Chemotherapy-induced anterior necrotising scleritis: A case report.

PURPOSE: We describe a case of anterior necrotising scleritis secondary to Gemcitabine and Carboplatin chemotherapy agents in a patient with metastatic breast cancer, which has not been previously reported. OBSERVATIONS: A 50-year-old lady with recurrent metastatic breast cancer presented with unilateral subconjunctival haemorrhage secondary to severe thrombocytopenia eight days following palliative chemotherapy in the form of Gemcitabine and Carboplatin. Twelve days following the initial presentation, the subconjunctival haemorrhage had resolved, however there was evidence of anterior necrot…

Trigeminal autonomic cephalgia caused by recurrent posterior scleritis.

A 40-year-old woman presented with a side-locked headache with autonomic features, which then switched sides before reverting to the original side. The atypical features of side swapping, partial response to indometacin and abnormal optic disc appearances ultimately led to a diagnosis of recurrent posterior scleritis. We discuss the differential diagnosis of trigeminal autonomic cephalgias and its secondary causes, and provide practical pointers for its investigation and management.

Certolizumab pegol therapy for rheumatoid arthritis-associated scleritis.

PURPOSE: To report the successful treatment of rheumatoid arthritis (RA)-associated scleritis with the tumor necrosis factor alpha antagonist certolizumab pegol. METHODS: Retrospective case report of therapeutic interventions for a 47-year-old woman with scleritis and RA. After not tolerating treatment with other tumor necrosis factor alpha inhibitors and rituximab, or after that treatment failed, certolizumab pegol was administered in an attempt to achieve better control of her disease process. RESULTS: Six months after beginning the new therapy, it was well-tolerated and the patient's RA a…

Subconjunctival triamcinolone treatment for non-necrotising anterior scleritis.

AIMS: To determine the outcome of treatment with subconjunctival triamcinolone acetate for non-necrotising anterior scleritis and to review the literature on this treatment. METHODS: A retrospective, interventional case series of 12 patients who had failed systemic therapy, treated with 25 subconjunctival triamcinolone for non-necrotising anterior scleritis. RESULTS: Complete resolution of symptoms and signs of scleral inflammation occurred after 23 out of the 25 injections administered. For the remaining two injections, it was necessary to increase oral corticosteroids to obtain complete re…

Necrotizing scleritis following diode laser trans-scleral cyclophotocoagulation.

We report a case of necrotizing scleritis following diode cyclo-photocoagulation (CPC). A-64-year-old-Asian Indian male presented with pseudophakic corneal decompensation with secondary angle closure glaucoma that was uncontrolled with maximum medical therapy. Trans-scleral diode CPC was performed in the inferior 180-degree section using a standard diode laser with a G-probe. One month later, the patient developed features suggestive of necrotizing scleritis that resolved with systemic steroid therapy. Necrotizing scleritis following diode CPC has been rarely reported. Ophthalmologists should …

Scleritis after glaucoma filtering surgery with mitomycin C.

BACKGROUND: Adjunctive intraoperative mitomycin C increases the success of glaucoma filtering surgery, but also the postoperative complications. This is the first report of scleritis after its use. PATIENTS: In five patients, scleritis developed 3 to 24 weeks after inferior trabeculectomy with mitomycin C. The onset was characterized by acute pain, redness of the eye, and decreased vision. Three patients had anterior scleritis and two had posterior scleritis. Infection or bleb leakage was not seen in any patient. RESULTS: All patients responded to a course of topical steroids and oral ibupro…

Necrotizing scleritis after ocular surgery. A clinicopathologic study.

Necrotizing scleritis may appear after trauma to the sclera. The authors studied 10 patients in whom necrotizing scleritis developed after ocular surgery. The interval between surgery and onset of scleritis varied from 2 weeks to 6 months. Nine patients (90%) were found to have an underlying autoimmune vasculitic systemic disease, which was subsequently treated with immunosuppression. One patient was found to have a local infectious process, which was treated with antibiotics. Appropriate studies led to the discovery and subsequent treatment of a systemic disease or an infectious process in 6 …

Anterior scleritis with IgG4 lymphoplasmacytic infiltration: a case report.

PURPOSE: To report a case of chronic unilateral nodular anterior scleritis as a rare, isolated presentation of IgG4-related ophthalmic disease. CASE: A fifty-two-year-old patient was evaluated for painful, nodular scleral injection of the right eye, diagnosed as nodular anterior scleritis. There was only a partial response to topical corticosteroid and systemic immunomodulatory therapy. This, in combination with a sharp delineation between normal and abnormal sclera lead to the decision to perform a diagnostic biopsy of the lesion. Histopathology showed an IgG4 lymphoplasmacytic infiltration …

Clinical spectrum and management of anterior scleritis: case reports.

Scleritis is a rare, potentially sight-threatening, painful eye disease. Based on its anatomical involvement, it can be categorized as anterior and posterior scleritis. There are several possible causes, among which infectious and noninfectious origins should be considered. From the therapeutic aspect, it is important to clarify the infectious origin, to provide target treatment, or to identify the possible underlying autoimmune disease. Corticosteroid therapy is considered to be the basis for the stepwise treatment of scleritis. In this article, we describe the management of three patients (i…

Rhegmatogenous Retinal Detachment in Anterior Scleritis With Ulcerative Colitis.

Rhegmatogenous retinal detachment (RRD) is an ocular emergency as it is sight-threatening and requires urgent surgical intervention. Ulcerat … ed visual acuity (BCVA) was 6/6 in both eyes (OU). On examination, sectoral inferotemporal anterior scleritis (AS) with subclinical inferior RRD with peripheral holes in the lattice at the 6 o'clock position was noted. There was no posterior vitreous detachment. Optical coherence tomography (OCT) delineated the RRD objectively and was non-progressive for ni …

Posterior Scleritis Following COVID-19 Vaccination: A Case Report.

PURPOSE: To report a case of posterior scleritis following COVID-19 vaccination. STUDY DESIGN: Case report. RESULTS: A 78-year-old female presented with headache and right-sided visual loss 10 days after the second dose of COVID-19 vaccine. 'Examination showed disc oedema and a serous retinal detachment. B-scan ultrasound showed thickening of the posterior sclera with retroscleral fluid. CT head with venogram excluded venous sinus thrombosis. The patient's condition improved rapidly with oral corticosteroids with restoration of vision and resolution of disc swelling and serous detachment by …

Atypical Noninfectious Surgically Induced Necrotizing Scleritis in a Child.

Surgically induced necrotizing scleritis (SINS) is recognized as a rare and vision-threatening complication of ocular surgery. In adults, it has been mostly described after multiple ocular surgical procedures such as pterygium excision, glaucoma, and retinal detachment in the same eye. SINS is relatively less likely with single ocular surgery. It has been postulated that multiple surgeries may result in exposure of an antigen that leads to hypersensitivity reaction. Interestingly, it has also been reported after single strabismus surgery in adults. We present a case of unilateral surgically in…
