Dural arteriovenous malformations of the skull base.

Dural arteriovenous malformations (AVMs) are interesting lesions, which can present as diagnostic and therapeutic challenges. Such AVMs were … 2000 we treated 400 AVMs, of which 46 were dural. Of these 46 dural AVMs, 18 could be classified as skull base AVMs. We restricted the designation of skull base AVMs to those AVMs located in the tentorium or adjacent to bones of the skull base. Two of these AVMs actually had their vascular nidus located in the bone. The common dural sinus AVMs were excl …

Acute (haematogenous) osteomyelitis of the skull in childhood.

A 13-year old boy presented with a 2-week history of right frontotemporal headache, fever and progressive painful swelling in that region. There was no preceding trauma. Clinical and radiological evaluation confirmed osteomyelitis of the right frontal bone. Staphylococcus aureus was found to be the causative organism. The affected bone was debrided and appropriate antibiotics administered. A subdural abscess developed requiring drainage. Hospital stay was 8 weeks. Haematogenous (pyogenic) osteomyelitis of the skull is rare in childhood and diagnosis may be missed or delayed. A high index of su…

[Giant cell reparative granuloma of the skull].

OBJECTIVE: To investigate the clinical course, histologic feature, diagnosis, differential diagnosis and treatment of giant cell reparative granuloma (GCRG). METHODS: Three cases of GCRG, which arose in the skull, were collected from 1987 to 1999 and analyzed clinicopathologically. RESULTS: Two patients had the lesion in the temporal bone and one in the orbit. One patient with GCRG had a recurrence after incomplete surgical resection. Histologically, all of three cases were characterized by fibroblastic proliferation with osteoclast-like giant cell rich areas, hemorrage, and newly formed bon…

[Asymmetry of the facial skeleton in Negroid skulls].

Measurements of 18 bilateral signs were carried out in 94 Negroid skulls. All parts of the facial skull are clearly asymmetrical, if regarded separately, while the summary sizes of the left and right halves of the facial skull are equal. In the upper part of the face the most asymmetrical parameters are the width of the bridge of the nose, width of the nose, and width of the palate, while the least asymmetrical are the height of the orbit and palate and length of the palate. In the mandibular part the greatest asymmetry was observed in the thickness and height of the mandibular corpus and heig…

Solitary plasmacytoma of the skull: a case report.

Solitary plasmacytoma of the skull is very rare and only 35 cases have been reported in the English literature. It remains controversial whether solitary plasmacytoma of the skull is essentially identical with solitary plasmacytoma of bone or not. Solitary plasmacytoma of bone including solitary plasmacytoma of the skull is characterized by a radiologically solitary bone lesion, neoplastic plasma cells in the biopsy specimen, fewer than 5% plasma cells in bone marrow, less than 2.0 g/dl monoclonal protein in the serum when present and negative urine test for Bence Jones protein (monoclonal lig…

Epidermoid cyst of the skull with nonpulsatile tinnitus.

We report an intradiploic epidermoid cyst of the skull responsible for transverse sinus compression and presenting with nonpulsatile tinnitus. Plain films and CT both demonstrated the tumour. Cerebral angiography showed best the degree of narrowing of the right transverse sinus, accompanied with turbulent flow probably leading to tinnitus. MRI demonstrated accurately both the tumour and the dural sinus compression. The tumour was totally removed, cranioplasty was performed, and the patient was discharged free of symptoms.

Congenital encephalocele of the medial skull base.

Meningoencephaloceles of the temporal bone are rare. Although most often seen following otologic surgery or trauma, congenital meningoencephaloceles can exist. The clinical presentation, diagnostic evaluation, and surgical management of three patients with congenital meningoencephalocele are presented. Two of the three patients presented to our institution with recurrent episodes of meningitis; one presented with partial complex seizures. Diagnostic evaluation included temporal bone computed tomography with magnetic resonance imaging. In two patients, defects were imaged following high-pressur

Alteration of rCBF in skull base lesions.

Cerebral functional imaging methods provide complementary information on brain function and large vessels regulatory controls which are compromised in cranial base lesions. The presence of a skull base tumor can alter regional cerebral blood flow (rCBF) in adjacent and remote brain tissue. This report presents the results of 104 CBF studies in 66 patients with skull base lesions (aged 17-75 years). The lesions included 36 meningiomas, 21 neurinomas and 9 other tumors. Regional CBF in tumor and brain tissue was measured prior to treatment using stable xenon enhanced computed tomography (Xenon(/…

[Repair of scalp defects with skull exposure].

The methods for repairing scalp defects and skull exposure are introduced. From 1980 to 1991, 9 cases of scalp defects and skull exposure from injury or tumor extirpation were treated with free omentum transfer with split skin graft, free flap or axial flap transfer. The surgical procedure was described in detail. For the whole defect of scalp with skull exposure the authors consider the free anterior-lateral femoral flap more appropriate and suggest that the free omentum flap be avoided as far as possible.

Growing skull fractures and their craniofacial equivalents.

Growing skull fractures occur most commonly after head injury; however, their "craniofacial equivalents" may occur after neurosurgical or craniofacial operations in pediatric patients. Experience with five separate cases is reviewed, including one case that involved the anterior cranial base and presented with vertical dystopia and proptosis. Necessary contributing factors appear to include (1) cranial bone defect, (2) dural tear, and (3) expanding intracranial process (e.g., growth of the brain). Prompt recognition and the diagnosis of the problem are essential to prevent the development of p…

Asymmetry of mandibular dimensions in European skulls.

Eighteen mandibular dimensions (mm) in sex-determined, 10th-12th-century European skulls from burial sites in South-Eastern Hungary were examined to discover whether differences exist between the right and left sides. There was a statistically significant difference in only one dimension, the smallest sagittal diameter of the mandibular neck, in both males and females. The frequencies with which the various 18 dimensions were identical between right and left sides ranged from 39.9% to 82.0% in males, and from 45.9% to 83.2% in females. The examined individual dimensions appeared to be smaller …

Radiographic manifestations of congenital anomalies of the skull.

Congenital anomalies of the pediatric skull are caused by a diverse group of disorders. For the purposes of this discussion, these entities can be classified according to the radiographic appearance of the skull, which may be similar in a variety of different diseases. Enlarged parietal foramina, sinus pericranii, aplasia cutis congenita, anterior fontanelle dermoid, cephaloceles, and craniolacunia are all examples of loceles, and craniolacunia are all examples of calvarial defects. Although there are numerous causes for wormian bones (Table 1), OI, cleidocranial dysplasia, congenital hypothyr…

[Fibrous dysplasia of the skull: neurologic manifestations].

The neurological manifestations of eight patients with osseous fibrous dysplasia of skull were retrospectively studied. In this series, the frontal bone was involved in 75% of the patients, the sphenoid or temporal bones in 37.5%, and the parietal in 25%. It was observed that cranial deformities occurred in all but one case, headache was present in 87.5% of the patients, visual disturbances in 50%, proptosis in 25%, and auditive symptoms in 12.5%. Six out of the eight patients were operated on: three of the four who presented progressive visual loss had postoperative improvement. In their foll…

Circumscribed lytic lesions of the thalassaemic skull.

Solitary or multiple circumscribed osteolytic areas are described in seven patients, selected from 250 patients with homozygous beta-thalassaemia (Cooley's anaemia). On X-ray examinations, these areas appear as purely osteolytic lesions with well-defined margins not associated with sclerosis. The possible mechanisms are discussed.

A case of aneurysmal bone cyst of the right temporal bone was reported. The patient was a 36-year-old male who was admitted to our hospital with complaints of decreased right hearing and transient impairment of the right vision. A large tumor was palpated on the right temporal bone. Neurological examinations revealed right auditory loss, along with right facial weakness of peripheral type, and minimal pyramidal signs on the right side. The results of the laboratory examination proved to be normal. Neuroradiological examinations tended to be quite impressive. Plain x-ray films of the skull show…
