Assessment of the Simultaneous Use of Biomaterials in Transalveolar Sinus Floor Elevation: Prospective Randomized Clinical Trial in Humans.

Implants inserted in the posterior maxilla frequently need additional surgery for successful bone augmentation. One of the most common procedures for this is transalveolar sinus floor elevation. There are different protocols for this procedure, and there is controversy over the simultaneous application of grafting material upon elevating. In this prospective randomized clinical study in humans, a total of 49 transalveolar sinus floor elevations were performed in 49 different patients, divided into a control group (without graft, 25 patients) and a test group (with graft, 24 patients). The anal

Sequential healing of the elevated sinus floor with different size of antrostomy: a histomorphometric study in rabbits.

PURPOSE: To study the influence of the access window dimensions on the healing at the antrostomy and within the augmented maxillary sinus. MATERIAL AND METHODS: A maxillary sinus augmentation was performed in twenty-four albino New Zealand rabbits. Antrostomies of 3 × 6 mm (small) or 5 × 6 mm (large) in dimensions were randomly prepared in each animal. A collagenated cortico-cancellous porcine bone was used to fill the elevated region, and an equine collagen membrane was placed on the antrostomies. Three different groups were formed, based on the time of euthanasia, i.e., 2, 4, and 8 weeks fr

Sinus lift through the use of a surgical guide: a case report.

AIM: Sinus lift is a common procedure to achieve bone height in the maxillary posterior region. This surgery can be performed through either a lateral or crestal approach. Preoperative planning usually entails CBCT and intraoperative evaluation at the time of surgery. This article presents a case in which a surgical guide was made by digitally planning for the performance of a sinus lift procedure through a lateral approach. CASE PRESENTATION: A 59-year-old partially edentulous female patient underwent sinus lift surgery using a stereolithographic guide designed in NemoStudio software and pri

Simultaneous sinus floor elevation and implant placement using leukocyte- and platelet-rich fibrin as a sole graft material.

PURPOSE: Sinus floor elevation (SFE) and simultaneous implant placement is predictable and reproducible. However, the graft material for the antral cavity remains a topic of debate. Considering the high osteogenic potential of the sinus membrane, most graft materials are generally accepted. This study aimed to assess the outcome of simultaneous SFE and implant placement, using leukocyte- and platelet-rich fibrin (LPRF) as a sole graft material. MATERIALS AND METHODS: This study was designed as a single cohort prospective study. Clinical and radiographic measurements (cone beam computed tomogr

A Less-Invasive Window Design for Lateral Wall Maxillary Sinus Augmentations.

The purpose of this article is to compare the larger (8 × 10-mm single implant and 15 × 20-mm multiple implant) lateral window designs used in the sinus elevation procedure with a newly proposed, less-invasive window design and discuss the advantages and limitations of the new design. The less-invasive window designs differ from the larger window designs in both the size and shape of lateral windows. Following creation of a round-window osteotomy, the quality of the sinus membrane and its ability to be elevated are assessed. The initial window can then be enlarged as a vertical oval window for

Osteogenic Potential of Porous Titanium. An Experimental Study in Sheep.

UNLABELLED: The search for osteoinductive as well as osteoconductive materials has led to the novel idea of using titanium in bone augmentations of the alveolar crest. Due to its excellent biocompatibility and favorable osteogenic properties, highly porous TiO2 granules has been proposed as a promising material for non-resorbable synthetic bone grafts in the restoration of large bone defects, and for bone augmentation in dental applications. OBJECTIVES: The aim of this study was to investigate the osteoconductive properties and biological performance of porous titanium granules used in osseou

The Palatal Window for Treating an Incompletely Augmented Maxillary Sinus.

Maxillary sinus augmentation through a lateral window is reported as one of the most predictable bone augmentation procedures before implant placement. The elevation of the membrane represents a delicate and crucial step that allows the creation of the space for the bone graft material. If the elevation is not completed, the regenerated bone might be inadequate for the implant placement. In this case, a new intervention will be necessary to complete the bone augmentation. Reaccessing from a lateral window, however, would be challenging due to thickness of the buccal boney wall because of the f

Accuracy of Schneiderian membrane thickness: a cone-beam computed tomography analysis with histological validation.

OBJECTIVES: Cone-beam computed tomography (CBCT) has been used in the literature to evaluate Schneiderian membrane thickness (SMT), but its accuracy has never been validated. The primary aim of this study was to compare the SMT measured by CBCT to the gold standard histological assessment. The correlations between SMT and anatomical structures of the maxillary sinus and alveolar bone were also tested. MATERIALS AND METHODS: Fourteen fresh cadaver heads were used for the study, and 28 sinus lift augmentation procedures were performed to obtain the membrane samples. Samples were fixed in formal

Maxillary Sinus Floor Elevation Surgery With BioOss Mixed With Enamel Matrix Derivative: A Human Randomized Controlled Clinical and Histologic Study.

Xenograft bone substitutes are commonly used to increase bone volume and height in the deficient posterior maxilla. The addition of enamel matrix derivate (Emdogain) could increase the efficiency of the bone healing process. The aim of this prospective randomized, controlled split-mouth design study was to compare the percentage of newly formed bone in sinus floor augmentation with deproteinized bovine bone mineral with or without the addition of enamel matrix derivative after 6 months of healing. Sixteen bilateral sinus floor augmentation procedures were performed. Deproteinized bovine bone m…

Successful Implant Placement via Simultaneous Nasal Floor Augmentation in an Inferior Meatus Pneumatization Case.

Partially edentulous patients who present with inadequate bone height in the posterior maxillary can predictably be rehabilitated with lateral wall sinus augmentation and subsequent implant placement. However, the sinus augmentation is defined by variations observed in the anatomical presentation of the maxillary sinus. Herein, we describe a case study managing sinus augmentation when a rare anatomic variant termed inferior meatus pneumatization was observed. A 65-year-old female patient presented, wherein the inferior meatus of the nasal cavity was located directly above the maxillary posteri

[Crestal core elevation technique--case series and literature review].

Sinus floor augmentation is the most common surgical procedure for gaining bone volume in the posterior maxilla. The purpose of this procedure is to enable implant placement un edentulous ridges. The most common techniques for sinus augmentation are: 1. Bone added osteotome sinus floor elevation (BAOSFE). 2. Crestal core elevation (CCE). 3. Lateral window technique (LWT). Since the early 80's many articles describing the successful use of different augmentation materials for sinus elevation have been published. Although many articles have been published on the lateral window technique and the …

Loss of pulp vitality after maxillary sinus augmentation: a surgical and endodontic approach.

BACKGROUND: Maxillary sinus augmentation is a routine procedure performed in implant dentistry in cases with sinus pneumatization. This study presents a series of clinical cases in which tooth devitalization occurred in conjunction with sinus augmentation. METHODS: In the three cases presented, a sinus-lift procedure was performed that resulted in devitalization of the adjacent teeth. Patients were referred to an endodontist for evaluation and treatment. Vitality of the teeth was determined by the use of a cold test, electric pulp test, and cavity test. The pulp was considered to be necrotic

Comparison of two different absorbable membranes for the coverage of lateral osteotomy sites in maxillary sinus augmentation: a preliminary study.

INTRODUCTION: Barrier membranes, both absorbable and non-absorbable, have been used in sinus augmentation for many years. Some years ago, a new autologous blood substrate called Platelet-Rich-Fibrin (PRF) was introduced, and to date, the supporting effect on bone regeneration has been controversial. This study aimed to evaluate the effect of PRF on bone regeneration when used as a barrier membrane at the lateral osteotomy site in sinus augmentation. MATERIAL AND METHODS: Twelve sinuses from six patients requiring bilateral sinus floor augmentation were treated with a two-stage surgical techni…
