Comparison of functional outcomes between medialized and lateralized reverse shoulder prostheses in acute proximal humerus fractures.

HYPOTHESIS/BACKGROUND: Reverse shoulder arthroplasty (RSA) is increasingly used for complex proximal humerus fractures (PHFs). Although lateralized designs have demonstrated advantages in elective indications, their role in acute PHFs remains uncertain. This study evaluated functional outcomes following RSA for acute PHFs, with particular focus on the influence of greater tuberosity (GT) healing and prosthesis design. METHODS: In this multicenter retrospective study, 122 patients with acute PHFs treated with RSA and a minimum follow-up of two years were included. Two centers used lateralized

Does the stemless reverse arthroplasty compare to a conventional stemmed implant? Clinical and radiographic evaluation at 2 years' minimum follow-up.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a well-recognized treatment for many shoulder conditions, including rotator cuff arthropathy, primary glenohumeral joint arthritis, and rheumatoid arthritis, and can be used in both trauma and revision settings. Over the past 10 years, its popularity in New Zealand has been increasing, with a 6%-7% annual growth rate during this period. Stemless RSA designs have the following proposed advantages: They can preserve humeral bone stock, they can limit periprosthetic fractures, and they can be indicated in patients with abnormal diaphyseal humeral

Prior nonarthroplasty shoulder surgery and modifiable risk factors negatively affect patient outcomes after shoulder arthroplasty.

BACKGROUND: Total shoulder arthroplasty frequently is performed in patients with a history of shoulder surgery. The purpose of this study was to evaluate clinical outcomes after primary shoulder arthroplasty in patients with a history of nonarthroplasty shoulder surgery, and whether certain modifiable risk factors (MRFs) were negatively associated with final outcome measures. The secondary purpose was to determine if costs or complications were higher in patients with prior shoulder surgery. METHODS: We conducted a retrospective cohort study of all patients who underwent primary shoulder arth

Medialized vs. Lateralized Reverse Total Shoulder Arthroplasty for Proximal Humerus Fractures: A Systematic Review and Meta-Analysis.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is increasingly used in the treatment of proximal humerus fractures (PHFs) with reliable clinical improvement. Lateralized RSA implants have conferred superior outcomes compared with the original Grammont design in patients with nontraumatic indications. However, in the setting of a PHF, lateralized components can place increased tension across the tuberosity fracture site and potentially compromise tuberosity healing and outcomes. This systematic review and meta-analysis sought to determine the effect of implant design on clinical outcomes after

Wear and damage in retrieved humeral inlays of reverse total shoulder arthroplasty-where, how much, and why?

BACKGROUND: Polyethylene (PE) wear and material degradation have been reported as complications in reverse total shoulder replacements (rTSAs). In this regard, scapular notching is associated with more clinical complications. Therefore, the purposes of the study were to quantify the linear and volumetric wear, as a measure for the amount of removed material, and to qualitatively assess the PE damage modes to describe the material degradation in retrieved rTSA humeral PE inlays that contribute to failure of shoulder replacements. Furthermore, this study aimed to evaluate the effect of scapular

Clinical and radiological comparison of three different reverse shoulder arthroplasty designs for patients with primary osteoarthritis.

AIMS: In reverse shoulder arthroplasty (RSA), different implant designs range from medializing implants to strongly lateralizing onlay designs with different neck-shaft-angles (NSA). Thus different degrees of lateralization are currently used. Aim of this study was to compare clinical and radiological outcomes of three different implant designs in a homogeneous patient cohort with primary osteoarthritis (OA). METHODS: Patients with OA who underwent RSA between 03/2014 and 01/2020 were included and categorized into three groups based on RSA design: group MD (medialized-distalized design: eccen

The biomechanical assessment of two stemless shoulder arthroplasty prostheses in uniformly poor-quality bone mineral density cadaveric specimens.

BACKGROUND: Stemless shoulder arthroplasty offers several advantages, such as preserving bone stock and reducing periprosthetic fracture risk. However, implant motion can deter osteointegration and increase bone resorption, where micromotion less than 0.150 mm is crucial for bony ingrowth and vital to the success of the implant. The interaction between the implant and the metaphyseal bone and its effects on stability remains unclear. Therefore, this cadaveric study aims to assess the immediate stability of two stemless prostheses in low bone density specimens. METHODS: Twenty cadaveric should

The effect of humeral tray thickness on glenohumeral loads in a reverse shoulder 'smart' implant.

PURPOSE: The aim of this study was to observe the effects of changing humeral tray thickness on the resultant of intraoperative glenohumeral joint loads using a load-sensing system (LSS). METHODS: An rTSA was performed on fresh frozen full-body cadaver shoulders by using an internal proprietary LSS on the humeral side. The glenohumeral loads (Newtons) and the direction of the resultant force applied on the implant were recorded during four standard positions (External rotation, Extension, Abduction, Flexion) and three "complex" positions of Activity Daily Life ("behind back", "overhead reach"

Influence of implant design and parasagittal acromial morphology on acromial and scapular spine strain after reverse total shoulder arthroplasty: a cadaveric and computer-based biomechanical analysis.

BACKGROUND: The purpose was to analyze the influence of deltoid lengthening due to different implant designs and anatomic variations of the acromion and scapular spine (SS) in the parasagittal plane on strain patterns after reverse shoulder arthroplasty (RSA). METHODS: Ten cadaveric shoulders with strain rosettes placed on the surface of the acromial body (Levy II) and SS (Levy III) were tested using a shoulder simulator. RSA using humeral onlay (+3, +5, +8, +10, +13 mm) and glenosphere lateralization (0, +6 mm) was performed. Arm lengthening and magnitude of strain on acromion/SS were measur

Osseous Integration of the Central Peg of an All-Polyethylene Glenoid With 3 Different Surgical Techniques.

All-polyethylene glenoid components designed for osseous integration of the central peg can be placed with no graft (NG), autogenous bone graft (ABG), or demineralized bone matrix (DBM). The purpose of this study was to compare osseous integration with these 3 techniques. A randomized controlled trial was performed of 153 total shoulder arthroplasties using a pegged allpolyethylene glenoid component designed for osseous integration. Central peg treatment included NG, ABG, or DBM. The primary outcome was central peg osseous integration defined as bone presence between the central fins 1 year po

Comparison of proximal humeral bone stresses between stemless, short stem, and standard stem length: a finite element analysis.

BACKGROUND: The stem lengths of humeral components used in shoulder arthroplasty vary; however, the literature on these devices is limited. This finite element study investigates the effect of humeral component length on stresses in the proximal humerus. METHODS: Intact and 3 reconstructed (standard length, short, and stemless implants) finite element models were created from shoulder computed tomography scan data (N = 5). Loading was simulated at varying abduction angles (15°, 45°, and 75°). The average bone stress (represented as a percentage of intact values) was reported at 8 transverse s

Outcomes of revision reverse shoulder arthroplasty utilizing bulk femoral head allograft with a monoblock central screw baseplate for severe glenoid defects with minimum 2-year follow-up.

BACKGROUND: Revision shoulder arthroplasty in the setting of severe glenoid bone loss presents significant reconstructive challenges. The use of femoral head allograft (FHA) has emerged as a potential solution to restore glenoid bone stock and provide a stable foundation for component fixation. This study evaluates the outcomes and viability of FHA in managing severe glenoid defects during revision procedures. METHODS: A retrospective review of data collected from 2008 to 2023 from a single surgeon at a single institution using a single reverse total shoulder implant system with a monoblock c

Influence of subscapularis stiffness with glenosphere lateralization on physiological external rotation limits after reverse shoulder arthroplasty.

BACKGROUND: Repair of the subscapularis following reverse shoulder arthroplasty (RSA) remains a controversial topic among surgeons. Poor rotator cuff muscle quality is associated with increased musculotendinous stiffness, and the subsequent effect of compromised tissue repair on RSA functional outcomes remains unclear. The objective was to investigate the influence of subscapularis stiffness together with glenoid component lateralization on pre- and postimpingement joint mechanics during external rotation after RSA. METHODS: A validated finite element model incorporating the Zimmer Trabecular

Complications and revisions in anatomic and reverse short stem shoulder arthroplasty.

INTRODUCTION: One current trend in the field of shoulder arthroplasty is a design shift to shorter and metaphyseal fixed humeral stem components. The aim of this investigation is to analyze complications resulting in revision surgery after anatomic (ASA) and reverse (RSA) short stem arthroplasty. We hypothesize that complications are influenced by the type of prosthesis and indication for arthroplasty. MATERIALS AND METHODS: A total of 279 short stem shoulder prostheses were implanted by the same surgeon (162 ASA; 117 RSA), and 223 of these prostheses were implanted as primary procedures; in

Improved Clinical Outcomes After Lateralized Reverse Shoulder Arthroplasty: A Systematic Review.

BACKGROUND: Lateralized reverse shoulder arthroplasty (RSA) has emerged as an attempt to improve on some of the drawbacks of conventional RSA, such as glenoid notching and decrease in ROM. Although this new design is being used in clinical practice, the evidence is mostly limited to case series and has not been systematically reviewed. QUESTIONS/PURPOSES: (1) How much did patient-reported outcome measures (PROMs) and ROM improve among patients who receive a lateralized RSA implant? (2) What proportion of shoulders experience complications, revision surgery, or scapular notching? METHODS: The

Reverse shoulder arthroplasty for the treatment of three and four part fractures of the proximal humerus in patients older than 75 years old.

Reverse shoulder arthroplasty is an attractive alternative option in treating three- or four-part fractures of the proximal humerus. The main goal of the current study was to evaluate the functional and radiographic results after primary reverse shoulder arthroplasty of three- or four-part fractures of the proximal humerus in patients older than 75 years old. Between 2008 and 2010, 29 consecutive patients with a three- or four-part fracture of the proximal humerus undergoing a reversed shoulder prosthesis were included. There were 16 women and 13 men, with a mean age of 81 years (range 78 to 8…

Arthroplasty for weight-bearing shoulders.

INTRODUCTION: Repeated transfers and wheelchair propulsion in patients with a neurological deficit of the lower limbs overloads the upper limbs mechanically, particularly the shoulders, which become weight-bearing. Under these conditions, arthroplasty implants are subjected to large stresses, even though this indication is controversial in such a context. We hypothesized that joint replacement in weight-bearing shoulders will relieve pain and improve range of motion, with a positive impact on function and autonomy, without increasing the complication rate relative to the able-bodied population

Radiographic results of augmented all-polyethylene glenoids in the presence of posterior glenoid bone loss during total shoulder arthroplasty.

BACKGROUND: Chronic osteoarthritis can result in glenohumeral subluxation and loss of posterior glenoid bone. This can alter normal glenohumeral biomechanics and affect the stress placed on the glenoid implant after total shoulder arthroplasty. This study evaluated the clinical and radiographic results of an augmented all-polyethylene glenoid for the treatment of glenoid osteoarthritis in the presence of posterior glenoid bone loss and determined whether any failures or complications occurred with short-term follow-up. METHODS: During a 2-year period, 21 patients were treated with an augmente
