BACKGROUND: Proximal humerus fractures are the second-most common fragility fracture in older adults. Although reverse total shoulder arthroplasty (RTSA) is a promising treatment strategy for proximal humerus fractures with favorable clinical and quality of life outcomes, it is associated with much higher, and possibly prohibitive, upfront costs relative to nonoperative treatment and other surgical alternatives.
QUESTIONS/PURPOSES: (1) What is the cost-effectiveness of open reduction internal fixation (ORIF), hemiarthroplasty, and RTSA compared with the nonoperative treatment of complex proxi

OBJECTIVES: The aim of this study was to investigate the effectiveness of the proximal humeral internal locking system (PHILOS) plate combined with a custom neutral-position shoulder and elbow sling for proximal humerus fractures.
METHODS: A total of 112 patients with proximal humerus fractures were assigned randomly into 2 groups. Group A (n = 56) was treated by open reduction and internal fixation (ORIF) with a PHILOS plate; group B (n = 56) was treated by ORIF with a PHILOS plate in combination with the use of a custom neutral-position shoulder and elbow sling for 30 days after surgery. BACKGROUND: Proximal humeral fractures (PHFs) are among the most common fractures in elderly patients, but there is insufficient evidence from randomized controlled trials (RCTs) to determine which interventions are the most appropriate for their management. To date, no RCT has directly compared reverse shoulder arthroplasty (RSA) with nonoperative treatment for 3- or 4-part PHFs in elderly patients.
METHODS: This was a prospective RCT. The primary objective was to compare pain and function 12 months after fracture using the Constant score in patients aged 80 years or older with 3- and 4-part

HYPOTHESIS: Is salvage reverse total shoulder arthroplasty (RTSA) a justifiable treatment for failed operative treatment (open reduction-internal fixation [ORIF] or primary and secondary hemiarthroplasty) of proximal humeral fractures in patients younger than 60 years?
METHODS: Thirty patients (mean age, 52 years; age range, 30-59 years) were reviewed after a mean follow-up period of 11 years (range, 8-18 years). Of the patients, 7 (23%) underwent RTSA for failed ORIF and 23 (77%) for failed hemiarthroplasty. Clinical and radiographic outcomes were assessed longitudinally.
RESULTS: At final

An 82-year-old woman presenting with an acute, 4-part proximal humerus fracture-dislocation after a ground-level fall was treated with reverse total shoulder arthroplasty using a novel FiberTape cerclage tuberosity fixation technique. Surgery was done within 24 hours of injury using a deltopectoral approach. The reconstruction technique incorporated a standard metaphyseal press-fit stem, autologous bone graft, and a modified Six-Tec-based suture tuberosity repair combined with a tension-band construct derived from a prophylactic humeral shaft FiberTape cerclage (Arthrex) anchored distal to fra

BACKGROUND: Reverse shoulder arthroplasty (RSA) is widely used for the treatment of advanced glenohumeral osteoarthritis and complex proximal humeral fractures. However, comparative data evaluating implant survival and outcomes between fracture and elective indications remain limited. This study aimed to assess mid- to long-term implant survival, complications, radiographic findings, and functional outcomes following RSA, with particular attention to the influence of surgical indication.
METHODS: A retrospective cohort study included 126 patients who underwent primary RSA using a single impla

CASE: A 71-year-old woman sustained a left proximal humerus fracture dislocation. Imaging demonstrated displacement of the humeral head medial to the axillary artery without initial injury. To avoid iatrogenic injury, a separate medial axillary incision was successfully used to remove the humeral head and then reverse total shoulder arthroplasty was performed through a deltopectoral approach. At 1 year, the patient had excellent range of motion and PROMIS Physical Function CAT (v2.0) score was 46.2.
CONCLUSIONS: A separate medial axillary approach can be used to safely remove anterior/inferio

We report a rare pathological proximal humerus fracture caused by marginal zone lymphoma. The patient underwent reverse total shoulder arthroplasty followed by systemic therapy. At 15-month follow-up, functional recovery was excellent with no recurrence. Low-energy fractures without trauma history warrant suspicion of underlying pathology.

BACKGROUND: Reverse total shoulder arthroplasty has roughly a 4% risk of acromial stress fracture (ASF) and is associated with poor outcomes. Previous finite element analyses indicated that glenoid center of rotation lateralization significantly increased acromial strain; however, clinical studies have reported conflicting data with regards to implant position parameters on the incidence of ASFs. The purpose of this study was to evaluate whether medializing the glenoid component in patients at higher risk of ASF was associated with a lower incidence of ASF.
METHODS: A retrospective analysis w

HYPOTHESIS AND BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has become an increasingly popular treatment option for proximal humerus fractures (PHFs) in older patients. However, postoperatively, there lacks a universal protocol for the duration of arm immobilization in a sling. This retrospective review aims to determine whether the clinical outcomes of patients treated at our institution varied depending on their immobilization status following rTSA for PHF.
METHODS: All patients aged >50 years with a diagnosis of PHF who were treated with rTSA at a level 1 trauma center between Au

BACKGROUND: As the use of reverse total shoulder arthroplasty (rTSA) has steadily increased since Food and Drug Administration approval in 2003, there have been a number of unique complications recognized, including postoperative acromial stress fractures (ASFs). The incidence of ASF after rTSA is rare; however, the impact on clinical outcomes and patient satisfaction can be devastating. Despite a growing body of literature exploring risk factors for ASF following rTSA, there has been minimal investigation into prior subacromial decompression/acromioplasty (SAD) as a risk factor. The purpose o

BACKGROUND: The three- and four-part fractures of the proximal humerus are often combined with rotator cuff injury, and shoulder joint replacement is often selected clinically. The concept of "shoulder preservation" can greatly restore the function of the shoulder joint through internal fixation of the proximal humerus fracture and rotator cuff suture. This study investigated the clinical effect of the concept of "shoulder preservation" in the treatment of three- and four-part fractures of the proximal humerus.
METHODS: We conducted a retrospective analysis of 66 patients with proximal humera

BACKGROUND: The objective of our study was to compare patient-reported outcome measures, range of motion (ROM), complication rates, and reoperation rates after reverse total shoulder arthroplasty (rTSA) for proximal humeral fractures between patients treated acutely (≤30 days after injury), those treated after initial nonoperative treatment (>30 days after injury), and those treated after failed open reduction-internal fixation. Secondarily, we aimed to identify any patient-, injury-, or surgery-related factors independently associated with our response variables.
METHODS: We identified 576 p

Acute traumatic posterior glenohumeral dislocation in association with a massive rotator cuff tear is rare. Moreover, only few cases with interposition of the long biceps head of the tendon has been described to prevent reduction in posterior dislocation of the shoulder. In addition, combined scapula fracture with posterior shoulder dislocation also extremely rare. We present a case of Irreducible posterior fracture and dislocation of shoulder with massive rotator cuff tear due to incarceration of biceps tendon. For the treatment arthroscopic in situ superior capsule reconstruction was perform

Non-displaced proximal humerus fractures are usually managed non-operatively despite of minor malalignment. Biceps tendon rupture due to attrition after malunion is rare around the proximal humerus. Rupture of the long head of biceps (LHB) tendon usually occurs inside the joint close to the origin at the labrum. Treatment is usually non-operative with good outcomes. We report a rare case of a 48-year-old female patient with persistent locking and internal impingement 8 months after a proximal humerus fracture with anterior angulation leading to extra-articular reversed LHB tendon rupture with

Scapular spine stress fractures are a rare but well-recognised complication following reverse total shoulder arthroplasty (RTSA). They present a challenge with no consensus on management. Both operative fixation and conservative measures are associated with high rates of mal- or non-union and decreased functional outcomes. We present the case of a 60-year-old female, who presented with a scapular spine fracture one year following RTSA. Treatment consisted of initial immobilisation, physiotherapy and the application of a portable low-intensity pulsed ultrasound (LIPUS) system (EXOGENÒ Ultrasou

BACKGROUND: The purpose of this study was to determine factors associated with early symptomatic acromial and scapular spine fractures in patients who underwent reverse total shoulder arthroplasty (RTSA).
METHODS: We retrospectively evaluated all RTSAs performed by the senior author between 1/1/2013 and 6/1/2019. We evaluated patient demographics including gender, age, prevalence of comorbidities including osteoporosis, inflammatory arthritis, diabetes, and endocrine disorders such as hypothyroidism. We also evaluated preoperative and 2-week postoperative radiographs for center of rotation me

BACKGROUND: Failure of closed manipulative reduction of an acute anterior shoulder dislocation is seldom reported in the literature and is usually due to structural blocks such as soft tissue entrapment (biceps, subscapularis, labrum), bony fragments (glenoid, greater tuberosity) and severe head impaction (Hill-Sachs lesion).
CASE REPORT: We present a case of an irreducible anterior shoulder dislocation in a 57-year-old male patient after a road-traffic accident. He had severe impaction of the head underneath glenoid rim and associated fracture of the greater tuberosity. Closed reduction perf
