traditional PTSD symptomology plus Disturbances in Self Organisation. Part of this construct involves feeling socially disconnected from others, suggesting that aspects of group and individual identity may be associated with this disorder. AIMS: The current study seeks to contribute to better understanding the association of individual social and personal identity in development of this disorder in post-conflict contexts. METHODOLOGY: This study analysed survey dat

Hierarchical cue control of cocaine seeking in the face of cost.

RATIONALE: Addiction is characterized by intermittent drug seeking despite rising costs. This behavior is heavily influenced by environmental stimuli that signal drug availability and reinforce drug seeking. OBJECTIVE: To establish the relationship between three key aspects of human drug use in rats: the intermittent, binge nature of drug intake, the motivational conflict of drug seeking in the face of escalating negative costs, and the ability of different drug cues to interact to modulate relapse. METHODS: Male and female rats were trained to self-administer cocaine on an intermittent acce

The Role of Food in the Family Relationships of Adolescents With Anorexia Nervosa and Bulimia in Northeastern Brazil: A Qualitative Study Using Photo Elicitation.

Family components can play roles both as protective factors and maintenance mechanisms of eating disorders. We aimed to investigate the role of food in the family relationships of adolescents with anorexia nervosa and bulimia in northeastern Brazil. Using photo elicitation, a visual narrative method that gives insight into the participants' perspective through photograph, we conducted semi-structured interviews with 26 people: four teenage girls with anorexia, four with bulimia, eight mothers, four fathers, five grandmothers, and one sister. Data were analyzed using the principles of Interpret

Comorbidities and the decision to undergo or forego destination therapy left ventricular assist device implantation: An analysis from the Trial of a Shared Decision Support Intervention for Patients and their Caregivers Offered Destination Therapy for End-Stage Heart Failure (DECIDE-LVAD) study.

BACKGROUND: Patients considering destination therapy left ventricular assist devices (DT LVAD) often have high comorbid burden but the association between these comorbidities and post-decision outcomes is unknown. METHODS: We included subjects in DECIDE-LVAD (NCT02344576), a stepped-wedge multicenter trial of patients considering LVADs, recording comorbidities per INTERMACS protocol. We compared decisional conflict, regret, perceived stress, quality of life (EQ-VAS), depression (PHQ-2), struggle with- and acceptance of illness by comorbid burden and amongst the most common comorbidities. Amygdalar activation of group I metabotropic glutamate receptors produces anti- and pro-conflict effects depending upon animal sex in a sexually dimorphic conditioned conflict-based anxiety model.

Women are more susceptible than men to develop anxiety disorders, however, the mechanisms involved are still unclear. In this study, we investigated the role of group I metabotropic glutamate receptors (mGluRs), a target for anxiety disorders, and whether estradiol may modulate conflict-based anxiety in female rats by using the Vogel Conflict Test (VCT). We used ovariectomized female rats with high (OVX+EB) and low (OVX) estradiol levels and intact male rats to evaluate sex differences. Infusion of (S)-3,5-Dihydroxyphenylglycine (DHPG), a group I mGluR agonist, into the basolateral amygdala, a

Further characterization of the prototypical nociceptin/orphanin FQ peptide receptor agonist Ro 64-6198 in rodent models of conflict anxiety and despair.

RATIONALE: Ro 64-6198, the prototypical non-peptide nociceptin/orphanin FQ peptide (NOP) receptor agonist, has potent anxiolytic-like effects in several preclinical models and species. However the effects of Ro 64-6198 on distinctive anxiety-provoking conditions related to unconditioned conflict behavior as well as its role in despair-like behavior remain to be addressed. OBJECTIVE: Here we examined the effects of Ro 64-6198 on unconditioned conflict anxiety using stimuli with different salience and on regulation of autonomic reactivity and compared these to the effects of benzodiazepine rece

Alignment and osteoarthritis of the knee.

Osteoarthritis is widely believed to result from local mechanical factors acting within the context of systemic susceptibility. Previous studies have demonstrated that malalignment is a potent predictor of disease progression in patients with osteoarthritis of the knee. Malalignment also mediates the effects of other risk factors, including obesity, quadriceps strength, laxity, and stage of disease. Recent studies have had conflicting results regarding whether malalignment increases the risk of incident tibiofemoral osteoarthritis. While the evidence is conflicting, malalignment appears to pla

Multiple personality disorder. A psychoanalytic perspective.

This article links multiple personality disorder (MPD) and dissociation to related theories of defense, resistance, and development in treatment. Trauma, conflict, and deficiency all play contributing roles in the genesis of MPD; the blend of each ingredient may be decisive in each patient. Internal conflict and deficiency tend to be overlooked, whereas trauma is emphasized. Other psychoanalytic concepts, such as transitional phenomena, identity, aggression, repetition, and the seeking after quiescence, are applied to MPD. In the discussion of working through, emphasis is placed on the process

Co-creation of community-based innovations to improve access to malaria treatment in conflict-affected regions of Cameroon.

BACKGROUND: In conflict-affected regions of Cameroon, access to malaria care is severely hindered by displacement, insecurity, and disrupted health systems. In response, we conducted an operational research aimed at breaking barriers to malaria services in conflict-affected communities of Cameroon. METHODS: In 2021, a participatory co-creation workshop was held on the 21st and 22nd of October 2021, bringing together stakeholders from government health services, community leaders, internally displaced persons (IDPs), and community health workers (CHWs)to collaboratively design interventions ai

Topography of the pain in classical trigeminal neuralgia: insights into somatotopic organization.

Trigeminal neuralgia is defined by its clinical characteristics of paroxysmal unilateral facial pain in a well-defined territory. Distribution of the pain may be in one or several of the cutaneous and/or mucous territories of the three divisions with V2 pain being the most frequent territory followed by V3 and V1. Factors determining the distribution of pain have not yet been systematically investigated. It is now well recognized that vascular compression factor is a predominant aetiology of classical trigeminal neuralgia. In this study we aimed to find whether there is a relation between the

Social Support and Peer Group Integration of Adolescents with Diabetes.

The aim of this study was to examine, through the roles of peers with regards to diabetes, the relationship between the support perceived by adolescents with diabetes and their peer-group affiliation. This is a descriptive, phenomenological and retrospective study based on a qualitative methodology. In-depth interviews with 15 people aged 18-35 with type 1 diabetes mellitus diagnosed in their childhood or adolescence were carried out. Data was analyzed through the interpretation of general discourses. Peers have considerable influence on adolescents and provide them social support from differe

Adolescents during wartime: Vulnerability-based profiles.

Adolescents in war zones face the dual burden of normative developmental challenges and stressors from their proximity to active conflict, raising critical questions about their vulnerability during wartime. The aim of the current study was to identify distinct vulnerability profiles among Israeli adolescents (ages 11-18) during Israel's ongoing Swords of Iron War. Drawing on Hobfoll's conservation of resources theory, we assessed protective factors (positive affect, resilience, and perceived social support) and one risk factor (war-related adjustment disorder symptoms). Differences in profile

Vector-borne parasites in dogs from Ukraine translocated to Poland following Russian invasion in 2022.

INTRODUCTION: Since 24 February 2022, the day the Russian aggression against Ukraine began, millions of refugees and thousands of pets crossed the Polish-Ukrainian border. Additionally, an unknown number of shelter and stray dogs and cats were rescued and translocated to Poland by private persons and non-profit organizations. The aim of the present study was to examine rescued dogs and cats for presence of canine vector-borne parasites to determine the role of armed conflict in spreading these parasites. METHODS: In July 2022 blood samples were collected from two animal shelters in central Po

Neural correlates of cognitive control deficits in children with reading disorder.

Reading disorder (RD) is characterized by deficient phonological processing, but children with RD also have cognitive control deficits, the neural correlates of which are not fully understood. We used fMRI to assess neural activity during the resolution of cognitive conflict on the Simon Spatial Incompatibility task and patterns of resting-state functional connectivity (RSFC) from task control (TC) regions in 7-12-year-old children with RD compared to their typically developing (TD) peers. Relative to TD children (n = 17), those with RD (n = 16) over-engaged a right superior/medial frontal clu

Observed Relationship Behaviors and Sleep in Military Veterans and Their Partners.

BACKGROUND: Emerging research has begun to examine associations between relationship functioning and sleep. However, these studies have largely relied on self-reported evaluations of relationships and/or of sleep, which may be vulnerable to bias. PURPOSE: The purpose of the study was to examine associations between relationship functioning and sleep in military couples. This is the first research to examine associations between observed relationship behaviors and subjective and polysomnographically measured sleep in a sample at-risk for both sleep and relationship problems. METHODS: The samp
