The present study aimed to investigate outcomes following open surgery for extensive skull base ORN. Open surgery through a personalized sequential approach was employed to deal with five cases of extensive skull base ORN. Two patients with mild cases underwent regional debridement and sequestrectomy, and three patients with severe cases underwent extensive resection with reconstruction using free anterolateral thigh (ALT) flap. Biological glues and vascularized flaps were used for obturation of the skull base bony defect to prevent postoperative cerebrospinal fluid (CSF) leakage. The infectio

OBJECTIVES: The parapharyngeal space (PPS) and infratemporal fossa (ITF) present significant challenges for endoscopic skull base surgery due to their complex anatomy and lack of clear bony landmarks. This study aims to propose a systematic compartmentalization of the PPS and ITF, based on key anatomical structures, to optimize surgical planning and approaches.
METHODS: To retain a precise bony reference framework, the walls of the maxillary sinus and pterygoid bone were preserved. Anatomical dissection was conducted along muscular planes, dividing the region into 3 spaces. The feasibility an

OBJECTIVE: As compared with the normal anatomy, structures of the skull base and its surroundings have been dramatically altered of the lesion near the skull base. How to remove the lesion at the skull base was mainly depending on surgeon's personal experience during past years. In this study, the authors explored the safety and accuracy of the surgical navigation system in resecting lesions near the skull base.
METHODS: The surgical cases consisted of 15 patients who underwent surgery involving the skull base with the using of surgical navigation technology. Five patients had adenoid cystic

Although conventional imaging can depict the anatomy of the head and neck with exquisite detail, it often falls short in its ability to characterize tissue physiology and abnormality; this is especially seen in the posttherapy setting where benign posttreatment changes and recurrent tumors can show intense postcontrast enhancement and similar features on conventional imaging. Advanced imaging can evaluate tissue physiology and, along with conventional imaging, provide a more accurate assessment of the skull base. This article describes the technical details and clinical applications of differe

BACKGROUND: The microsurgical transoral approach has traditionally been the preferred access for ventral decompression of the craniovertebral junction. This natural corridor, although direct, may be a challenging approach because of potential morbidities. The evolution of endoscopic methods in skull base surgeries has reduced morbidity and improved results. Endoscopic approaches are also being used for ventral decompression at the craniovertebral junction. Some technical modifications in this approach are described.
METHODS: An endoscope is inserted endonasally for a panoramic view of the ent

INTRODUCTION: Dural and anterior cranial base reconstruction is essential in the surgical resection of a craniofacial tumor that extends from the paranasal sinuses to the subdural space. Watertight reconstruction of vascularized tissue is essential to prevent postoperative liquorrhea, especially under conditions that prevent wound healing (e.g., postoperative irradiation therapy).
METHOD: We successfully treated two cases of olfactory neuroblastoma by endoscopic-assisted craniotomy with primary dural closure and anterior cranial base reconstruction using a multi-layered flap technique. Dural

Cerebrospinal fluid (CSF) leaks most commonly arise during or after skull base surgery, although they occasionally present spontaneously. Recent advances in the repair of CSF leaks have enabled endoscopic endonasal surgery to become the preferred option for management of skull base pathology. Small defects (<1 cm) can be repaired by multilayered free grafts. For large defects (>3 cm), pedicled vascular flaps are the repair method of choice, resulting in much lower rates of postoperative CSF leaks. The pedicled nasoseptal flap (NSF) constitutes the primary reconstructive option for the vast maj

BACKGROUND: Endoscopic endonasal approaches to the ventral skull base are categorized based on their orientation in coronal and sagittal planes. For all of these approaches, the sphenoid sinus is the starting point, and provides orientation to important vascular and neural structures. Surgical approaches to the petrous apex include 1) a medial approach, 2) a medial approach with internal carotid artery (ICA) lateralization, and 3) a transpterygoid infrapetrous approach (inferior to the petrous internal carotid artery). The choice of a surgical approach depends on the relationship of the lesion

INTRODUCTION: Cerebrospinal fluid (CSF) diverticula are uncommonly associated with anterior cranial base tumors. When they occur, they often complicate the surgical management of these tumors via a transsphenoidal approach. This report examines the effectiveness of transsphenoidal surgery in the treatment of these rare entities.
METHODS: We performed a review of four sellar and parasellar tumors (three pituitary adenomas and one chordoma) that contained an arachnoid diverticulum communicating with the intracranial subarachnoid space.
RESULTS: One of these tumors (a prolactinoma) was successf

BACKGROUND AND OBJECTIVES: Selection of the safest and most effective surgical approach for complex skull base lesions is a matter of great relevance and significant controversy. A combination of endoscopic endonasal and transcranial approaches for complex skull base pathology is underinvestigated. We aim to analyze our experience with combination of endonasal endoscopic and transcranial approaches, either simultaneously or as staged procedures.
METHODS: Thirty-two patients underwent a combined strategy. Patients were divided into simultaneous or staged group. Lesions were classified accordin

The nasal cavity has a robust vascular supply, and bleeding is a primary obstacle to the minimally invasive skull base technique. Venous bleeding, including the cavernous sinus, can be managed with various techniques using hemostatic materials and pressure. A thorough understanding of the skull base vascular anatomy is vital for avoiding injury to major arteries and having confidence to control venous bleeding to optimize the endoscopic view and tumor resection.

The skull base is an anatomically challenging location that is unique in that it requires three-dimensional considerations when formulating a reconstruction plan. Various methods have been described for repair including open, endoscopic and endoscopic-assisted approaches. Each of these can have complications that can arise either directly from access to the lesion or defect, or indirectly arising from patient characteristics, anesthetic considerations, as well as postoperative care. It is of utmost importance for the surgeon to select the appropriate technique and surgical access depending on

Solitary plasmacytoma (SP) of the skull is an uncommon clinical entity that is characterized by a localized proliferation of neoplastic monoclonal plasma cells. This case report describes a 50-year-old male that presented with a headache and an exophytic soft mass on the occiput. The diagnosis of SP was based on the pathological results and imaging examinations. The patient underwent occipital craniotomy, skull reconstruction and lower trapezius myocutaneous flap (LTMF) transplantation under general anaesthesia. The tumour was capsulized and extended to the subcutaneous and the subdural space

Carcinoma of the nasal cavity and paranasal sinuses occupies only 3% of head and neck malignant tumors in the West, but 10% in Japan. Frontal sinus carcinoma is much rarer, occupying less than 1% of carcinoma of the nasal cavity and paranasal sinuses. In Japan, only 38 cases have been reported for 40 years, from 1924 to 1964. Almost all patients died within one year because only facial resection was performed. We report a case of frontal sinus carcinoma (squamous cell carcinoma) invasive the to skull base for which we performed anterior skull base surgery and can confirm two years survival in

BACKGROUND: The craniofacial approach has greatly facilitated resections of tumours involving the base of the anterior cranial fossa when compared to either the transcranial or transfacial approach alone.
MATERIAL AND METHODS: This approach was used in 11 patients with malignant tumours localized to the ethmoid sinus, orbit and bone or soft tissue of the base of the anterior part of the skull. By combining a low frontal or frontolateral craniotomy with resection of the facial skull, en bloc resections were accomplished. A frontogaleal periostal flap or a muscle flap from the temporal muscle w
