[Long-term results of treatment of rare aseptic necrosis of the scaphoid bone (Preiser disease) with the Swanson implant].

In 1911 Preiser described the avascular necrosis of the scaphoid bone, which is regarded as a relatively rare clinical and radiological condition. The diagnosis is not established easily, and a small, avascular proximal fragment, e.g. in a scaphoid pseudarthrosis, should not be confused with Preiser's disease. Two histologically diagnosed cases are described: one of thema at operation 42-year old woman received a scaphoid Swanson implant in 1980. At follow-up eight years later, she presented with very satisfying clinical and radiological findings.

Cancellous compression bone grafting using headless screw as a strut in scaphoid nonunion by a single volar approach.

PURPOSE: An array of fixation and grafting techniques for scaphoid nonunion have been described over time, achieving varied results pertaining to union and scaphoid alignment. The aim of this study was to check for union rates and correction of scaphoid parameters achieved by our technique of using screw as a strut and cancellous compression grafting harvested from the distal radius by a single volar approach. METHODS: Retrospective analysis of all patients operated by the said technique was done from 2013 to 2019. Thirty-eight patients including 36 males and two females in the age range of 2

Arthroscopic Bone Graft and Internal Fixation of Non-Union of the Proximal Pole of the Scaphoid: Surgical Technique and Outcomes.

Background  Fractures of the proximal pole of the scaphoid have been associated with delayed union, non-union, and avascular necrosis. This has been attributed to avascularity of the proximal pole of the scaphoid. While proximal pole non-unions can be successfully treated using open techniques, there is little information in the literature regarding arthroscopic bone graft and internal fixation of proximal pole non-unions. Description of Technique  After insertion of a 1.2-mm radiolunate K-wire, the scaphoid non-union was arthroscopically excised, bone grafted with iliac crest cancellous bone,

Reconstruction of scaphoid nonunion fractures of the proximal one third with a vascularized bone graft from the distal radius.

OBJECTIVE: The treatment of proximal located scaphoid nonunion is a well-known and common problem. For these patients, we used a vascular pedicled bone graft of the distal radius. METHODS: In the last 7 years, 75 patients were treated with the vascular pedicled bone graft. Retrospectively, patients' data, healing rates, and factors influencing scaphoid healing were analyzed. RESULTS: The overall healing rate in cases with proximal located nonunions (n = 54) was approximately 70%. Out of these 54 patients, 47 patients showed avascular proximal fragments. Multivariate analysis showed no signif

Treatment for scaphoid fracture and nonunion--the application of 3.0 mm cannulated screws and pedicle vascularised bone grafts.

SUMMARY BACKGROUND: Scaphoid fractures are very common in wrist trauma, and scaphoid nonunions with avascular necrosis are frequent complications of a fractured scaphoid. The purpose of these two retrospective studies was to examine the clinical and x-ray results of treatments for acute scaphoid fracture and scaphoid nonunion. The surgical techniques of cannulated screw fixation and pedicled vascularised bone graft are described. METHODS: From 2001-2004, 80 patients with scaphoid fractures were treated with 3.0 mm cannulated screws and 5.5 mm threaded washers in our hospitals. The average age

Scaphoid nonunion treated with vascular bone grafts pedicled on the dorsal supra-retinacular artery of the distal radius.

BACKGROUND: Nonunion and avascular necrosis (AVN) are frequent complications of scaphoid fractures. We describe the operative technique and long-term results of treatment with pedicle vascularized bone grafts (VBGs) of the distal radius in patients with scaphoid fractures in whom nonunion and AVN developed. METHODS: During a 4-year period (1997-2000), VBGs pedicled on either the first and second or second and third compartments of the supraretinacular artery of the distal radius were in our hospital used to treat 11 patients with symptomatic scaphoid nonunion involving AVN of the proximal fra

[Bilateral bipartite carpal scaphoid bone: a congenital disease or unrecognized pseudarthrosis? Discussion a propos of a clinical case].

PURPOSE OF THE STUDY: Bipartite scaphoid has long been debated, and is of traumatic origin for some authors or congenital for others. The aim of the study is to distinguish clinical and radiographic criteria for the diagnosis of congenital bipartite scaphoid. MATERIAL AND METHOD: A case of bilateral bipartite scaphoid is presented. To distinguish pseudarthrosis from congenital bipartite scaphoid we used, the revised carpal height ratio of Nattrass, the CT, and magnetic resonance imaging. RESULTS: These examinations showed no collapse and they revealed evidence of articular cartilage around t

Fractures and non unions of the proximal pole of the carpal scaphoid bone. Internal fixation by a proximal to distal screw.

We report a new proximal to distal screw fixation technique for proximal pole fractures and non unions of the carpal scaphoid (type I of the Schernberg, Elzein and Gerard classification). Seven cases of non union with a live proximal polar fragment were operated from 1984 to 1986 with 4 excellent and 2 fair results and one failure. Three recent displaced fractures were also treated this way, with 3 excellent results. This useful technique is worth knowing and deserves a place in the treatment of scaphoid fractures, especially type I proximal pole fractures which represent a challenging problem

A comparison of the use of distal radius vascularised bone graft and non-vascularised iliac crest bone graft in the treatment of non-union of scaphoid fractures.

We compared two surgical techniques for the treatment of scaphoid non-union, namely, using distal radius vascularised bone graft and iliac crest non-vascularised bone graft. Eighty patients with symptomatic scaphoid non-union underwent surgical treatment, including 35 patients treated with distal radius vascularised bone graft and 45 treated by iliac crest non-vascularised bone graft. Patients were assessed objectively by examination of wrist range of motion, grip strength and radiographic findings in the postoperative period after a mean time of 2.8 (1.4) (range 1-5.2) years. Similar function

Treatment of non-union scaphoid fractures by Kirschner wires and ipsilateral distal radius bone grafts: a case series.

BACKGROUND: Non-union scaphoid fractures are a hazard to the wrist because of the increasing risk of avascular necrosis and degenerative changes. We managed non-union scaphoid fractures by open reduction, Kirschner (K)-wire fixation, and bone graft from distal radius. METHODS: This retrospective case series included seven patients with missed, established cases of non-union scaphoid fractures diagnosed by imaging at a tertiary care center between 2015 and 2017 in Nepal. All patients were treated with open reduction, K-wire fixation, and ipsilateral distal radius bone grafting. The patients we

Robot-assisted double screw fixation of minimally displaced scaphoid waist fracture nonunions or delayed unions without bone graft.

We retrospectively reviewed 12 minimally displaced fractures of the scaphoid waist in 12 patients who developed delayed or nonunions with or without conservative treatment. Mean time between injury and surgery was 6 months (range 3-12). The fractures were stabilized with double screws, which were percutaneously inserted with robot assistance, and without bone grafting. All fractures united at a mean of 8 weeks (range 6-10) after surgery. The patients were followed-up at 6 months and 1 year. The patients recovered good wrist function. No major postoperative complications were reported, and the

Scaphoid fracture: Bone marrow edema detected with dual-energy CT virtual non-calcium images and confirmed with MRI.

We aimed to determine whether bone marrow edema (BME) in acute traumatic scaphoid fracture could be demonstrated with dual-energy CT (DECT) using MRI as the gold standard. In recent years, virtual non-calcium (VNCa) images have been used to demonstrate BME in trauma cases, for example, in vertebral compression fractures, hip trauma to detect occult fractures and knee fractures. We present three cases of acute scaphoid trauma. Two patients had subtle or invisible fractures on x-ray and conventional CT images, while DECT VNCa images clearly visualized the BME, which was confirmed by MRI. One pat
