OBJECTIVE: To outline the problems around overlap between social phobia (SAD) and avoidant personality disorder (AVPD) and provide guidelines that may assist clinicians to differentiate these conditions.
CONCLUSIONS: A constellation of symptoms can be identified that may distinguish AVPD from SAD, with key features being a strong and pervasively negative self-concept, a view of rejection as equating to a global evaluation of the individual as being of little worth and a sense of not fitting in socially that dates from early childhood. It is important to identify the presence of AVPD in order

The author uses Joseph Campbell's (1949) account of the "Journey of the Hero" to examine a sequence of stages in which the hero must first use omnipotence in order to embark on an adventure, and then relinquish it in order to return to the ordinary world. The author detects parallels to this theme in descriptions of the fate of Lucifer in Milton's Paradise Lost (1674), and in the development of toddlers as described in Mahler's (1972) observations on separation-individuation. Finally, he traces a similar process in clinical material drawn from near the end of a patient's analysis.

Research on social anxiety and social anxiety disorder has proliferated over the years since the explication of the disorder through cognitive-behavioral models. This review highlights a recently updated model from our group and details recent research stemming from the (a) information processing perspective, including attention bias, interpretation bias, implicit associations, imagery and visual memories, and (b) emotion regulation perspective, including positive emotionality and anger. In addition, we review recent studies exploring the roles of self-focused attention, safety behaviors, and

OBJECTIVE: Bulimia nervosa (BN) is associated with low self-esteem. This study was designed to assess whether low self-esteem persists in nondepressed individuals who have recovered from BN (BN-R).
METHOD: Study groups included BN (n = 22), BN-R (n = 20), and healthy controls (n = 42). Participants were medication-free, and none met criteria for current major depression. Assessment instruments included the State Self-Esteem Scale (SSES), a self-rating scale designed to measure state-related changes in self-esteem.
RESULTS: Consistent with previous reports, SSES scores for BN were lower than

The concept of recovery is integral to the work of bereavement professionals, though there remains considerable debate as to the precise definition and applicability of the term. A number of factors have contributed to the controversy; competing bereavement models, for instance, have given rise to fundamental differences in the way that recovery is conceptualized, whereas an epistemological divide between researchers and practitioners has further hindered theorists' efforts to arrive at a definition with far-ranging explanatory power. An examination of fundamental concepts from bereavement lit

Four domains of evidence regarding the relationship between attention and social phobia are reviewed: (1) possible maintaining factor, (2) causal relationship, (3) specific relationship and (4) mediator of change during treatment. Two areas of research are covered and integrated: vigilance-avoidance of social threat stimuli and self-focused attention. There is empirical support for these processes as possible maintaining factors that are specific to patients and non-clinical samples with high levels of social anxiety. There is reasonable evidence to promote the use of attentional strategies in

OBJECTIVE: This study was designed to test two competing hypotheses regarding bias in self-report of weight and height in bulimia nervosa.
METHOD: General population samples of 102 young women with bulimia nervosa and 204 age and social class-matched healthy control women were recruited. Subjective and measured values of height and weight were obtained and compared within and between the groups.
RESULTS: The healthy control subjects reported that they were lighter and taller than they actually were, and the degree of this discrepancy was related both to their actual weight and, more strongly

Recent studies have begun to unravel the brain mechanisms that underlie the mental representation of the body. Imitation of movements by neonates suggests an implicit knowledge of the body structure that antedates the adult body schema. This can include inanimate objects that bear systematic relations to the body, as shown by the elimination from self awareness of a body part and its associated paraphernalia after selective brain lesions. Dynamic aspects of the body schema are revealed by spontaneous sensations from a lost body part as well as by orderly phantom sensations elicited by stimulat

The occurrence of a sports-related adjustment disorder can inhibit optimum athletic performance and scholastic achievement and impair the overall functioning of athletes. Physicians and trainers caring for athletic teams must maintain an acute awareness of the athlete's mental and physical well-being to detect subtle signs of a mood or psychological disturbance. The case of the collegiate basketball player discussed here exemplifies the subtle presentation and stages of an adjustment disorder with the predominant manifestation of depression mixed with anxiety and disordered conduct. This playe

Excessive motor activity in anorexia nervosa has been variously described as a symptom of a wider perceptual disorder, as the cause of anorexia, and as a neurologically based compulsive behavior. It is also considered a secondary symptom used to burn calories. In this paper, this symptom is considered from an existential perspective. It is first shown that the anorexic's mode of being is overly rational and not irrational as claimed by cognitive-behaviorists. It is then hypothesized that overactivity may help the anorexic person achieve a sense of existential permanence by dynamizing her stati

The results of experimental psychological examinations of the activating and protective functions of the level of ambitions in three groups of subjects (70 patients with moderately progressive shift-like and paranoid schizophrenia with a pronounced defect, 70 patients suffering from schizophrenia with an unmarked defect and 70 healthy controls) are presented. The study demonstrated that in schizophrenic patients with a marked defect as compared with healthy subjects and schizophrenics without any prominent defect, there is a decrease in the activating and an increase in the protective function

The comparative self-esteem and psychological well-being of female-to-male transsexuals, homosexuals, and heterosexuals (patient and nonpatient) were studied. Results indicated that individual candidates for sex-change surgery varied appreciably in self-concept and adjustment, and as a group were indistinguishable from psychiatric controls. Nonpatient homosexuals and heterosexuals demonstrated the most positive self-esteem and level of adjustment.

BACKGROUND: This study examined the structure of the self-concept in a sample of sexual trauma survivors with posttraumatic stress disorder (PTSD) compared to healthy controls using a self-descriptive card-sorting task. We explored whether individuals with PTSD possess a highly affectively-compartmentalized self-structure, whereby positive and negative self-attributes are sectioned off into separate components of self-concept (e.g. self as an employee, lover, mother). We also examined redundancy (i.e. overlap) of positive and negative self-attributes across the different components of self-con

The goal of this research was to expand theoretical models of adolescent suicide by exploring whether individual differences in adolescent girls' need for approval (NFAavoid and NFAapproach) contribute to risk for, or protection against, self-injurious thoughts and behaviors (SITBs). We examined these novel hypotheses in a series of concurrent and longitudinal analyses in two samples of adolescent girls (Study 1: N = 89, Mage = 16.31 years, SD = 0.84, 67.4% White; Study 2: N = 229, Mage = 11.80, SD = 1.80, 49.8% White). Hierarchical linear and logistic regressions revealed that NFAavoid was ge

Non-suicidal harmful behaviors pose a health problem of increasing importance, with a prevalence in community samples of adolescents of 15-20 %. Self-harm can have adverse meaning, qualifying in intrapersonal or interpersonal; they are a risk and evolutionary factor, with an age of onset in early adolescence. We present the case of a 12-year-old adolescent with psychosocial risk factors, who went to the consultation for self-cuts in arms and legs, made with a razor, detected in the school. It was enhanced her self-steem an guided in regulation of emotions with multidisciplinary intervention (e
