Incidental gynaecologic malignancies are detected in up to 1.3% of pelvic organ prolapse (POP) surgeries. PURPOSE: To determine if a patent fallopian tube associated with hydrosalpinx can be associated with unexplained implantation failure of morphological superior embryos.
METHODS: Salpingectomy was performed on a woman with a patent fallopian tube who had failed to conceive despite the previous transfer of 27 superior quality embryos. A single embryo was transferred using minimal stimulation.
RESULTS: Conception and normal delivery occurred.
CONCLUSIONS: A patent tube with a hydrosalpinx may be associated with failure of embryos to implant. Salpingectomy can correct this defect.

We describe a rare case of bilateral tubal pregnancy following natural conception in a woman with no other known risk factor than two former caesarean sections. Intraabdominal adhesions following the caesarean sections complicated the salpingectomy thus diminishing certainty of a healthy contralateral tube. The woman was readmitted three weeks later and had an additional salpingectomy on behalf of an unrecognized tubal pregnancy. Considering the rising caesarean section rates clinicians need to be certain of no additional ectopic pregnancy while performing the primary operation.

In 1978-1985 we operated on 111 tubal pregnancies pelviscopically. In 12 cases a salpingectomy was performed, in 99 patients the tubes both remained. 17 tubal abortions were extracted, in 82 cases the pregnancy was removed by salpingotomy. One second pelviscopy and one laparotomy were required because of postoperative bleeding. In one patient, a salpingectomy became necessary because of a post-operative infection. The pregnancy rate in 45 controlled infertility patients was 53%. A recurrence of ectopic pregnancy in the same or in the other tube occurred in 11%.
