Treatment of Ewing sarcoma family of tumors: current status and outlook for the future.

BACKGROUND: The Ewing sarcoma family of tumors (ESFT) comprises a group of well-characterized neoplasms with aggressive behavior. Despite significant progress with the use of intensive multiagent chemotherapy and local control measures, a significant proportion of patients die of disease progression. Chemotherapy dose intensification and autologous hematopoietic stem cell transplant (HSCT) have been explored by many institutions without obvious benefit in high-risk patients. Our current understanding in the biology and treatment of ESFT suggests that a more rational approach to the development

Fatal complications in a young adult with large Ewing sarcoma of the chest wall.

Ewing sarcoma is an aggressive bone and soft tissue malignancy arising predominantly in children and young adults, with a peak incidence at 15 years of age with a slight male predominance. The ribs are a recognized primary site of Ewing sarcoma, accounting for approximately 10% of cases. Tumors arising from the ribs/chest wall, often called Askin tumors, are the most common primary malignant chest wall tumor in children and young adults. These tumors are highly aggressive, often presenting with early micrometastatic disease and requiring prompt multimodal therapy. We present the case of a 20-y

Combined 177Lu-FAP-2286 Therapy and Chemotherapy in Extraskeletal Ewing Sarcoma With Multiple Metastases.

A 23-year-old man with metastatic extraskeletal Ewing sarcoma, who was unable to tolerate standard-dose chemotherapy, received 1 cycle of 177Lu-FAP-2286, followed by 2 cycles of reduced-dose ifosfamide/epirubicin/mesna chemotherapy. Follow-up 68Ga-FAP-2286 PET/CT at 2 months showed a marked metabolic response with reduced tumor size and tracer uptake. No adverse events were observed. The patient remained symptom-free at 2 months. This case suggests that 177Lu-FAP-2286 combined with chemotherapy may have clinical utility in advanced Ewing sarcoma.

Role of Nkx2.2 immunohistochemistry in distinguishing Ewing sarcoma from neuroendocrine neoplasms at different sites.

OBJECTIVES: Ewing sarcoma is a high grade round cell sarcoma that exhibits considerable histomorphological overlap with other round cell tumors, especially neuroendocrine neoplasms. In the present study, we compared the immunohistochemical expression of Nkx2.2 in cases of Ewing's sarcoma, neuroendocrine neoplasms at different sites, and a small subset of other mesenchymal and nonmesenchymal tumors in order to assess its utility in their distinction. MATERIALS AND METHODS: This descriptive retrospective study lasted for 14 months and investigated 60 cases, with 17 cases of Ewing sarcoma, 34 ca

Sacral Ewing Sarcoma Presenting With Cauda Equina Syndrome: A Case Report and Radiological Review.

Ewing sarcoma is an aggressive neoplasm that primarily affects children and young adults. Due to the complex anatomy of the sacral region and the proximity to critical neurovascular structures, these tumors often present a significant diagnostic and therapeutic challenge. In this case, the patient initially presented with lower back pain before worsening and presenting with cauda equina syndrome. This is a rather rare case of sacral Ewing sarcoma presenting with cauda equina syndrome in a pediatric patient. A high index of clinical suspicion and timely imaging play a vital role in diagnosis an

Primary Unilateral Ethmoidal Ewing Sarcoma With Orbital Extension: A Report of a Rare Case.

Ewing Sarcoma (EWS) is a rare, aggressive, small round blue cell tumor that predominantly arises in children and young adults. Sinonasal localization is a rare and difficult presentation to diagnose due to nonspecific clinical and histopathological features.  A 21-year-old male patient presented with acute left-sided epistaxis associated with periorbital swelling and epiphora. Contrast-enhanced CT revealed a well-defined lobulated heterogeneously enhancing iso-hyperdense lesion in the left ethmoid sinus with extensions and mass effect. Histopathological examination demonstrated features of a s

SNHG family lncRNAs promote Ewing sarcoma cell fitness through intronic snoRNA-guided rRNA modifications.

Over 90% of Ewing sarcomas (EWS) are driven by the EWS-FLI1 fusion oncoprotein. EWS-FLI1 acts as a pioneer transcription factor, altering the expression of hundreds of genes including many long non-coding RNAs (lncRNAs). The role of these lncRNAs in the oncogenesis of EWS is mostly unknown. We developed a CRISPR interference (CRISPRi) strategy to knock down expression of lncRNA loci expressed in EWS and evaluate their effect on proliferation in vitro and tumor growth in vivo. While most lncRNA knockdown had minimal effect on cell proliferation, selective inhibition of SNHG1, SNHG12, and SNHG30

Challenges in Diagnosing Primary Intracranial Ewing Sarcoma/Peripheral Primitive Neuroectodermal Tumor: A Case Report.

BACKGROUND: Primary intracranial Ewing Sarcoma/peripheral Primitive Neuroectodermal Tumor (EWS/pPNET) is exceedingly rare and easy to misdiagnose. CASE PRESENTATION: We present a case involving a 23-year-old male who presented with headaches and vomiting. The preoperative brain imaging revealed an irregular mass in the left parietal lobe, initially misdiagnosed as meningioma. However, the surgical specimen was ultimately diagnosed as primary intracranial EWS/pPNET. The patient underwent a total tumor resection, followed by adjuvant chemotherapy and radiotherapy. No recurrence or distant metas

Askin Tumor in an Adult: Recognizing a Rare Chest Wall Presentation of Ewing Sarcoma.

Ewing's sarcoma (ES) is an uncommon and aggressive bone tumor most commonly seen in children and adolescents. Chest wall involvement in adults is particularly rare. We report a 39-year-old woman with chest wall ES. She presented to the emergency department with unrelated symptoms, including flank pain and fever. A chest wall mass was incidentally identified on abdominal CT. Initial imaging had shown a well-circumscribed lesion that resembled a benign entity such as a lipoma or inclusion cyst, but over time, it progressively enlarged. Diagnosis was confirmed by imaging, histopathology, and mole

Updates on WHO classification for small round cell tumors: Ewing sarcoma vs. everything else.

The WHO Classification of Soft Tissue and Bone Tumours currently recognizes four categories of undifferentiated small round cell sarcoma: Ewing sarcoma, round cell sarcoma with EWSR1-non-ETS fusions including NFATc2 and PATZ1, CIC-rearranged sarcoma, and sarcoma with BCOR genetic alterations. These neoplasms frequently pose significant diagnostic challenges due to rarity and overlapping morphologic and immunohistochemical findings. Further, molecular testing, with accompanying pitfalls, may be needed to establish a definitive diagnosis. This review summarizes the clinical, histologic, immunohi

Primary Ewing Sarcoma in the Right Ventricle in an Adult Patient: A Case Report.

Ewing sarcoma, a highly aggressive round-cell neoplasm of unknown origin, rarely occurs as a primary cardiac tumor. In this intriguing case, we describe an instance of primary Ewing sarcoma in an adult patient's right ventricle (RV). The patient, a 27-year-old man, presented with symptoms including epigastric pain, fever, tachycardia, nausea, vomiting, shortness of breath, and a dry cough. Imaging studies, including a computed tomography scan and echocardiogram, revealed an anterior mediastinal mass arising from the free wall of the RV and extending into the pericardium. The mass exerted press

Characterization of myeloid signature genes for predicting prognosis and immune landscape in Ewing sarcoma.

Myeloid cells as a highly heterogeneous subpopulation of the tumor microenvironment (TME) are intimately associated with tumor development. Ewing sarcoma (EWS) is characterized by abundant myeloid cell infiltration in the TME. However, the correlation between myeloid signature genes (MSGs) and the prognosis of EWS patients was unclear. In this research, we synthetically characterized the expression of MSGs in a training cohort and classified EWS patients into two subtypes. Immune cell infiltration analysis revealed that MSGs subtypes correlated closely with different immune statuses. Furthermo

Intraneural Ewing Sarcoma of Fibular Nerve: Case Report, Radiologic Findings and Review of Literature.

BACKGROUND: Intraneural Ewing sarcoma (ES) was first described in 1918 by Stout in a tumor of the ulnar nerve. These tumors are in the category of ES family of tumors, together with ES of bone, extraosseous ES, and primitive neuroectodermal tumor. ES typically occurs in the second decade of life; only 20% of cases affect elder people. The most frequently involved sites are the craniospinal vault and cauda equina, while the peripheral nerve location is extremely rare (only 11 cases mentioned in the literature to date). CASE DESCRIPTION: This case report documents a rare case of a 46-year-old w

Tumorigenicity of Ewing sarcoma is critically dependent on the trithorax proteins MLL1 and menin.

OBJECTIVE: Vitamin D is a fat-soluble secosteroid hormone that regulates calcium, magnesium, and phosphate homeostasis and plays a pivotal role as antiproliferative and immunomodulatory mediator. Considering the different sources of synthesis and dietary intake as well as the pleiotropic actions in extremely diverse (micro)environments of the body, the supplementation of this Vitamin should be carefully evaluated taking into account the several pathways that it regulates. In the current brief review, we aimed to summarize the available evidence about the topic, in order to suggest the best evi
