Motion sickness.

Motion sickness affects approximately 90 percent of the population at some time during life, and for many the problem is recurrent and severe. Labyrinthine-defective individuals are "immune." Adaptation is highly specific to one type of motion. Recent studies have focused on the role of the limbic system in mechanisms of motion sickness. Medications are sometimes useful, but they have a high incidence of side effects. Oral agents are ineffective after symptoms develop.

Emotional reactivity and the overreport of somatic symptoms: somatic sensitivity or negative reporting style?

OBJECTIVE: The purpose of this study is to examine the role of emotional reactivity (ER) in symptom reporting and conceptualize somatizing processes as a signal detection task. Emotional reactivity has been theorized to influence symptom reporting through somatic sensitivity as well as via a negative reporting style. We assess the degree to which these two competing theories about the role of ER are accurate within the signal detection framework. METHODS: We used a multimethod approach that included using both static and prospective self-reports as well as a signal detection task. RESULTS: R

[Fake pearls... imperative day somnolence--a personal experience].

In this review, narcolepsy is described based on personal experience of the author with this disease, especially with imperative day-somnolence and cataplexy. Narcolepsy is frequently underestimated and repressed. The economical (disability) and ethical (traffic accidents) implications of the disease may be significant. Optimal treatment and changes of behaviour are important.

"It isn't fair": postoperative depression and other manifestations of survivor guilt.

The objective of this paper is to demonstrate and explain an array of phenomena, clinical and social, that share a common dynamic although they seem to be quite disparate. These experiences range from combat neuroses to postoperative depressions. Situations and case histories demonstrating this dynamic are presented, namely, the guilt of getting more than one's share of life, material possessions, or physical gifts than the people one loves. The relationship of this getting more and the concept of the "zero-sum game" is discussed. Suggestions are presented for the treatment of depressions resu

[Skin disease and sexuality. An empirical study of sex behavior or patients with psoriasis vulgaris and neurodermatitis in comparison with skin-healthy probands].

Fifty-three patients with psoriasis, 24 patients with atopic eczema and 52 controls with healthy skin were compared with regard to their sexual behavior. For the test, questionnaires developed by Arentewicz as well as our own questionnaire on sexuality and partnership were used. Patients with skin diseases had a significantly impaired sex life, compared to those with healthy skin. There was a highly significant reduction in the exchange of tenderness in patients of both sexes, and in the capacity for orgasm in female patients. On the other hand, no significant difference was found concerning t

Heart-focused anxiety, illness beliefs, and behavioral impairment: comparing healthy heart-anxious patients with cardiac and surgical inpatients.

Psychological features and complaints of persons presenting to medical settings with heart-focused anxiety and noncardiac chest pain are poorly understood. Comparing 20 healthy heart-anxious patients to cardiac and surgical inpatients and nonpatient controls, we found that healthy heart-anxious patients (a) were as afraid of chest pain and heart palpitations as inpatients with heart disease, (b) were as incapacitated by symptoms and using medical services as much as both inpatient groups; and (c) reported higher levels of cardiac disease conviction, heart awareness, and behaviors designed to p

Procedural pain in a paediatric surgical emergency unit.

Pain induced by various types of procedures was assessed in the Paediatric Surgical Emergency Department at St Göran's Children's Hospital in Stockholm. Assessments of pain were obtained from the nurse, the parent, and children over 10 years of age by means of a visual analogue scale. In children aged 3-9 years, the Smiley Five-Face Scale was used. The nurse and the parent also answered questionnaires about analgesic medication, the child's behaviour, and the parent's overall opinion of the pain management, etc. Irrigation of the glans penis because of balanitis, treatment of fractures and par

Stressful events and psychological symptoms in patients with functional urinary disorders.

The relationship between micturition, stressful events, and psychological symptoms was studied in 58 female patients with functional micturition disorders (urethral syndrome) and 21 control patients. All received three questionnaires (Symptom Questionnaire, Illness Behavior Questionnaire, and the Biographic Questionnaire). Analysis highlighted the appearance of the urethral syndrome in highly stressful situations and the increase in the patient's anxiety, depression, dysphoria, hostility, and irritability. A tendency to complain of other psychophysiologic symptoms was also noted.

Incidence of significantly altered pain experience among individuals with developmental disabilities.

Absence of basic pain behaviors when an injury or illness is typically painful suggests that the experience of pain may be significantly altered. In the present study third-party reporting of injury/illness incidents was used to estimate the incidence of pain insensitivity/indifference and provide evidence of its impact. A significantly elevated pain threshold was noted for 25.2% of 123 individuals with developmental disabilities. Those with more severe mental retardation were also more likely to display signs of pain insensitivity or indifference. These individuals risk avoidable death and in

Psychosocial variables that affect the psychological adjustment of IVDU patients with AIDS.

The influences of coping strategies and of perceived social support from family and friends on the psychological adjustment of intravenous drug users (IVDUs) with Acquired Immunodeficiency Syndrome (AIDS) were investigated. Twenty-seven male AIDS patients with a history of intravenous drug use completed a demographic questionnaire, Trails-B, the Ways of Coping Questionnaire, Perceived Social Support from Family, Perceived Social Support from Friends, and the Mental Health Inventory. Unexpected results were obtained: The only coping strategy to correlate significantly with psychological adjustm

[Situational anxiety in myasthenia gravis].

Neuromuscular block can cause anxiety. These known experimental results lead to the expectation of conditioned phobic reactions in myasthenia gravis. A sample of 240 patients with the disease do not show the theoretically expected results in the Fear-Survey-Schedule. But interindividual variation of anxiety corresponds to some clinical parameters, relevant for the disease. Patients with facial muscular symptoms have higher values on social phobia but no higher scores on a depression-scale. Opposite to the facial-feedback theory, this result supports the hypothesis, that intersubjective feedbac

Conceptions of illness by children with juvenile rheumatoid arthritis: a cognitive developmental approach.

Investigated the conceptions of illness and accuracy of understanding about their disease for children with juvenile rheumatoid arthritis (JRA). 54 children between the ages of 6 and 17 were interviewed individually about various aspects of JRA, with results suggesting that accuracy and illness conceptions could be reliably measured. As predicted, children's understanding about their disease followed a developmental progression, with older children demonstrating a more sophisticated understanding of JRA than younger children (significant differences between age groups on 3 of the 5 questions).

Self-regulation and assessment approaches for vaso-occlusive pain management for pediatric sickle cell anemia patients.

An interdisciplinary program with the goal of teaching self-regulation and minimizing excessive narcotic use in the management of vaso-occlusive pain (V-O) in pediatric sickle cell anemia patients is described. A small group of patients do not respond well to any of our outpatients or inpatients medical regimens, leading us to suspect that psychological factors are influencing pain reports. We outline our multi-faceted assessment approach to evaluating such psychological factors, which includes a pain flow sheet, symptom check list, and a psychophysiologic profile. [Depressive reactions and psychological processing of HIV-positive homosexual males].

Interviews and psychotherapeutic treatments performed with HIV-positive males reveal frequent depressive crises in some of the men, whereas others are mentally relatively stable after having gone through a period of mourning over the loss of long-term life perspectives and prospects. The present study examined in 54 HIV-positive male homosexuals not suffering from AIDS, the causes of the depressive management by comparing a depressive and a non-depressive group. The results show: 1. that three well-defined styles of coping can be discerned, namely, "self-confrontation", "avoidance", and "seeki

[Psycho-rheumatic problems in patients with soft tissue rheumatism].

Patients with non-articular rheumatism syndrome demand a multidimensional judgment and an interdisciplinary care in a biopsychosocial understanding of the disease. In the psychosomatic consideration factors from the person-environment interaction of the patient gains increasedly importance. Thereby the communication of pain in the doctor-patient relationship and the valuation of pain in the family play a particular role. These regions of psychorheumatological problems are shown and discussed.

[Sudeck syndrome and its "psychosomatic disposition": a comparative clinico-psychologic study of the etiology in accident patients].

This study comprises a large psychological investigation of 10 patients with Posttraumatic Sympathetic Dystrophy of hands and compares the results with a control-group. It was found that a "psychosomatic disposition" with three factors completing each other occurs: a neurotic-depressive structure of personality including narcisistic problems, a posttraumatic disorder of adaptation with various emotional traits (as a reaction to the stress of the somatic trauma) and finally a less confidential style of interaction between the medical practitioner and the patient. [Clinical symptoms, psychopathology and intestinal motility in patients with "irritable bowel"].

Recent investigations of pathogenesis of the irritable bowel syndrome (IBS) either suggested psychopathology as the cause of bowel symptoms or proposed abnormalities in colonic motor and myoelectrical activity. Therefore, we prospectively compared clinical symptoms, personality traits, subjective stress reports, and motor and myoelectrical activity of the sigmoid colon and rectum in patients with IBS, patients with lactose malabsorption (LMA) who had not consulted a doctor for bowel complaints, and normal subjects. It could be shown that neither colonic activity nor stress distinguished patien

Mood changes associated with blood glucose fluctuations in insulin-dependent diabetes mellitus.

Individuals with insulin-dependent diabetes mellitus (IDDM) and their healthcare practitioners believe that extreme blood glucose (BG) fluctuations are characterized by changes in subjective mood states and emotional behavior, as well as physical symptoms. This study examined relationships between BG levels and self-reported mood in a group of 34 IDDM adults. The method followed a within-subject, repeated-measures design employed in previous studies of physical symptoms associated with diabetic glucose. Four times each day, participants completed a mood/symptom checklist just prior to a self-m
