Sphenoid sinus mucocele masquerading as a skull base malignancy.

Mucoceles of the sphenoid sinus are rare. Only occasional reports have appeared in the medical literature since they were first described by … riety of neurological signs and symptoms. The radiographic findings were felt to be consistent with skull base malignancies. The diagnosis was corrected only after surgical exploration of the sphenoid sinus revealed a mucocele.

Anterior skull base surgery for benign and malignant disease.

Teamwork between the head and neck surgeon and the neurosurgeon utilizing the craniofacial resection technique greatly extends the frontiers of surgery for tumors of the anterior half of the skull base. Tumors for which this technique may be used include benign and malignant tumors arising in the frontal bone or frontal sinus, nasal vault, ethmoid, maxilla, sphenoid or orbit. Selected nasopharyngeal lesions such as angiofibromas with anterior or middle fossa extensions and sphenoid or clival chordomas may also be approached in this manner. This report evaluates the surgical techniques currentl…

Effect of routine extradural optic canal decompression performed by skull base trained surgeons on visual outcomes in patients with anterior skull base meningiomas.

PURPOSE: Optic canal decompression is a surgical option in anterior skull base tumors with optic nerve involvement. Meningiomas may grow into the optic canal even without evidence of involvement on MRI studies. We aim to investigate the effect of routine optic canal unroofing performed by skull base trained surgeons versus general neurosurgeons on the postoperative visual outcomes in anterior skull base meningiomas. METHODS: Between January 2013 and October 2023, consecutive patients in our institution who underwent craniotomies due to visual impairment were retrospectively reviewed. Patient …

The Making of a Skull Base Team and the Value of Multidisciplinary Approach in the Management of Sinonasal and Ventral Skull Base Malignancies.

The management of sinonasal and ventral skull base malignancies is best performed by a team. Although the composition of the team may vary, it is important to have multidisciplinary representation. There are multiple obstacles, both individual and institutional, that must be overcome to develop a highly functioning team. Adequate training is an important part of team-building and can be fostered with surgical telementoring. A quality improvement program should be incorporated into the activities of a skull base team.

Prospective validation of a patient-reported nasal quality-of-life tool for endonasal skull base surgery: The Anterior Skull Base Nasal Inventory-12.

OBJECT: Patient-reported quality-of-life (QOL) end points are becoming increasingly important health care metrics. To date, no nasal morbidity instrument specifically designed for patients undergoing endonasal skull base surgery has been developed. In this study, the authors describe the development and validation of a site-specific nasal morbidity instrument to assess patient-reported rhinological outcomes following endonasal skull base surgery. METHODS: Eligible patients included those with planned endonasal transsphenoidal surgery for sellar pathology identified in outpatient neurosurgical…

Modified three-dimensional skull base model with artificial dura mater, cranial nerves, and venous sinuses for training in skull base surgery: technical note.

Experience with dissection of the cavernous sinus and the temporal bone is essential for training in skull base surgery, but the opportunities for cadaver dissection are very limited. A modification of a commercially available prototype three-dimensional (3D) skull base model, made by a selective laser sintering method and incorporating surface details and inner bony structures such as the inner ear structures and air cells, is proposed to include artificial dura mater, cranial nerves, venous sinuses, and the internal carotid artery for such surgical training. The transpetrosal approach and ep…

Giant Prolactinoma Presenting As a Base of Skull Tumor With Nasopharyngeal Extension: A Potential Diagnostic Pitfall in Neuroendocrine Lesions of the Base of Skull.

Pituitary adenomas presenting in uncommon anatomical locations are commonly misdiagnosed. Dramatic clinical presentation with hemorrhage and infarction, along with a lack of endocrine symptoms may further confound the diagnosis in some patients as illustrated in one of our two previously reported cases of non-small cell neuroendocrine carcinoma of the sinonasal tract and nasopharynx. This report presents the clinical progress of case number 2, which has a revised diagnosis of giant lactotroph pituitary adenoma. Common clinical, radiological and pathological pitfalls in diagnosis of neuroendocr

Incidence, Clinico-Radiological Features and Outcome of Skull Base versus Non-Skull Base Meningiomas Treated in Kuala Lumpur General Hospital: A Five-Year Experience.

BACKGROUND: Meningiomas are the most common intracranial tumours; they account for 13%-26% of all the primary intracranial tumours. Skull base meningiomas make up 25% of all meningiomas and are one of the most difficult intracranial tumours to be managed surgically. This is due to the fact that it is difficult to approach the lesions which are also close to vital structures such as cranial nerves and major blood vessels. Despite the abundance of these cases in Malaysia, local data on meningiomas is scarce. METHODS: This is a retrospective study consisting of 199 patients with meningiomas who …

[3-dimensional (3-D) image reconstruction of the facial skull and skull base in computerized tomography].

The diagnostic value of three-dimensional (3-D) image reconstruction in conditions affecting the craniofacial skeleton and the skull base has been assessed and the results compared with the CT scans used. Owing to reconstruction artefacts and some limitations in the exact display of 3-D images, no substantial improvement in diagnosis can be expected. Nonetheless, 3-D images facilitate the analysis and detection of complex fractures of the craniofacial skeleton and improve surgical planning and postoperative evaluation.

Skull Base, Bone, Pituitary-Regions around Orbit that Affect Vision.

This article provides an imaging-focused overview of conditions affecting vision that localize to the central skull base. Both neoplastic and nonneoplastic processes may arise from the sphenoid bone, sella turcica, and adjacent structures, with potential involvement of critical neurovascular pathways. Because clinical presentations in this region often overlap, identifying distinguishing features on computed tomography and MR imaging is essential for accurate diagnosis. The discussion highlights key imaging findings of pathology affecting visual function arising within and around the central s…

Skull Base Anatomy Presented in 360° Photogrammetry 3-Dimensional Models.

BACKGROUND AND OBJECTIVES: Realistic 3-dimensional (3D) anatomical models may enhance three-dimensional comprehension of the complex anatomy of the skull base. Such models may be generated by photogrammetry, a technical algorithm for photorealistic surface scanning. This study presents a 360° visualization of key neuroanatomical landmarks of the central skull base. METHODS: One formaldehyde-fixed head specimen with polymer-injected vessels underwent extensive exocranial and endocranial dissections on the right side after a sagittal cut. Initially, the dissection of the orbital region, caverno…

Case Report: Genomic characterization of a rare skull-base plasmacytoma.

Skull-base plasmacytoma (SBP) is a rare plasma cell neoplasm that typically presents as a slow-growing skull-base lesion in older adults; however, its molecular underpinnings are poorly characterized, limiting targeted therapeutic interventions and precise prognostication. We describe the case of a 67-year-old female who presented with progressive headaches, whose imaging and biopsy confirmed a solitary SBP with no evidence of systemic disease. Whole-exome sequencing revealed an elevated somatic copy number alteration burden and pathogenic variants in ARID1A, KMT2D, BCL7A, PTPN11, and NUP214,

An Unusual and Discrepant Presentation of a Skull Base Paraganglioma.

Paragangliomas are rare tumors of neuroendocrine origin. Within the head and neck, these tumors are slow-growing and locally destructive, with a small malignant potential. Vagal paragangliomas (VPs) originate from paraganglia around the vagus nerve, typically at the level of the skull base. Cranial nerve deficits are common at presentation, with the vagus nerve and hypoglossal nerves being most affected. Similarly, hypoglossal paragangliomas (HPs) originate from around the hypoglossal nerve but are extremely rare and less documented. We describe the case of a patient presenting with an isolate…

Do maxillary dentures protect the skull base from penetration injury?

KEY CLINICAL MESSAGE: Foreign bodies penetrating from the oral cavity can damage surrounding tissues. This case is considered an extremely rare and fortunate instance in which a maxillary denture appeared to weaken the external force and change the direction of the scissors, preventing damage to vital organs. ABSTRACT: The patient was a 73-year-old man. While on a ladder pruning a plant, he accidentally fell. The gardening scissors passed through the maxillary sinus from the maxillary alveolus and penetrated below the zygomatic arch. At the time of injury, the patient was wearing a metal-fram

Preoperative Vascular and Cranial Nerve Imaging in Skull Base Tumors.

Skull base tumors such as meningiomas and schwannomas are often pathologically benign. However, surgery for these tumors poses significant challenges because of their proximity to critical structures such as the brainstem, cerebral arteries, veins, and cranial nerves. These structures are compressed or encased by the tumor as they grow, increasing the risk of unintended injury to these structures, which can potentially lead to severe neurological deficits. Preoperative imaging is crucial for assessing the tumor size, location, and its relationship with adjacent vital structures. This study rev…

Extensive Base of Skull Osteomyelitis: Why Does It Still Occur?

Skull base osteomyelitis (SBO) is a rare yet serious intratemporal infection that often masquerades as a skull base malignancy. It is most common in diabetic and immunocompromised patients. We present a case of an elderly diabetic patient with end-stage renal disease with progressive malignant otitis externa. The disease progressed to involve the base of the skull, causing multiple cranial neuropathies. Early initiation of intravenous (IV) antibiotics, along with supportive treatment, may improve the long-term prognosis of the disease. This case highlights the importance of keeping a high inde…
